Homograft replacement of the aortic valve in cases of acute bacterial endocarditis is considered the ideal choice because of the resistance of the homograft to reinfection. We report a case of aortic and mitral valve bacterial endocarditis, secondary to Streptococcus viridans, with severe aortic and mitral valve regurgitation and hemodynamic instability requiring surgical interventions with the use of aortic and mitral valve homografts.

A 22-year-old man had severe pulmonary congestion and required mechanical ventilation. Endocarditis was suspected because a 2/6 systolic murmur was heard at the apex and because Osler nodes were present. Transthoracic and transesophageal echocardiography allowed correct diagnosis of papillary muscle rupture causing massive mitral regurgitation. To our knowledge, this is the first reported case of papillary muscle rupture caused by bacterial endocarditis diagnosed by transthoracic and transesophageal echocardiography.

By means of enzyme-linked immunosorbent assays (ELISAs) the antibody responses were detected in sera from normal persons (n = 275), patients with various types of bacteremia (n = 137), and patients with endocarditis (n = 28). Four ELISAs were evaluated. As antigens ultrasonic extracts of Staphylococcus aureus, Staphylococcus epidermidis, Enterococcus faecalis, and nonhemolytic streptococci were used. The antibody levels against S. aureus increased with age, while antibodies against the other bacteria showed no age variation. The different ELISA systems showed many serological cross-reactions.

The diagnosis of aortic ring abscess by cross-sectional echocardiography has not been reported. In this paper, a case of bacterial endocarditis of the aortic valve extending to the aortic ring and the mitral valve is reported. The role of cross-sectional echocardiography in defining the anatomic distortion of the aortic valve in bacterial endocarditis and its complications is emphasized. This study would indicate that early diagnosis of aortic ring abscess is feasible by cross-sectional echocardiography.

We report 11 cases of bacterial endocarditis with muscular and articular manifestations seen over the past ten years. There was arthralgia in 7 cases, vertebral pain in 7 cases and myalgia in 3 cases. Arthritis consisted of a monoarthritis of the ankle in 2 cases and oligoarthritis in 2 cases. BACKGROUND: Non-bacterial thrombotic endocarditis (NBTE) is an uncommon pathological situation, which involves the presence of bland, fibrin-platelet thrombi. It usually occurs at the endocardium of cardiac valves, in association with endothelial injury and a hypercoagulative state. A 24-weeks pregnant woman presented with bacterial endocarditis due to Streptococcus viridans and severe aortic and mitral valve incompetence. Progressive deterioration in haemodynamic function and the presence of systemic emboli necessitated the urgent performance of aortic and mitral valve replacement; both the patient and the fetus survived. Double valve replacement for infective endocarditis in pregnancy has not previously been reported.
