Megacolon for a giant faecaloma with unlucky outcome: case report and review of the literature.

We herein report a case of megacolon with fecaloma in an 83-year-old man who presented with constipation, no intestinal occlusion, and a left hydroureteronephrosis, with A.S.A. 4. The patient asymptomatic, was treated Primariely with laxatives. During the conservative therapy the patient presented an abrupt abdominal distension with a bowel obstruction and abdominal compartment syndrome. After the laparotopy and a Hartmann left colon resection the patient died for cardiovascular and metabolic complications. The aim of this report is to give a brief review of this entity and discuss the treatme

Barolith as a rare cause of acute appendicitis: a case report.

A barolith consists of inspissated barium associated with feces and is seen, rarely, after barium studies for imaging the gastrointestinal system. The barium used in such studies can enter the appendiceal lumen and, rarely, cause appendicitis by obliterating or narrowing the lumen of the appendix. The appendix fills with barium and the entire appendix is visualized in 80-90% of barium swallow or enema studies, and this is accepted as a reliable sign of a non-diseased appendix Post-examination retention of barium in the appendix is very common (90~95%), and 10% of the patients retain barium in

Faecaloma in ano.

INTRODUCTION: Faecal concretions or faecalomas ('stone of faeces') are symptomatic of many well-recognised colorectal conditions. Faecalomas are usually located in the colon or rectum and associated with disorders of colorectal transit. We describe an unusual case of 'faecaloma in ano' secondary to a chronic fissure in ano. A 67-year-old woman with a 10-year history of chronic constipation and intermittent anal discomfort presented with a painless perianal lump of similar duration. METHODS: The patient's case notes were reviewed and a literature search was carried out. RESULTS: Examination u

The role of biofeedback in the treatment of gastrointestinal disorders.

Biofeedback is a form of treatment that has no adverse effects and can be provided by physician extenders. The therapy relies on patients' a … ay be equally effective, and children whose fecal incontinence is associated with constipation plus fecal impaction do no better with biofeedback than medical management. For dyssynergic defecation, RCTs show that biofeedback combining pelvic floor muscle relaxation training, practice in defecating a water-filled balloon, and instruction in effective straining is …

Occult constipation masking as irritable bowel syndrome with predominant diarrhea in Bangladeshi children.

BACKGROUND: Occult constipation (OC) is defined by the absence of classical symptoms of constipation on initial clinical history, despite objective evidence of fecal retention, such as the presence of hard stool on digital rectal examination or fecal impaction identified on plain abdominal radiography. It is frequently undiagnosed in children and presents with a diverse array of symptoms, such as abdominal pain and frequent defecation with mucoid feces. These symptoms are strikingly similar to irritable bowel syndrome (IBS) with diarrhea (IBS-D). AIM: To verify the hypothesis that OC may pres…

Spiral colon bypass in a geriatric Vietnamese potbellied pig.

An 8-year-old potbellied pig was evaluated for anorexia, decreased fecal production, signs of depression, inappetence, and abdominal distension. During hospitalization, a tooth root impaction and abscess were diagnosed, and abdominal radiography revealed severely distended, gas-filled large and small intestines. Exploratory laparotomy revealed a stricture of the proximal centripetal loop of the spiral colon and megacolon of the proximal portion of the spiral colon and cecum. A side-to-side spiral colon anastomosis was performed to create a partial bypass of the spiral colon. The success of thi

Stercoral Ulcer Presenting in a Patient with Cauda Equina Syndrome Secondary to Postoperative Epidural Hematoma.

Chronic constipation can lead to fecal impaction in the large bowel, which can cause pressure necrosis followed by perforation, known as a stercoral ulcer. In extensive posterior thoracolumbar surgery, a long operation time, large blood loss, and perioperative narcotic use may aggravate constipation. Moreover, sacral root palsy due to cauda equina syndrome (CES) can lead to the deterioration of fecal impaction. This report describes the case of a 77-year-old woman with CES who presented with saddle anesthesia, neurogenic bladder, bowel incontinence, and paraplegia. Five days prior, she had und…

Cecal fecaloma: A rare cause of right lower quadrant pain.

Fecalomas are masses of hardened feces that become impacted and accumulate. They are much harder in consistency than fecal impaction due to the associated coprostasis and are typically found in the sigmoid colon or rectum. Cecal fecalomas are much rarer. To date, only five cases have been previously reported in the English-language literature. We present the CT appearance of a surgically confirmed cecal fecaloma in a patient who presented with right lower quadrant pain, nausea, and vomiting, mimicking an acute appendicitis.

Mesh erosion after laparoscopic posterior rectopexy: A rare complication.

Laparoscopic posterior mesh rectopexy (LPMR) is now an accepted surgical treatment for complete rectal prolapse. It is associated with complications such as partial mucosal prolapse, fecal impaction, constipation, and rarely recurrence. Erosion of the mesh into the rectum after LPMR is very rare. We report herein the case of 40-year-old man who presented with mesh erosion into the rectum and managed successfully by the laparoscopic excision of mesh. This is probably the first such case managed by the laparoscopic approach.

Evaluating the Patient with Left Lower Quadrant Abdominal Pain.

Left lower quadrant pain is a frequent indication for imaging in the emergency department. Most causes of pain originate from the colon, including diverticulitis, colitis, fecal impaction, and epiploic appendagitis. Left-sided urolithiasis and spontaneous hemorrhage in the retroperitoneum or rectus sheath are additional causes of pain. Computed tomography is the preferred imaging modality in the emergent setting for all of these pathologic conditions. Gynecologic, testicular, and neoplastic pathology may also cause left lower quadrant pain but are not discussed in this article.

Stercoral Colitis Complicated by Septic Shock Due to Clostridium perfringens and Clostridium ramosum Bacteremia.

Clostridium perfringens and Clostridium ramosum are anaerobic, Gram-positive bacilli commonly found in the gastrointestinal tracts, which can lead to life-threatening infections and sepsis under certain conditions. We present a case of a 60-year-old man with a history of chronic constipation who presented with hypotension, altered mental status, and respiratory distress and was found to have bacteremia due to Clostridium perfringens and Clostridium ramosum in the setting of stercoral colitis, a rare and serious condition caused by fecal impaction leading to colonic ischemia.

Bidirectional endoscopic and pharmacological approach for the treatment of severe right-sided colonic stool impaction.

A female patient in her 40s with scleroderma and Behçet's disease presented with dehydration, weight loss, fever and stool impaction. She failed aggressive laxatives and rectal enemas and was referred for colectomy. Abdominal X-ray revealed severe proximal colonic faecal impaction. We used a bidirectional approach to stimulate laxation composed of jejunal infusion of magnesium citrate using enteroscopy, gastrografin infusion into the right colon using colonoscopy, and intravenous neostigmine to stimulate peristalsis. This novel, three-pronged, outpatient approach using osmotic laxatives and pe

Faecal impaction in children aged 0-18 years: a systematic review and metanarrative analysis of definitions used.

BACKGROUND: Faecal impaction is the result of functional constipation in the majority of cases. Surprisingly, a uniform definition for the term faecal impaction is lacking, leading to heterogeneity across study results. AIM: To conduct a metanarrative systematic review to ascertain how trial studies define faecal impaction among children aged 0-18 years with functional constipation. METHODS: We conducted a systematic metanarrative review to uncover what criteria are used to define faecal impaction and to recommend directions for creating a globally accepted definition. Twice perforated stump appendicitis: a case report.

Stump appendicitis is an acute inflammation of the residual appendix and one of the rare complications after appendectomy. Stump appendicitis is an under-reported and poorly defined condition related to obstruction and inflammation of the residual appendix after an appendectomy, usually by a fecolith. It remains a clinical challenge because of delayed diagnosis and subsequent treatment with increased morbidity or mortality. Herein, we describe the case of a 42-year-old male who presented with periumbilical pain with progression to generalized abdominal pain and signs of peritonitis 14 months p

Physical characteristics of free-living sea turtles that had and had not ingested debris in Microregion of the Lakes, Brazil.

The ingestion of detritus by sea turtles results in high mortality and morbidity. The objective of this study was to describe the characteristics of free-living sea turtles that ingested anthropogenic inorganic detritus in comparison to those that did not. A total of 186 necropsy files were analyzed in marine turtles from the beaches of the Microregion dos Lagos, Rio de Janeiro. Among the turtles that ingested detritus, the mean turtle was female and cachectic, with a carapace length of 36,6 cm, detritus accumulated in the large intestine, and fecal compaction. It seems most likely that's low

[A giant fecalith complicated by acute urinary retention, hydronephrosis and acute obstructive pyelonephritis].

The article reports a rare case of a 30-year-old man with Hirschsprung's disease, who developed a giant fecalith in the rectum and sigmoid (weight 3.5 kg, the largest diameter 20 cm). The fecalith impaired urine flow by compressing urinary tract, thereby causing acute urinary retention and right-sided hydronephrosis with acute obstructive pyelonephritis. Removing fecalith resulted in the patient recovery and normal functioning of genitourinary system.
