From 1981 to 2000 sixty-one cases of human fascioliasis were detected in the province of Gipuzkoa (Spain), most of them during the period of parenchymal invasion. Four family outbreaks were recorded. Thirty, 15, 13 and three cases were detected during the four consecutive 5-year periods. The decrease in this infection over the years is probably related to the change in eating habits.

We present a case report of invasive hepatic fascioliasis. Ramified hypodense lesions with peripheral distribution were found in the CT-scan. These lesions disappeared after treatment with Bithionol. CT-scan plays an important role in the diagnosis of invasive fascioliasis whereas ultrasound is useful in the chronic phase. Both are helpful to evaluate response to treatment.

Fascioliasis is the parasitic infestation of the liver and biliary tract related to Fasciola hepatica. Bithionol is proposed as the treatment of choice for human fascioliasis without major side effects. However, the efficacy of bithionol has been evaluated in chronic but not in acute fascioliasis. The diagnosis of human fascioliasis was established in an 11 year old boy with prolonged fever by complement fixation and ELISA tests. Good clinical response followed the treatment with emetine. Four other family members were also affected with the disease and their treatment was equally effective. The origin of the infection was traced to the ingestion of infected watercress at a site 15 km away from Santiago.

The authors would record the fifth case of human fascioliasis in Korea, occurring in a 48 year old Korean man who lived in Munkyong, Kyongsangbuk-do. The diagnosis was based on the morphology and measurements of the eggs which were collected in necrotic debris from the tunnel-like ulcerations of the gallbladder wall which was surgically removed for a gall stone. Although we failed to find out the fluke in the specimen, there found an evidence of once presence of Fasciola in the inflamed wall of the gallbladder in this patient.

A case of hepatic fascioliasis is reported in which the diagnosis was made by direct visualization of the parasite by means of percutaneous transhepatic cholangiography. The peculiar radiological image should be included in the differential diagnosis of biliary tract pathology.

Concerning two cases of biliary distomiasis, the authors describe the sonographic patterns leading to the ultrasonic diagnosis of this parasite. Gallbladder distomiasis appears as an echogenic, oval structure, without acoustic shadow. It characteristically displays mobility and variable shape. The aspect of common bile duct distomiasis is reported for the first time. It is a nonshadowing linear structure and however non specific.

This case was a 19-year old Korean female who had a history of postprandial aggravation of intermittent abdominal pain of 3 weeks duration. She was presented with an abdominal mass in the right upper quadrant, for which an operation(right hemicolectomy) was performed. The prevalence and severity of fascioliasis was studied in 87 dairy cows from 36 irrigated farms in Victoria. Evidence of Fasciola hepatica infection was found in 85 (98%) cows representing all but one of the herds. The mean fluke burden (7 +/- 6) in cows treated with anthelmintics at drying off was lower (p < 0.05) than in untreated cows (24 +/- 7).
