Periorbital (eyelid) dermatides.

Physicians in various specialties-and dermatologists in particular-frequently encounter various forms of inflammation of the eyelids and of the anterior surface of the eye. Distinguishing the cause of itchy, painful, red, edematous eyelids is often difficult. Because the uppermost layer of the eyelids is part of the skin that wraps the entire body, almost every skin disease in the textbook can affect the periorbital area as well. In this contribution, we focused on the most common such disorders that require special consideration, as a result of their special appearance, their challenging diag…

Solitary mastocytoma of the eyelid.

Mastocytosis is a heterogeneous group of diseases characterized by the proliferation and abnormal infiltration of mast cells in tissue. These collections of mast cells are called mastocytomas. Solitary mastocytomas are cutaneous lesions, most commonly involving the extremities and trunk. Rubbing a cutaneous mastocytoma lesion leads to the release of histamine and other chemical mediators causing the lesion to become elevated and urticarial, similar to an allergic reaction. We describe a rare location of a presumed solitary mastocytoma of the lower eyelid in a young otherwise healthy child.

Syringocystadenocarcinoma papilliferum of the eyelid.

An 86-year-old man presented with an ulcerated, painless right lower eyelid lesion of unknown duration. Excisional biopsy was performed to rule out suspected basal cell carcinoma. Pathologic evaluation demonstrated syringocystadenocarcinoma papilliferum. Three months after complete excision of the tumor, the patient remains well with no evidence of local recurrence or metastases. Review of the literature identified 20 cases of syringocystadenocarcinoma papilliferum, none of which presented in the skin of the ocular adnexa. The authors review the typical presentation, clinical course, and outco

Primary eyelid leiomyoma.

A 10-year-old white male presented with a recurrent painful nodule of the left lateral lower eyelid margin with associated madarosis. The initial pathologic diagnosis at an outside institution was reported as chalazion. The lesion was re-excised, and histopathologic examination with immunohistochemical evaluation confirmed the diagnosis of leiomyoma. This is the first reported case of primary leiomyoma originating in the eyelid.

[Suspicious eyelid spots].

Flat lesions of the eyelids are in most cases related to precancerous skin lesions or malignant tumors of the eyelids. Rarely, the following diagnoses can be established: alkaptonuria, Addison disease, argyrosis, or toxicity to systemic medications.

Steatocystoma simplex of the eyelid.

A 68-year-old woman presented with a nodular pretarsal tumor on her right upper eyelid that had slowly grown over 6 months. Excisional biopsy was performed through an eyelid crease approach, and histopathology evidenced the lesion to be steatocystoma simplex. Steatocystoma simplex is an uncommon benign cutaneous lesion thought to be a circumscribed malformation arising from the pilosebaceous duct junction, rarely reported to occur in the periorbital region.

The management of eyelid burns.

Eyelid involvement is common in facial burns. Ocular sequelae, including corneal ulceration, are usually preventable and secondary to the development of eyelid deformities, exposure keratopathy, and rarely, orbital compartment syndrome. Early ophthalmic review and prophylactic ocular lubrication is mandatory in burns involving the eyelids. Early surgical intervention, often requiring repeat procedures, is indicated if eyelid retraction causing corneal exposure occurs. Permanent visual impairment is rare with such prompt management. No binding aphorisms exist regarding the tissue used for eyeli…

Myiasis of the upper eyelid.

Myiasis rarely occurs in the eyelids. We report a 3-year-old girl with a myiasis of the upper eyelid after a trip to Central America. A 1.5 cm larva of a Dermatobia hominis was excised. Infestation with D. hominis should be suspected when a localized swelling with a central fistula in an eyelid is present in patients who have been to Central and South America.

A solitary fibrofolliculoma in the eyelid.

PURPOSE: To report the first case of a solitary eyelid fibrofolliculoma and to review the literature. METHODS: A 37-year-old female patient visited the outpatient department with a lesion in the right upper lid that had been growing steadily for a year. The patient had visited the local clinic, and under the diagnosis of chalazion had received incision and curettage twice, but the lesion had recurred. RESULTS: The 5x5 mm lesion was located near the upper lid margin. It was a red, hemispheric, smooth nodule, relatively solid to palpation and not painful. Excision and biopsy were performed, an

Eccrine hidrocystoma of the eyelid.

AIM: To report on the clinical features of eccrine hidrocystoma involving the eyelid. METHODS: Data on a series of consecutive patients with histopathologically confirmed diagnosis were reviewed. RESULTS: Among 34 patients, 69 tumours were identified. The mean age at diagnosis was 59 years (range 39-91 years). The majority (71%) of patients had only a single tumour. The tumours appeared as a small (median size=1 mm) clear cystic lesion with 87% located near the eyelid margin. CONCLUSIONS: The eccrine hidrocystoma is a benign small cystic tumour that characteristically occurs close to but do

Eyelid involvement in paracoccidioidomycosis.

PURPOSE: To describe a series of patients with eyelid lesions caused by paracoccidioidomycosis and to estimate the prevalence of eyelid involvement in this disease METHODS: The medical records of 439 patients with paracoccidioidomycosis admitted to our hospital from 1992 to 2002 were reviewed. Age, sex, and clinical forms of the disease were recorded. All patients with eyelid involvement had a skin biopsy positive for paracoccidioidomycosis and were examined by an ophthalmologist with oculoplastic training. RESULTS: Of 439 patients with acute, subacute, or chronic paracoccidioidomycosis, 11

[Etiology of eyelid edema].

Lidedema can be a common symptom of diseases of the orbital part of the face. One can differentiate lidedema in two groups, those that are related to an inflammation and those that are not related to an inflammation. Lidedema of the eyelid can also be secondary to a systemic disease. There is also another group that is common in the ageing population that consists of blepharochalasis and dermatochalasis where the skin becomes loose with the years and forms redundant folds.

Anthrax of the lower eyelid.

Because cutaneous anthrax, caused by Bacillus anthracis, is rare in developed countries, sporadic cases of anthrax may easily be overlooked because the diagnosis is often difficult to make. Lower eyelid involvement of anthrax is rare in clinical practice. A 40-year-old woman with a history of contact with animals was evaluated and treated for a left lower eyelid lesion with a black eschar on it. A diagnosis of cutaneous anthrax had been made and intravenous penicillin had been given by the department of infectious diseases before plastic surgery consultation. Any kind of surgical intervention
