Randomized Sham-Controlled Trial of Translumbosacral Neuromodulation Therapy for Fecal Incontinence.

BACKGROUND & AIMS: Fecal incontinence (FI) is a multifactorial disorder that lacks effective, disease-modifying treatments. We investigated the efficacy and safety of translumbosacral neuromodulation therapy (TNT), a novel noninvasive treatment targeting neuropathy, in a multicenter, double-blind, randomized sham-controlled trial. METHODS: FI patients (≥1 leakage/wk) were randomized to receive TNT (1 Hz) with either 2400 or 3600 repetitive magnetic stimulations or sham stimulations at 4 lumbosacral sites, weekly for 6 weeks. Daily FI episodes and bowel symptoms were assessed with prospective

Risk factors for fecal incontinence after intersphincteric resection for rectal cancer.

BACKGROUND: Restoration of bowel continuity is a major goal after surgical treatment of rectal cancer. Intersphincteric resection allows sphincter preservation in low rectal cancer but may have poor functional results, including frequent bowel movements, urgency, and incontinence. OBJECTIVE: This study aimed to evaluate long-term functional outcome after intersphincteric resection to identify factors predictive of good continence. DESIGN: Descriptive observational study. SETTING: Follow-up of surgery in tertiary care university hospital. PATIENTS: Eligible patients were without recurrence

Internal anal sphincter augmentation for fecal incontinence using injectable silicone biomaterial.

PURPOSE: A disrupted or weak internal anal sphincter can lead to passive fecal incontinence. This muscle is not amenable to direct surgical repair. Previous preliminary attempts to restore functional continuity have included a cutaneous flap to fill an anal canal defect, and injection therapy using polytetrafluoroethylene, collagen, or autologous fat. Urologists have also used injections of collagen or silicone to enhance bladder neck function. This pilot study aimed to assess the efficacy of single or multiple injections of the silicone-based product Bioplastique for the symptoms of passive f

High-resolution anorectal manometry in children with constipation-associated fecal incontinence.

UNLABELLED: Functional constipation with retentive fecal incontinence indicates disturbed anorectal sensation, sphincter function, and defecatory coordination. High-resolution anorectal manometry (HR-ARM) evaluates anorectal sensorimotor function; however, pediatric reference data remain limited, particularly for Middle Eastern populations. To assess anorectal sensorimotor functions in children suffering from constipation and retentive fecal incontinence, using HR-ARM. In addition to contextually compare findings with published pediatric data, and to explore associations with demographic and n

Translumbosacral Neuromodulation Therapy for Fecal Incontinence: A Randomized Frequency Response Trial.

INTRODUCTION: Treatments for fecal incontinence (FI) remain unsatisfactory because they do not remedy the underlying multifactorial dysfunction(s) including anorectal neuropathy. The aim of this study was to investigate the optimal dose frequency, clinical effects, and safety of a novel treatment, translumbosacral neuromodulation therapy (TNT), aimed at improving neuropathy. METHODS: Patients with FI were randomized to receive 6 sessions of weekly TNT treatments consisting of 600 repetitive magnetic stimulations over each of 2 lumbar and 2 sacral sites with either 1, 5, or 15 Hz frequency. Anal sphincteric neurogenic injury in asymptomatic nulliparous women and fecal incontinence.

While anal sphincter neurogenic injury documented by needle electromyography (EMG) has been implicated to cause fecal incontinence (FI), most studies have been uncontrolled. Normal values and the effects of age on anal sphincter motor unit potentials (MUP) are ill defined. The functional significance of anal sphincter neurogenic injury in FI is unclear. Anal pressures and EMG were assessed in 20 asymptomatic nulliparous women (age, 38 ± 5 yr; mean ± SE) and 20 women with FI (54 ± 3 yr). A computerized program quantified MUP duration and phases. These parameters and MUP recruitment were also se

Sacral nerve stimulation for fecal incontinence related to external sphincter atrophy.

BACKGROUND: Atrophy of the external anal sphincter, a pathologic muscle volume anomaly associated with fecal incontinence, has been shown to be a negative predictor of the outcome of surgery for defects of the external anal sphincter. It is unclear whether external anal sphincter atrophy also affects the outcome of sacral nerve stimulation for fecal incontinence. OBJECTIVE: Our aim was to assess the effectiveness of sacral nerve stimulation in patients with fecal incontinence and external anal sphincter atrophy and to determine whether severity of atrophy and concomitant presence of a sphinct

All Incontinence is Not Created Equal: Impact of Urinary and Fecal Incontinence on Quality of Life in Adults with Spina Bifida.

PURPOSE: We previously reported that the self-reported amount of urinary incontinence is the main predictor of lower health related quality of life in adults with spina bifida. In this study we sought to determine the impact of fecal incontinence on health related quality of life after correcting for urinary incontinence. MATERIALS AND METHODS: An international sample of adults with spina bifida was surveyed online in 2013 to 2014. We evaluated fecal incontinence in the last 4 weeks using clean intervals (less than 1 day, 1 to 6 days, 1 week or longer, or no fecal incontinence), amount (a lot

Pelvic floor architectural defects in female patients with urge fecal incontinence versus passive fecal leakage: a dynamic ultrasound study.

INTRODUCTION AND HYPOTHESIS: Fecal incontinence (FI) has two primary subtypes: urgency fecal incontinence (UFI) and passive fecal leakage (PFL). The pathophysiology underlying the subtypes is incompletely understood. OBJECTIVES: To compare the bowel habits, physical examinations and pelvic floor anatomical defects in patients with UFI-dominant FI versus patients with PFL-dominant FI. STUDY DESIGN: This is a retrospective cross-sectional study of female patients who presented with fecal incontinence symptoms to our tertiary urogynecology center. All subjects underwent a comprehensive history,

Sacral nerve stimulation for urinary urge incontinence, urgency-frequency, urinary retention, and fecal incontinence: an evidence-based analysis.

OBJECTIVE: The aim of this review was to assess the effectiveness, safety, and cost of sacral nerve stimulation (SNS) to treat urinary urge incontinence, urgency-frequency, urinary retention, and fecal incontinence. BACKGROUND: CONDITION AND TARGET POPULATION Urinary urge incontinence, urgency-frequency, urinary retention, and fecal incontinence are prevalent, yet rarely discussed, conditions. They are rarely discussed because patients may be uncomfortable disclosing their symptoms to a health professional or may be unaware that there are treatment options for these conditions. Briefly, urge
