Mild to moderate essential arterial hypertension affects almost 15 p. 100 of middle-aged adults in the general population. Although its individual vascular complications are few, it constitutes a problem of public health since a large number of patients implies a large number of complications in the population. Hypotensive therapy has proved capable of preventing mechanical complications of vascular rupture, but epidemiological studies have not yet demonstrated that it reduces the frequency of atherosclerosis of the coronary arteries and, probably, of other arteries at risk. A more detailed ph

Primary thrombocythemia may cause vascular thrombosis; it has been rarely involved in coronary atherosclerosis with myocardial infarction. We report three cases of renal arteries atherosclerosis occurring in association with primary thrombocythemia. These cases are three young women (20, 40 and 42 years old) with severe hypertension secondary to atherosclerosis with stenosis of renal arteries, one or both sided, and in association in one case with diffuse arterial stenosis. Systematic investigation revealed thrombocytosis with latent myeloproliferative syndrome of megacaryocytic colony. Thromb

Vascularization and morphology of carotid bodies in 100 necropsies of patients with essential hypertension in comparison with the 83 normotensives were studied. The statistically significant differences between the two groups were found. The carotid bodies of hypertensive subjects were vascularized by a lower number of arteries than those of the normotensives. This difference seems to be genetically determined. The mean combinated mass of carotid bodies of hypertensive subjects was higher than this of normotensive ones. The hyperplasia of the type I cells in carotid bodies of hypertensives was…

Despite recent appreciation of a role for calcium in clinical hypertension, evidence at present is conflicting. Thus, certain studies suggest increased calcium availability may be associated with increased levels of blood pressure, while others suggest that a calcium deficiency may contribute to the pathogenesis of hypertensive disease. Our own group has thus far demonstrated deviations of circulating levels of ionized calcium and of magnesium in essential hypertension, linked with concurrent deviations in the activity of the renal pressor hormone, renin. Furthermore, calcium metabolic indices…

In so-called essential hypertension, at least three different subsets of pathophysiologic findings can be identified: mild hypertension in nonobese juvenile subjects characterized by elevated cardiac output, normal peripheral resistance, increased sympathetic activity as measured by norepinephrine and plasma renin activity; hypertension in the elderly with a low cardiac output, often with left ventricular hypertrophy, elevated total peripheral resistance, nephrosclerosis contracted intravascular volume, and low plasma renin activity; and obese hypertensive patients with a high cardiac output, …

Regulation of aldosterone secretion by sodium chloride is impaired in a group of essential hypertensives: high-salt diet fails to suppress aldosterone in these patients despite low renin values. The mechanism of this impaired regulation of aldosterone has not been clarified so far. We tested the sensitivity of aldosterone secretion and blood pressure to A II in 20 normotensive controls (aged 20-60, MAP 92 +/- 3 mm Hg), in ten normotensives with one or two parents with hypertension, and in 21 patients with essential hypertension (aged 17-65, MAP 119 +/- 4 mm Hg). After a period of 6 days on hig…

Among vascular beds, that of the kidney is especially responsive to angiotensin II, perhaps a reflection of the fact that the renin-angioten … n phylogeny, with additional cardiovascular and endocrine responses arising later. In patients with essential hypertension, in whom renal vascular tone is commonly increased, converting enzyme inhibitors such as teprotide and captopril induce a potentiated acute renal vascular response: renal blood flow increases more than it does in normal subjects. The result is a con …

This report describes a double-blind, parallel, comparative study of trimazosin (+/- polythiazide) and propranolol (+/- polythiazide) in 130 patients with essential hypertension. Both treatment regimens were shown to be effective in achieving statistically significant sustained reduction in blood pressure. Propranolol alone was somewhat more effective, at the doses selected, than trimazosin alone, but the hypertension of nonresponders in each treatment group was effectively controlled by the addition of low doses of polythiazide. Trimazosin had no effect on heart rate, whereas propranolol sign…

The active sodium transport of white cells and red cells obtained from patients with essential hypertension was impaired. Incubating white cells from normotensive subjects in serum obtained from patients with essential hypertension caused an impairment in sodium transport in the white cells of normotensive subjects similar to that found in the white cells of hypertensive patients. The impairment in sodium transport was due to a fall in the ouabain-sensitive component of the total sodium efflux rate constant. These results show that the serum of patients with essential hypertension contains a s…

In order to evaluate the relationship between aldosterone status and the blood pressure-lowering effect of spironolactone, 38 patients with essential hypertension were treated with spironolactone (400 mg/day) during one week in hospital on a rigidly sodium-restricted diet. The degree of hyperaldosteronism was assessed by the aldosterone secretory rate after 5 days of salt loading (315 mmol Na+/day). The mean arterial pressure decreased 5.6% (range, -21 to +8%). When the patients were divided into subgroups with low and normal renin activity, there was no difference in the change in mean arteri…

Thirty patients suffering from essential hypertension were treated successively with placebo, hydralazine alone, and pindolol (Visken) combined with hydralazine. Hydralazine caused a statistically significant reduction in blood pressure and an increase in pulse rate as compared with the placebo: palpitations were an irritating side-effect. The combination of hydralazine and pindolol resulted in an additional fall in blood pressure (p less than 0.001) and the pulse rate decreased (2 p less than 0.001). The combined treatment also had a favourable effect on palpitations and angina pectoris.

OBJECTIVE: To compare 24 h ambulatory blood pressure changes between patients with renovascular hypertension and essential hypertension.
METHODS: The 24 h ambulatory blood pressure of patients with age and gender matched renovascular hypertension (RVH, n = 51) was compared with that of patients with essential hypertension (EH, n = 51).
RESULTS: The 24 h, daytime and nighttime systolic blood pressures (SBP), diastolic blood pressures (DBP) and pulse pressures (PP) in RVH were significantly higher than in EH (all P < 0.05), especially the nocturnal SBP (P < 0.05). The SBP and DBP loads in RVH …

The abnormal cellular contractile mechanisms causing the development of essential hypertension in humans and rats were investigated, primarily with the use of Ca2+ and Ca antagonists. Helical strips isolated from mesenteric arteries of spontaneously hypertensive rats (Aoki SHR) showed an increased Ca2+ -induced tension development in the presence of noradrenaline or a high potassium concentration. The tension development of the SHR strips was greatly inhibited by Ca antagonists. In the patients with essential hypertension, both elevated blood pressure and elevated total peripheral resistance w…

Gastroesophageal reflux disease (GERD) is a prevalent global health concern with a rising incidence. Various risk factors, including obesity, hiatal hernia, and smoking, contribute to its development. Recent research suggests associations between GERD and metabolic syndrome, cardiac diseases, and hypertension (HTN). Mechanisms linking GERD to HTN involve autonomic dysfunction, inflammatory states, and endothelial dysfunction. Furthermore, GERD medications such as proton-pump inhibitors may impact blood pressure regulation. Conversely, antihypertensive medications like beta-blockers and calcium
