Advances in hip preservation after slipped capital femoral epiphysis.

The metaphyseal deformity, in even a mild slipped capital femoral epiphysis (SCFE), results in acetabular labral and cartilage injury. SCFE is the most extreme form of femoroacetabular impingement, and the mechanism of cartilage and labral injuries is similar. Recent surgical advances for treating femoroacetabular impingement have made it possible to consider applying these techniques to the surgical treatment of SCFE deformities to lessen the risk of secondary osteoarthritis. The goals of treatment are to arrest slip progression and restore normal proximal femoral anatomy, thereby decreasing

Application of finite element analysis in pre-operative planning for deformity correction of abnormal hip joints--a case series.

In experimental and clinical research, it is difficult to directly measure responses in the human body, such as contact pressure and stress in a joint, but finite element analysis (FEA) enables the examination of in vivo responses by contact analysis. Hence, FEA is useful for pre-operative planning prior to orthopaedic surgeries, in order to gain insight into which surgical options will result in the best outcome. The present study develops a numerical simulation technique based on FEA to predict the surgical outcomes of osteotomy methods for the treatment of slipped capital femoral epiphyses.

Valgus slipped capital femoral epiphysis without posterior displacement: two case reports.

Valgus slipped capital femoral epiphysis is a rare entity; approximately, 26 cases in 31 hips have been reported in the orthopedic literature since 1926. It has been argued that epiphyseal displacement occurs only in a posterior or posteromedial direction, and that the radiographic appearance of true valgus or lateral slippage is an optical illusion owing to the rotation of the femur. We present the cases of two sisters with bilateral lateral slipped capital femoral epiphysis without a posterior component of displacement. In these cases, the radiographs suggested that the direction of displace

[Slipped capital femoral epiphysis and overweight].

The slipped capital femoral epiphysis (SCFE) is defined as a nontraumatic epiphyseal separation and slipping of the proximal femoral epiphysis, which usually occurs during the adolescent growth spurt. Slipping of the upper femoral epiphysis may be classified as acute, chronic, and acute on chronic. The etiology of the disease is still not fully understood but seems to be multifactorial. The typical SCFE during puberty has to be differentiated from the atypical form, which may be associated with an endocrinological disorder or with its therapy. The typical SCFE may be found in male patients, wi

Can GnRH-agonist treatment cause slipped capital femoral epiphysis?

Slipped capital femoral epiphysis (SCFE) mainly occurs in pubertal children and is associated with delayed skeletal maturation, obesity, high growth velocity and tall stature. Furthermore, SCFE often coincides with endocrine disorders. This is the first report of a possible relationship between SCFE and GnRH agonist treatment: four patients developed SCFE during or shortly after treatment with GnRH agonists was stopped. We compared the clinical aspects of these patients with patients described in the literature who developed SCFE. Puberty started at the age of 3.3, 9.6, 0.0 and 5.6 years respe

Slipped capital femoral epiphysis following radiation and chemotherapy.

Patients who received radiation to the proximal femoral epiphysis and chemotherapy in childhood appear to have an increased risk of subsequently developing epiphyseolysis. Every effort should be made to exclude the proximal femoral epiphysis from the radiation port whenever possible. If the epiphyseal plate is widened and irregular and the adjacent bone is sclerotic, prophylactic pinning may be indicated even in the absence of a grossly visible slip.

[Modified bridle-plasty in recurrent luxation of the patella].

For local operative correction of slipping patella, we recommend a modified plasty of the patellar retinaculum apparatus. This simple operation can even be performed on adolescents with still open epiphyseal cartilages. The technique of our operation is described. The early results during the last 30 months are very good. None of our 20 patients noted a reluxation, and all have resumed their sports.

Pseudohypoparathyroidism presenting as renal osteodystrophy.

Pseudohypoparathyroidism (pseudo HPT) is the prototype of a group of diseases with end organ unresponsiveness to parathyroid hormone (PTH). Patients with the classic form of this disease have both renal and osseous resistance to PTH. We describe a rare variant of pseudo HPT with classic renal unresponsiveness to PTH but normal skeletal responsiveness to this hormone. The latter patients develop metabolic bone disease in response to depressed calcium and elevated PTH levels. Skeletal abnormalities are histologically and radiologically indistinguishable from renal osteodystrophy and these patien

Strength of the porcine proximal femoral epiphyseal plate: the effect of different loading directions and the role of the perichondrial fibrocartilaginous complex and epiphyseal tubercle - an experimental biomechanical study.

BACKGROUND: The high loads on adolescent athletes' musculoskeletal system are known to cause morphological and degenerative changes in bone, intervertebral discs and joints. It has been suggested that the cam deformity of the proximal femoral head originates from a subclinical slipped capital femoral epiphysis (SCFE) as a result of non-physiological loading. The perichondrial fibrocartilaginous complex (PFC) and the epiphyseal tubercle are believed to stabilise the proximal femoral epiphysis, but their role is still unclear. The aim of the present study was to develop an experimental, biomecha

Multiple metaphyseal involvement of a thymic lymphoma associated with hypercalcemia in a puppy.

A six-month-old, female German shepherd dog was presented because of depression, anorexia, vomiting, polyuria, and polydipsia of approximately 10 days' duration. The puppy was depressed, and pain could be elicited on palpation of both shoulders and hips. The most significant results of serum chemistries and hematology were hypercalcemia; increased blood urea nitrogen, creatinine, and alkaline phosphatase; and leukocytosis with neutrophilia. Thoracic radiographs revealed a large thymic mass, diagnosed on histological examination as a thymic lymphoma. Radiographs of the shoulders revealed destru

Relative contribution of epiphyseal tubercle and peripheral cupping to capital femoral epiphysis stability during daily activities.

Epiphyseal tubercle and peripheral cupping can influence the development of slipped capital femoral epiphysis (SCFE) and Cam morphology. During normal skeletal growth, epiphyseal tubercle shrinks while the peripheral cupping grows. We hypothesized that epiphyseal tubercle act as the primary stabilizer of the head-neck junction at early stages and this role is gradually transferred to epiphyseal cupping as the tubercle shrinks and cupping grows. From a cohort of 80 boys and girls (8-15 years old) with normal hips, CT scans of 5 subjects corresponding to minimum, 25th percentile, median, 75th pe

Subclinical slipped capital femoral epiphysis. Relationship to osteoarthrosis of the hip.

We examined the femora of 2665 adult human skeletons from an osteological collection to determine the prevalence of post-slip morphology termed femoral head-tilt deformity by Murray and pistol-grip deformity by Stulberg et al. The hypothesis was that primary osteoarthrosis of the hip is a secondary manifestation of a subclinical developmental disorder of the hip. The prevalence of post-slip morphology was 8 per cent (215 of 2665 skeletons). Severe osteoarthrosis was more prevalent in association with post-slip morphology (116 [38 per cent] of 306 hips) than in the matched controls (seventy-nin

Maturation factors in slipped capital femoral epiphysis.

Several maturation factors relative to growth and epiphyseal development were reviewed retrospectively in 191 patients with slipped capital femoral epiphysis, including bone age, height and weight, thyroid functions, sex hormone levels, and growth hormone levels. Seventy-one percent of 138 patients had weights above the 80th percentile. Active thyroid (T3) was significantly low in 25% of 80 patients studied. Testosterone levels were markedly depressed in 76% of 64 patients tested. In this same group, 87% had low growth hormone levels. The consistently low testosterone and growth hormone levels

[Slipped Capital Femoral Epiphysis].

Slipped capital femoral epiphysis is the most common hip disease during infancy and adolescence. The incidence of this disease increases continuously. The reason is the likewise increasing body mass index of these age groups. Early diagnostic and treatment are necessary to avoid complications and consequential damages. Primary aims of the treatment are to avoid a further slip of the epiphysis and to reduce the resulting cam-deformity by reposition or osteotomy. After the end of physiological remodelling the rest of cam-deformity should be treated before an early osteoarthritis after slipped ca

Disarticulation-Associated Femoral Head Separation in Clinically Normal Broilers: Histologic Documentation of Underlying and Predisposing Cartilage Abnormalities.

Routine and quantitative histologic studies on femoral head separation (FHS) associated with coxofemoral joint disarticulation at necropsy were conducted on 125 femoral heads collected from 21- to 50-day-old clinically normal broilers. The study compared groups demonstrating grossly detached femoral heads (DFHs) with those having attached femoral heads (AFHs). Marked microscopic lesions compatible with osteochondrosis (OCD) consistently occurred along the separation surface in the DFH population. The histologic changes consisted of cartilage degeneration and necrosis sometimes forming small cl

Surgical treatment of femoroacetabular impingement in patients with slipped capital femoral epiphysis: A review of current surgical techniques.

Slipped capital femoral epiphysis (SCFE) is relatively common in adolescents and results in a complex deformity of the hip that can lead to femoroacetabular impingement (FAI). FAI may be symptomatic and lead to the premature development of osteoarthritis (OA) of the hip. Current techniques for managing the deformity include arthroscopic femoral neck osteochondroplasty, an arthroscopically assisted limited anterior approach to the hip, surgical dislocation, and proximal femoral osteotomy. Although not a routine procedure to treat FAI secondary to SCFE deformity, peri-acetabular osteotomy has be

Arthroscopically assisted reduction of an unstable severe slipped capital femoral epiphysis: a case report.

Slipped capital femoral epiphysis (SCFE) is considered to be one of the most common disorders of the hip in children and adolescents. If left untreated, it may lead to progressive deformity, pain and decreased range of motion, and predisposes to early onset degenerative arthritis. Surgical treatment is advised, with in situ pinning across the physis being the gold standard for stable slips. Closed or open reduction can be considered in unstable or severe types. We report the arthroscopically assisted reduction of an unstable severe SCFE, followed by canulated screw fixation. A follow-up of 2.5
