Randomized, double-blinded, controlled non-inferiority trials evaluating the immunogenicity and safety of fractional doses of Yellow Fever vaccines in Kenya and Uganda.

Introduction: Yellow fever is endemic in specific regions of sub-Saharan Africa and the Americas, with recent epidemics occurring on both continents. The yellow fever vaccine is effective, affordable and safe, providing life-long immunity following a single dose vaccination. However, the vaccine production process is slow and cannot be readily scaled up during epidemics. This has led the World Health Organization (WHO) to recommend the use of fractional doses as a dose-sparing strategy during epidemics, but there are no randomized controlled trials of fractional yellow fever vaccine doses in A

Evaluation of the geriatric co-management for patients with fragility fractures of the proximal femur (Geriatric Fracture Centre (GFC) concept): protocol for a prospective multicentre cohort study.

INTRODUCTION: Treatment of fractures in the elderly population is a clinical challenge due partly to the presence of comorbidities. In a Geriatric Fracture Centre (GFC), patients are co-managed by a geriatrician in an attempt to improve clinical outcomes and reduce morbidity and mortality. Until now the beneficial effect of orthogeriatric co-management has not been definitively proven. The primary objective of this study is to determine the effect of GFC on predefined major adverse events related to a hip fracture compared to usual care centres (UCC). The secondary objectives include assessmen

Ethical principles for psychiatric administrators: the challenge of formularies.

Rising costs continue to plague healthcare and mental healthcare. The latest variation of managed care techniques has been to try to reduce medication costs by instituting formulary restrictions. To date, the results have been spotty at best, with a suggestion of a ballooning of more general mental healthcare costs, as well as possibly worsening of quality of care. New challenges are looming in terms of the results of the CATIE study comparing typical and atypical antipsychotic medication, as well as the beginning of Medicare D medication coverage. In the midst of the uncertainty and complexit

Moral injury in neurointerventional practice: multinational survey.

BACKGROUND: Neurointerventional practice involves making time-critical decisions and facing potentially devastating complications and clinical outcomes. Clinician distress in this field has traditionally been framed through the call burden, burnout, or the 'second victim' phenomenon, but these constructs may not fully capture ethical and identity-based distress. Moral injury offers a complementary framework, yet remains largely unexplored among neurointerventionalists. METHODS: We conducted an anonymous, multinational, cross-sectional survey of practicing neurointerventionalists and trainees.

Examining the immune response of participants receiving Modified Vaccinia Ankara vaccine in Africa (MpoxVax AFRIVAC): a prospective cohort study - protocol.

INTRODUCTION: Mpox remains an ongoing global health concern, driven by an ongoing clade I outbreak in Central Africa, which has caused substantial morbidity and mortality. Despite deployment of the Modified Vaccinia Ankara-Bavarian Nordic (MVA-BN) vaccine, data on the durability of vaccine-induced immune responses remain limited, particularly in African populations, women and people with HIV, highlighting the need for context-specific immunogenicity data. METHODS AND ANALYSIS: Mpox-Vax AFRIVAC is a prospective, multicentre observational cohort study conducted in Uganda, Tanzania and the Democ

Reduced-fluence photodynamic therapy versus placebo for central serous chorioretinopathy (REPLAY trial): rationale and study protocol of a pivotal double-blind randomised controlled trial.

INTRODUCTION: Chronic central serous chorioretinopathy (CSC) can cause progressive and permanent vision loss. Although photodynamic therapy (PDT) is a primary treatment option globally, it is not approved for CSC worldwide, limiting therapeutic access. The REPLAY trial is a phase III, investigator-initiated trial to evaluate the efficacy and safety of reduced-fluence PDT (rf-PDT) for chronic CSC to seek the first regulatory approval globally. METHODS AND ANALYSIS: This study comprises two cohorts. The 'untreated cohort' is a multicentre, randomised, placebo-controlled, double-masked trial inv

Structuring access and responsibility in transfusion refusal: a document analysis of relative non-transfusion policies in Japan.

Refusal of blood transfusion is often framed as a conflict between competent patients' refusal and clinicians' responsibility to prevent death or serious harm. In Japan, many hospitals have adopted relative non-transfusion policies, which seek to avoid transfusion where possible while reserving emergency transfusion when necessary. Existing debates have asked whether such policies are ethically or legally justified. This study examines how they structure access and responsibility as hospital policies. We conducted a document analysis of publicly available transfusion refusal policies from all

Why do so many in academic health care tend to assume that subspecialty surgical training begets higher quality health care than general pediatric surgical training?

BACKGROUND: The author spent all his attending level practice in general hospitals, resulting in early exposure to developing technologies, allowing early adaptation. ILLUSTRATIVE EXAMPLES: The adaptation of balloon dilation of arterial stenoses to treat esophageal strictures in children, and the use of MRI in anorectal malformations. RESULTS: Balloon dilatation of esopageal strictures and MRI assessment of anorectal anomalies became routine care. CHANGES IN ETHICAL LANDSCAPE: Both of these innovations were discussed with the Ethics Committee. Neither were subjected to further ethical overs

High-Intensity Statin versus Upfront Equivalent-Dose Combination of Moderate-Intensity Statin with Ezetimibe Following Acute Myocardial Infarction (ROSUZET-AMI): protocol of a multicentre, open-label, randomised non-inferiority trial.

INTRODUCTION: High-intensity statin therapy is recommended as a first-line strategy for lowering low-density lipoprotein cholesterol (LDL-C) levels in patients with acute myocardial infarction (AMI). A combination of moderate-intensity statin and ezetimibe at an equivalent dose to high-intensity statin may achieve similar LDL-C reduction with fewer side effects. This study evaluates the long-term efficacy and safety of this approach, initiated following AMI, compared with high-intensity statin monotherapy. METHODS AND ANALYSIS: The ROSUZET-AMI trial is a multicentre, prospective, open-label,

Improvement of rheolytic thrombectomy for acute deep vein thrombosis of the whole lower limb by primary popliteal vein thrombosis clearance: protocol for a prospective, multicenter, randomized controlled trial (the Reformation study).

INTRODUCTION: Pharmacomechanical thrombectomy (PMT) can be a useful treatment for restoring vein patency quickly, especially for extensive acute deep vein thrombosis (DVT). However, previous evidence failed to validate the effectiveness of PMT in reducing the incidence of post-thrombotic syndrome (PTS). To address this controversy, the reformation study aims to improve rheolytic thrombectomy for acute DVT of the lower limb through primary popliteal vein thrombosis clearance. METHOD AND ANALYSIS: Reformation is a prospective randomised multicentre trial. It has 160 patients in two groups: the

Understanding the impact of a residential housing programme for people living with HIV/AIDS: a realist evaluation protocol.

INTRODUCTION: Housing instability and homelessness are significant barriers to medical treatment for people living with HIV/AIDS. For these individuals, lack of stable housing and stigma is associated with insufficient access to care, poor adherence to medication and higher cost burdens to the healthcare system. This protocol reports on the efforts to evaluate Sanctum V.1.0, a hospice and transitional care home for adults with HIV/AIDS in Saskatoon, Saskatchewan, Canada. The current project was developed out of a need to identify how Sanctum V.1.0 produces varying programme outcomes to assist
