Three hundred and five undisplaced subcapital femoral neck fractures managed by pinning in situ with Knowles' pins were evaluated to elucidate the role and effect of such treatment. The protocol of management and follow up, and evaluation, both radiographically and functionally, were set up prospectively. The duration from injury to management was 3.5 (1-14) days, the operation time was 22 (9-48) min and most of the patients were discharged without hospitalization. The follow-up period was 75 (28-136) months. The final results showed 282 (92.5 per cent) fractures united without complications (

Although the rate of fracture healing has been studied in adults, little such work has been done in children. The authors' objective was to develop staging criteria for assessing callus formation in fractures in children and to determine the relation between the age and sex of the patient and callus formation. They studied callus formation in healing fractures of the femoral diaphysis in 25 patients, 15 boys and 10 girls, ranging in age from birth to 14 years. The patients were selected on a random basis from children presenting with femoral fractures (and no other injuries) to a tertiary-care…

One hundred and forty-two consecutive patients with proximal femoral fractures were audited prospectively over a 1-year period. Mobility, age and sex were recorded along with timing of surgery, complications, 'will to live', length of admission, mortality, mobility and housing requirements on discharge. Operative procedures were performed mostly by intermediate surgical staff, on night-time emergency lists shared with other specialties. Patients were treated on a ward with nursing staff levels less than the minimum recommended by professional bodies. Mean hospital stay was 31 days. In-patient …

Twenty-two femoral shaft fractures in 20 patients aged 10-14 years with open physes treated with closed reamed intramedullary nailing were studied retrospectively. Follow-up averaged 26.7 months in 18 of 20 patients. Eleven additional patients with 11 femoral shaft fractures treated with casting and traction were included for comparison of hospitalization time, cost, and time to mobilization. All of the fractures treated with an intramedullary nail healed without malunion or leg length inequality, and there was no evidence of growth plate arrest. The patients treated with an intramedullary nai

A consecutive series is reported of 17 patients who underwent early surgical treatment for acetabular or unstable pelvic fractures associated with ipsilateral fractures of the femur. Treatment included external and internal fixation, and required careful consideration of the surgical approach and the positioning of the patient. The multiple injuries sustained by these patients required simultaneous procedures by several surgical teams. All the femoral fractures were internally fixed at the initial operation and eight patients had primary definitive treatment of all their other fractures as wel…

Thirty-three patients with ipsilateral intracapsular femoral neck and shaft fractures were treated with antegrade reamed intramedullary (IM) nails and cancellous screw fixation of the femoral neck. The shaft fractures were fixed prior to definitive neck stabilization. A "reversed" nail construct was used in 13 patients, a conventional interlocked nail was used in 6, and a reconstruction nail was used in the remaining 14. Thirty-one (94%) of the femoral shaft fractures healed primarily. In two patients, the shaft fracture failed to unite and was bone grafted and plated, respectively; the fractu

Of 104 supra-/intercondylar femur fractures in adults (AO/ASIF Classification types A 1-3 and C 1-3), who were treated by open reduction and internal fixation from 1975 to 1985 59 fractures in 57 patients could be traced and reviewed clinically and radiographically after a mean follow-up period of 5 years and 7 months (range: 2-11 years). Among these, 47 knees/patients without preexisting knee-joint pathology were analyzed for the development of degenerative arthritis. The incidence for grade 2 and 3 changes in the femoropatellar compartment were 23% for supracondylar fractures (A type) and 62

The role of external fixation in the management of femoral shaft fractures is reviewed based on a study of 24 femoral fractures managed by either an AO or a Wagner external fixator during 1983-1986. Indications included open fractures with soft-tissue injury (13 patients), comminuted shaft fractures (six patients), and unstable operative candidates (five patients). In 14 patients, external fixation was a temporary method (Group I), while in ten patients it was the definitive method of stabilization (Group II). Twenty-one patients achieved solid union, two developed a delayed union, and one had…

An unusual case of bilateral posterior fracture-dislocation of the hip (Pipkin Type IV) occurred in a 63-year-old man with Paget's disease of the pelvis. Other injuries included a displaced humeral shaft fracture and patellar ligament disruption. Bilateral cemented total hip arthroplasty was performed to avoid the need for prolonged immobilization. Postoperative low-dose irradiation was used because of the risk of heterotopic ossification.

We reviewed 12 patients with chronic drainage after intramedullary nailing of a femoral shaft fracture. The fractures tended to be the result of high-speed trauma and were frequently comminuted. Six were open fractures and six were closed injuries. All but one had been managed initially with the open nailing technique, exposing the fracture site. In six cases we left the nail in place until bone union occurred, an average of 33 months after injury, removing the hardware thereafter. The remaining six patients, each with bone sequestra at the fracture site, underwent nail removal, debridement of

Femoral fracture in patients with lower extremity amputations is relatively rare (reported incidence in 341 amputee patients, 2.35%). Lack of adequate support and balance, and osteoporosis on the amputation side, may explain the predisposition to fracture of the ipsilateral extremity. The prosthesis was in use at the time of the accident in only 3 of 9 cases studied, most falls occurring during transfer activity without a prosthesis. Proximal femoral fractures in below-knee amputee patients responded to open reduction with internal fixation and early ambulation. Others were treated with closed…

The primary surgical approach to the femoral neck fracture should be one with the lowest mortality and morbidity. Internal fixation appears to be the safe and logical approach for most of the reducible intracapsular fractures. Careful follow-up of these patients then is important. 85Sr scintimetry is a simple and accurate method in the early detection and evaluation of secondary complications in these patients. The 85Sr scintimetric pattern in the normal healing process was defined in relation to time interval after the fracture. At any time after 4 months following fracture, those hips destin

This paper reports a retrospective study of acute femoral neck fractures in 123 patients treated by primary prosthetic replacement. The average age in our patient population was 80-5 years. A Christiansen trunnion-bearing hip prosthesis (Christiansen, 1969 and 1974) was used in all patients except one. The prosthesis was cemented to the femoral shaft. The postoperative mortality rate was low, and the mortality rate was 8-7 per cent. The patients were examined between 6 months and 3 1/2 years after operation. Excellent or good results according to Stinchfield's hip assessment system (Stinchfiel

We reviewed thirty-eight cases, in thirty-six patients, of fracture of the femur distal to the base of the neck incurred in conjunction with total hip replacement. There were thirteen preoperative, eighteen intraoperative, and seven postoperative fractures. The cases were contributed by thirteen surgeons. We found that the preoperative fractures with pre-existing disease in the hip joint were effectively treated by primary total hip replacement using custom-made femoral components with long necks or long stems, or both. The intraoperative femoral fractures usually occurred while the surgeon wa…

Femoral head fractures, though rare, represent a significant challenge in orthopaedic practice due to their anatomical complexity, association with high-energy trauma, and potential for severe complications. These fractures often result from mechanisms such as motor vehicle accidents or falls from heights and are frequently accompanied by posterior hip dislocations. The primary aim of this systematic review was to evaluate the effectiveness of various management strategies for femoral head fractures, identify factors influencing outcomes, and highlight gaps in the current literature. This revi

OBJECTIVE: Femoral head fractures are rare, high-energy injuries with a relatively high complication rate. This study evaluated the femoral head fracture literature published since the systematic review in 2009 and determined changes in: classification systems, management of these injuries, and the associated adverse outcomes.
METHODS: A PubMed search from January 2009 to January 2025 was performed. Terms associated with femoral head fractures were entered. Meta-analysis was performed for the pooled proportion of patients who experienced adverse outcomes in the evaluated studies. Heterogeneit

Distal femoral fractures (DFF) cause significant morbidity and mortality comparable with neck of femur fractures. In view of this, NICE, National Institute of Clinical Excellence, has introduced a Best Practice Tariff (BPT) for management of distal femoral fractures in the elderly to improve morbidity and reduce regional variation of standards. Operative management includes application of external fixation, open reduction and internal fixation, or arthroplasty. While these are associated with significant morbidity, non-operative management may also be considered with immobilisation or traction…
