In Parkinson's disease (PD), sudden unexpected sleep episodes and excessive daytime sleepiness (EDS) while driving and engaging in social activities are important problems. We conducted a multi-center study to clarify the prevalence and contributing factor of EDS and sleep episodes in Japanese patients with PD. We evaluated 188 patients with PD (85 men, 103 women) and 144 age-matched controls for sleepiness. EDS was defined as an Epworth sleepiness scale (ESS) score of >or=10. Using isolated salivary glands of the ixodid tick Amblyomma hebraeum Koch, we tested the effectiveness of butaclamol and sulpiride in blocking fluid secretion stimulated by a number of agonists. (+)-Butaclamol was a potent inhibitor of dopamine, N-methyldopamine and noradrenaline (Ki congruent to 30-60 nM), but was less effective on ergometrine (Ki congruent to 310 nM). Ergotamine is used to abort or prevent vascular headache. Valvular heart disease as an adverse effect of long-term ergotamine therapy has been rarely reported in the English literature, with only a few cases published. It is hypothesized that ergot-derived agents stimulate serotonergic receptors (5-HT2B), causing proliferation of myofibroblasts, with subsequent thickening of valve leaflets and chords. This case presentation aims at increasing clinicians' awareness of this potential complication.

A number of conditions can cause color and temperature changes in the extremities. These include physical and chemical insults (eg, frost-bite; ergot), vasculitis (eg, systemic lupus erythematosus), or conditions associated with vascular tone. This article reviews two conditions that are commonly seen in children (Raynaud syndrome and reflex sympathetic dystrophy) and a recently recognized condition associated with antiphospholipid antibody. The emphasis is on differences between the adult and childhood forms of these disorders.

A case of severe vasospasm producing ischemia associated with the administration of dihydroergotamine and heparin for prophylaxis of deep venous thrombosis is presented. We feel that the intensity of the vasospasm was related to the ergot in combination with the use of pressors and the presence of an operative site contiguous to the spastic artery. Based on our experience and a review of the world's literature, we feel that risk of vasospasm in trauma patients outweighs the potential additional benefit of this combination of drugs over heparin prophylaxis alone.

We report a primigravida woman with acute myocardial infarction caused by coronary artery spasm induced by intravenous administration of methyl ergometrine maleate just after delivery. Despite the frequent usage of ergot derivatives to promote uterine contractions, cardiac complications related to this drug are rare. Myocardial infarction may be overlooked in young women in the early postpartum period. Careful monitoring and prompt evaluation should be performed when this drug is administered for obstetrical purposes.

Pleuropulmonary fibrosis is a rare, but well-recognized adverse effect of ergot alkaloids. We report on four patients who developed pleural and/or pulmonary fibrosis during treatment with pergolide and give characteristics of 87 cases with one or more symptoms of serosal fibrosis. Retroperitoneal and pleuropulmonary fibrosis are serious conditions, which are often irreversible after drug withdrawal. Increased awareness may help to diagnose these complications at an earlier stage and to minimize any permanent damage to the patient.

OBJECTIVES: This paper reviews retroperitoneal fibrosis secondary to ergotamine and evaluated the current tools in its diagnosis.
METHODS/RESULTS: A well documented case of a young patient with retroperitoneal fibrosis resulting from chronic use of ergotic agents is presented. The fibrous plaque remitted after withdrawal of the causal agent.
CONCLUSIONS: The importance of contrast-enhanced CT in the diagnosis of retroperitoneal fibrosis is widely accepted. The usefulness of this technique in determining the extent of the lesion and for subsequent follow up is underscored.

The present report describes an unusual case of recurrent puerperal uterine inversion causing major postpartum hemorrhage. Blood replacement, oxytocin, and ergot therapy along with manual reduction failed to prevent immediate recurrence, but treatment with 15(S)-15-methyl prostaglandin F2 alpha (Prostin 15M) and uterine packing were successful. It is recommended that 15(S)-15-methyl prostaglandin F2 alpha be available in all obstetric suites for the management of similar emergencies.

Forbes Albright syndrome is a hyperprolactinemia syndrome linked to a pituitary tumor associated with galactorrhea and amenorrhea. Cabergoline, an ergot derivative, is its drug of choice. Here, we report the oral manifestations and management of a case of a 32-year-old female, diagnosed and treated with the same. The patient had an alarming increase in the incidence and progression of dental caries. Her diagnosis and management have been highlighted. This can have overbearing effects on the psychology and function of the individual, thus making early diagnosis and precise management important.
