Implementation and evaluation of Missouri's Medication First treatment approach for opioid use disorder in publicly-funded substance use treatment programs.

BACKGROUND: Leaders of Missouri's State Targeted Response to the opioid crisis (STR) grant have prioritized increasing access to treatment medications for opioid use disorder (MOUD) through a "Medication First" approach. This conceptual framework prioritizes rapid, sustained, low-barrier access to MOUD for optimal impact on decreased illicit drug use and mortality. Medication First principles and practices were facilitated through state-level structural changes and disseminated to participating community treatment programs via a multi-pronged, multi-disciplinary approach. In the first nine mon

Home-based Testing for SARS-CoV-2: Leveraging Prehospital Resources for Vulnerable Populations.

INTRODUCTION: Expanded testing for SARS-CoV-2 is critical to characterizing the extent of community spread of COVID-19 and to identifying infectious cohorts. Unfortunately, current facility-based testing compounds shortcomings in testing availability, neglecting those who are frail or physically unable to travel to a testing facility. METHODS: We developed an emergency medical service (EMS)-based home testing and evaluation program, leveraging existing community EMS resources. This program has kept vulnerable populations out of the emergency department, reduced cost, and improved access to ca

Care coordination for persons with mental health challenges: a scoping review.

BACKGROUND: Care coordination is commonly employed to assist individuals with mental health challenges [MHCs]. However, its implementation in mental health contexts is inconsistent. The term, 'care coordination' is also used interchangeably with integrated care and case management. This review aims to (1) consolidate the literature on how, and in what contexts, care coordination has been used to help adults with MHCs access care from more than one service and (2) describe the challenges and benefits of implementing care coordination for adults with MHCs from the perspective of service provider

Designed to Do Good: Key Findings on the Development and Operation of First Responder Deflection Programs.

Opioids and drug overdoses have claimed more than 750 000 American lives since the late 1990s. Overdoses since the mid-2010s have risen dramatically, due to synthetic opioids such as fentanyl whose lethality is disproportionately greater than street drugs of earlier decades. Until recently, most police and other first responders lacked resources beyond arrest to respond to overdoses and other nonviolent crimes. Largely in response to the opioid crisis and synthetic opioid-related overdoses, first responder deflection (FRD) has emerged as an alternative. First responder deflection has enabled f

Implementation phase of a multicentre prehospital telemedicine system to support paramedics: feasibility and possible limitations.

BACKGROUND: Legal regulations often limit the medical care that paramedics can provide. Telemedical solutions could overcome these limitations by remotely providing expert support. Therefore, a mobile telemedicine system to support paramedics was developed. During the implementation phase of this system in four German emergency medical services (EMS), the feasibility and possible limitations of this system were evaluated. METHODS: After obtaining ethical approval and providing a structured training program for all medical professionals, the system was implemented on three paramedic-staffed am

Interpreter use in Nordic paediatric oncology care - a multicentre cross-sectional study.

BACKGROUND AND PURPOSE: Tailored, language-appropriate information is fundamental for children and their families in paediatric oncology, influencing both the quality of care and clinical outcomes. Yet, for families who speak another language, interpreter services are not used to the extent necessary to ensure equitable and patient-safe communication. Knowledge about how healthcare professionals (HCPs) (registered nurses [RNs] and medical doctors [MDs]) experience and manage interpreter use is limited. To address this gap, this study aimed to explore the obstacles to interpreter use in Nordic

Using Chronic Care Management Visits to Reduce the Number of Frequent Emergency Department Visits: A Pilot Study.

Background Emergency department (ED) visits are increasing in number and cost and becoming a major way patients are interacting with the healthcare system. Patients who frequently visit the ED are deemed "super-utilizers" who visit for a variety of reasons, including, but not limited to, multiple chronic medical illnesses, homelessness, and substance use disorder, but fail to have an established long-term treatment plan. Methodology We enrolled our hospital's top 50 super-utilizing patients into the Chronic Care Management Program. These patients received monthly telehealth visits to discuss c

Characteristics of health care related to mental health and substance use disorders among Community Health Centre clients in Ontario: a population-based cohort study.

BACKGROUND: Community Health Centre (CHC) client populations with a history of mental illness or substance use disorders, or both, are not described well in the literature. We identified CHC clients in Ontario with a history of health care related to mental health or substance use disorders, or both, and describe their demographic characteristics, health system use and related health risks in comparison to other people in the province with similar diagnoses who did not use CHC services. METHODS: We conducted a population-based cohort study using provincial health administrative data among Ont

Mechanisms of resilience and coping to intersectional HIV prevention and drug-use stigma among people who inject drugs in rural Appalachian Ohio.

BACKGROUND: Intersectional stigma of drug-use and HIV hinders provision and utilization of HIV prevention services for people who inject drugs (PWID), particularly within rural US communities. Resilience and coping may be critical for PWID to counter pervasive stigma. METHODS: Between October 2021 and July 2022, 35 in-depth interviews were conducted in Appalachian Ohio to understand the intersection of drug-use and HIV prevention stigma and how resilience and coping processes are displayed, shared, and enacted. Interviews were audio-recorded and transcribed verbatim. Thematic analysis was con

The Shift in Delivery of Care from Hospital to Community Care Settings: What Changes in Terms of Healthcare Workers' Exposure to Violence.

Background: Despite the general interest in WPV against healthcare workers, there is evidence that this topic has comparatively fewer studies conducted in the context of community settings than in hospital settings. Given the current general transition of care from hospital to community, this study aims to analyze whether community settings present different characteristics in comparison with hospital settings on this topic in Italy. Methods: A retrospective observational study was conducted from 2020 to 2024 on aggressions reported by HCWs in hospitals and community settings belonging to a Lo

Customizing and fine-tuning opioid overdose machine learning models for community-specific dashboards.

Drug overdose is a leading cause of death in the United States. Targeted community interventions and allocation of resources for reducing overdose deaths require timely and actionable data to make a meaningful public health impact. We present our work on tuning geographic-specific machine learning forecast models to predict the number of opioid-involved overdoses involving Emergency Medical Services (EMS) in Kentucky, with a focus on optimizing covariates representing measurable opioid overdose risk and protective factors. We aggregated suspected opioid-involved overdoses in EMS data into mont

[Acute Stroke Care Systems:Pre-Hospital and In-Hospital Management].

Acute ischemic stroke care has evolved substantially since mechanical thrombectomy was established as an effective reperfusion therapy. As clinical outcomes are strongly dependent on the time to reperfusion, the optimization of pre-hospital and in-hospital stroke care systems has become increasingly important. In this review, the key components of an effective acute stroke care pathway are summarized, with a focus on pre-hospital triage using large-vessel occlusion scales, regional stroke networks, and institutional workflow optimization. Effective pre-hospital management includes standardized
