An interleukin-1 genotype is associated with fatal outcome of meningococcal disease.

To determine whether known variants of the interleukin-1 (IL-1) and tumor necrosis factor (TNF) gene families are associated with severe manifestations of meningococcal disease, 276 white patients 4-70 years of age (median, 17 years) were genotyped. All patients had microbiologically proven Neisseria meningitidis infection; 39 died and 237 survived. A significant association (P<.001) was found between fatal outcome and genotype at IL1B (nucleotide position -511). Homozygous individuals, both for the common (1/1) and the rare (2/2) alleles, had increased odds ratios (ORs) for death, compared wi…

Lysosomal glycogen storage disease with normal acid maltase with early fatal outcome.

In a male infant who had cardiomyopathy, generalized muscle weakness and increased serum creatine kinase levels, his muscle biopsy revealed myopathic changes with tiny intracytoplasmic vacuoles containing PAS-positive material and high acid phosphatase activity, but had normal acid maltase activity biochemically. These findings were consistent with those seen in lysosomal glycogen storage disease with normal acid maltase (Danon disease). Sarcolemmal indentations commonly seen in this disease were missing, but a complement membrane attack complex, C5b-9 was positive along the surface membrane o

Primary intestinal lymphoma in Crohn's disease: minute tumor with a fatal outcome.

A 54-yr-old man with a 22-yr history of Crohn's disease was found to have a microscopic focus of immunoblastic lymphoma within a segment of small bowel resected to relieve intestinal obstruction. There was no other clinically evident disease. Thirty months later, he developed axillary adenopathy with recurrent lymphoma of the same immunophenotype (IgA lambda) and was given combination chemotherapy, with complete clinical response. Lymphoma recurred 6 months later in the axilla and progressed rapidly over the next 3 months, despite chemotherapy. He developed extensive mediastinal, mesenteric, a

Obstruction-strangulation of post-traumatic diaphragmatic hernia--delayed diagnosis and fatal outcome.

The radiological and clinical features of 9 cases of obstructed post-traumatic diaphragmatic hernia are reviewed. In none of these patients was the diagnosis of a diaphragmatic hernia considered before radiography, all cases being clinically diagnosed as 'acute abdomen', most frequently pancreatitis or perforated peptic ulcer. Even after radiographs had shown opacity at the left base in all 9 cases, together with a distended proximal bowel in 6, the correct diagnosis was made in only 4. Through lack of correct radiological diagnosis, there was a delay of at least 1 day between admission and op

A fatal maternal outcome after delayed delivery in a midtrimester abdominal pregnancy.

Abdominal pregnancy has been reported to have a maternal mortality rate of 0% to 18%. Presence of the condition is difficult to diagnose, but such features as failure to induce labor after an apparent intrauterine fetal death should make one suspect it. Ultrasound examination may be misleading.

Intravascular hemolytic transfusion reaction due to anti-Vw+Mia with fatal outcome.

Because of the increasing use of type, screen and hold protocols and minimal surgical blood order protocols in transfusion services, a number of patients are receiving un-cross-matched blood in elective situations. There are many low-frequency red blood cell antigens which are lacking on reagent cells used for antibody screening procedures, and alloantibodies directed against these antigens are relatively common and occur in either natural or immune forms. A case of an intravascular hemolytic transfusion reaction resulting in death is reported. The patients had a naturally occurring anti-Vw+Mi

[Analysis of fatal outcomes of meningococcal infection in adults].

Clinical characteristics and pathomorphological manifestations in 69 patients aged 18 to 86 years with a fatal outcome of the disease were examined in order to analyze the causes of severe course and high mortality of generalized forms meningococcal infection. It was found that the main clinical form was meningococcemia (90%), in the majority in combination with meningitis (52%). The fulminant course in 77% of patients with meningococcal sepsis manifested itself as a sudden onset, rapid development of typical symptoms. Hemorrhagic exanthema was detected on the first day of meningococcemia. The…

[Analysis of fatal outcomes issues of typhus fever].

The nature and frequency of complications in patients with typhus fever are of importance for its outcome. The causes of death were studied in 126 patients who had died from this disease. The immediate causes of death were as follows: infectious-toxic shock (31.7%), pneumonia (28.6%), and perforative peritonitis (26.2%). The pathomorphism of Payer's patches does not allow the stages of the disease to be identified. An immunohistochemical study established a drastic preponderance of granulomatous macrophages without proliferation markers with a simultaneous reduction in the count of T and B lym

Two cases of HELLP syndrome with fatal outcomes.

Severe preeclampsia and HELLP syndrome are still the leading causes of maternal and perinatal morbidity and mortality. We present two cases of pregnancies which were complicated by HELLP syndrome at 31 weeks of gestation and 25 weeks of gestation, the first one with maternal and the second with perinatal fatal outcomes. The aim of this report is to draw attention to the life-threatening complications that might occur in cases of preeclampisa and HELLP syndrome. The importance of early diagnosis with implications for management is also discussed.

Fatal outcome after immunotherapy in metastatic Merkel cell carcinoma of unknown primary cause.

Merkel cell carcinoma (MCC) is a rare form of skin cancer characterised by a high mortality rate. Despite limited evidence regarding the treatment of metastatic MCC, anti-programmed cell death protein 1 immunotherapy has emerged as a primary strategy for managing advanced stages of the disease. This report presents the case of a woman in her early 80s with stage IV MCC after consultation for declining general health, with the primary lesion remaining unidentified. Pembrolizumab therapy was initiated, which led to acute renal failure after the first infusion. The case highlights the high pathog

Atypical Intracerebral Hemorrhage With a Fatal Outcome Following the ChAdOx1 nCoV-19 Vaccine.

Intracerebral hemorrhage (ICH) is a severe form of stroke and is usually associated with established vascular risk factors. Neurological adverse events following ChAdOx1 nCoV-19 vaccination are uncommon, and isolated ICH occurring without thrombocytopenia or coagulation abnormalities is rarely observed. We report here the case of a 41-year-old woman, without cardiovascular risk factors or coagulation abnormalities and with no remarkable past medical history, admitted to our hospital with an afebrile altered level of consciousness that appeared 15 days after receiving her dose of ChAdOx1 nCoV-1

Prolonged mpox infection and fatal outcome in an HIV patient treated with tecovirimat.

We report a prolonged 59-day MPXV infection in a highly immunocompromised HIV-positive patient. We found progressive within-host viral diversification across lesions from different anatomical sites, including the emergence of a mutation previously associated with tecovirimat resistance. These findings highlight the need to investigate alternative therapeutic strategies for MPXV in severely immunocompromised patients.

Seronegative disseminated neosporosis with fatal outcome in an adult dog following prednisone therapy.

A 4-y-old, male English Mastiff with a history of inflammatory neuromuscular disease developed progressive tetraparesis, ataxia, and severe temporal muscle atrophy, culminating in non-ambulatory status and euthanasia. The autopsy revealed diffuse muscle atrophy and pale pulmonary nodules. Histologically, polyphasic myositis was evident, with apicomplexan organisms within skeletal muscle myofibers. Encephalomyelitis, myocarditis, and hepatitis with protozoal cysts and tachyzoites also were observed. Immunohistochemistry of brain tissue was strongly positive for Neospora caninum and Toxoplasma g

A Pheochromocytoma With Adrenocorticotropic Hormone Secretion and Subsequent Fatal Outcome: A Case Report.

Hypercortisolemia and hypercatecholaminemia in ectopic adrenocorticotropic hormone (ACTH)-secreting pheochromocytoma represent life-threatening conditions, particularly when diagnosis is delayed. In this case, a 66-year-old male patient with poorly controlled diabetes presented with severe constipation in the absence of the classic Cushingoid phenotype. Computed tomography revealed bilateral adrenal masses, and 123I-metaiodobenzylguanidine scintigraphy revealed radiotracer uptake in the left adrenal gland. Endocrine assessments confirmed elevated catecholamine, ACTH, and cortisol levels, indic

Spurious Presentation of Pulmonary Kaposi Sarcoma as Unresolved Pneumonia Led to Fatal Outcome.

Acquired immunodeficiency syndrome (AIDS)-associated Kaposi sarcoma (KS) is an angioproliferative neoplasia caused by infection with human herpesvirus 8 (HHV-8). It typically presents with mucocutaneous involvement, but it can be disseminated. Initial presentation with primarily pulmonary KS is rare. We present a case of a 32-year-old male with untreated human immunodeficiency virus (HIV) diagnosed 1 year before presentation who developed progressively worsening cough and shortness of breath for 6 months. He was hospitalized twice and treated for unresolved pneumonia in an outside hospital. Th

Unintentional direct intraventricular injection of gadolinium with fatal outcome: report of a case.

Gadolinium diethylenetriamine penta-acetic acid (Gd-DTPA) is the main contrast agent used in MRI, known for its good tolerance and rare toxicity. Even intrathecal injection of limited doses of Gadolinium can be performed in some indications. To our knowledge, only 3cases of accidental intraventricular injection of Gadolinium have been yet reported in the literature. We report the case of a 40-year-old male patient, who presented with headaches and vomiting. Brain MRI showed a right parietal abscess. The patient underwent emergent surgery for drainage of the septic collection. Postoperative MRI
