Accuracy of motor assessment in the diagnosis of fetal alcohol spectrum disorder.

BACKGROUND: To evaluate the accuracy of motor assessment tools listed in Fetal alcohol spectrum disorder: a guideline for diagnosis across the lifespan (Canadian Guideline) for the purpose of fetal alcohol spectrum disorder (FASD) diagnosis. Specifically, we aimed to determine: 1) diagnostic accuracy of motor assessment tools and subtests; 2) accuracy of multiple subtests versus total scores; and 3) accuracy of alternate cut-offs. METHODS: Cross-sectional diagnostic study of 63 children aged 6-17 years. Diagnostic accuracy and alternate cut-offs were calculated for the Movement Assessment Bat

Deferred diagnosis in children assessed for fetal alcohol spectrum disorder.

Early intervention for individuals with FASD is paramount, thus exploring factors that affect the diagnostic process is critical. This process can be complicated by challenges gathering background information, accurately evaluating higher-level cognitive skills across ages, and teasing apart the impact of life adversities from the effects of prenatal alcohol exposure. This study is a retrospective file review of 154 children (44% female; mean age 8.4 years, range 1.0 to 16.9) deferred at their first FASD assessment, and 51 (43% female; mean 9.9 years, range 2.7 to 17.2) who returned for a seco

The Economic Burden of Fetal Alcohol Spectrum Disorder in Canada in 2013.

AIM: To estimate the economic burden and cost attributable to Fetal Alcohol Spectrum Disorder (FASD) in Canada in 2013. METHODS: This cost-of-illness study examined the impact of FASD on the material welfare of the Canadian society in 2013 by analyzing the direct costs of resources expended on health care, law enforcement, children and youth in care, special education, supportive housing, long-term care, prevention and research, as well as the indirect costs of productivity losses of individuals with FASD due to their increased morbidity and premature mortality. RESULTS: The costs totaled ap

College students' knowledge about fetal alcohol spectrum disorder.

BACKGROUND: Fetal Alcohol Spectrum Disorders (FASDs) are the leading known preventable birth defects in North America. Knowledge surveys about FASD have been conducted with various health and allied healthcare providers and have proven useful in identifying gaps in knowledge and differences among provider groups to support prevention efforts. To date, no research has been conducted exploring FASD knowledge among college students. OBJECTIVE: This study explored FASD knowledge in a sample of college students, a group at particularly high risk for alcohol-exposed pregnancies. Findings are compar…

Caregivers' management of schooling for their children with fetal alcohol spectrum disorder.

In this article we describe a grounded theory study of how caregivers of school-aged children with fetal alcohol spectrum disorder (FASD) managed their children's schooling. We completed 30 interviews with 17 caregivers residing in a western Canadian province, as well as document analysis and 25 hours of participant observation. We used constant comparative analysis to construct our substantive theory: intertwining to fit in. The core variable is an iterative cycle caregivers used to resolve their main concerns: preventing their children from failing academically and in social interactions and

Sensory processing and ADHD in children with fetal alcohol spectrum disorder.

BACKGROUND: Sensory processing problems are prevalent in children who have fetal alcohol spectrum disorder. It is unclear to what degree these problems are distinct from attention deficits as measured during the diagnostic process in these children. PURPOSE: To understand sensory processing in these children, which may assist with early identification and intervention. METHOD: The relationship between attention and sensory processing was studied in a retrospective sample of 26 Canadian children diagnosed with fetal alcohol spectrum disorder. FINDINGS: A very low correlation (r = .02) betwee

Executive function and memory in children with Fetal Alcohol Spectrum Disorder.

A complex relation exists between memory and executive functioning (EF), particularly when learning and recalling multifaceted or extensive information ( Moscovitch & Winocur, 2002 ). A common instrument for evaluating this relationship is the Rey-Osterrieth Complex Figure (ROCF; Rey, 1941 ; Osterrieth, 1944 ). The ROCF has proved particularly useful in pediatric research; however, little research has been conducted among children with Fetal Alcohol Spectrum Disorders (FASD). Seventy children (35 FASD, 35 control), aged 6 to 12 years, were tested using the ROCF. All participants with FASD had …

Young children with fetal alcohol spectrum disorder--communication profiles.

The aim of the article is to describe the communication profiles of five young children with fetal alcohol spectrum disorder (FASD) from 4 to 58 months of age. A collective case-study design following a quantitative and descriptive approach was used to describe the communication profiles of the participants. The results are described according to the participants' case histories and a four-level early communication assessment framework. The significant findings were that all participants were in foster care, and presented with incomplete case histories, general developmental delays and delays

Brain diffusion abnormalities in children with fetal alcohol spectrum disorder.

BACKGROUND: Children with fetal alcohol spectrum disorder (FASD) have a variety of cognitive, behavioral, and neurological impairments, including structural brain damage. Despite the importance of white matter connections for proper brain function, little is known about how these connections, and the deep gray matter structures that act as relay stations, are affected in children with FASD. The purpose of this study was to use diffusion tensor imaging, an advanced magnetic resonance imaging technique, to examine microstructural differences of white and deep gray matter in children with FASD.

Development of Canadian screening tools for fetal alcohol spectrum disorder.

BACKGROUND: Fetal alcohol spectrum disorder (FASD) is the most common cause of neurobehavioural handicap in North America. Screening for FASD may facilitate diagnosis and hence management of these children. We present a variety of screening tools for the identification of children at risk for FASD. METHODS: We critically reviewed and evaluated published and practiced methods for their potential of screening suspected cases, their epidemiological characteristics (sensitivity, specificity, positive and negative predictive values) [Phase I], as well as their feasibility [Phase II]. RESULTS: The

Executive functioning and working memory in fetal alcohol spectrum disorder.

The goal of this report is to critically review research on executive functioning (EF) and working memory in individuals with fetal alcohol spectrum disorder (FASD). Individuals with FASD exhibit EF deficits in the areas of cognitive flexibility, planning and strategy use, verbal reasoning, some aspects of inhibition, set shifting, fluency, working memory, and, recently, on tests of emotion-related or hot EF. Some researchers have linked prenatal alcohol exposure to abnormalities in the development of the frontal cortex of affected individuals or animals. One common finding is that these EF de

Fetal alcohol spectrum disorder resources for educators: A scoping review.

Children with foetal alcohol spectrum disorder (FASD) can experience neurodevelopmental, physical, psychological and behavioural impairments that can result in a disrupted school experience. However, educators often have limited knowledge or experience in the identification and support of students with FASD, and there is a critical need for effective tools and resources to ensure students with FASD are supported in their ongoing learning and development. This scoping review aimed to identify and evaluate publicly available educator resources that aid in the identification, and support of stude

Fetal alcohol spectrum disorder predisposes to metabolic abnormalities in adulthood.

Prenatal alcohol exposure (PAE) affects at least 10% of newborns globally and leads to the development of fetal alcohol spectrum disorders (FASDs). Despite its high incidence, there is no consensus on the implications of PAE on metabolic disease risk in adults. Here, we describe a cohort of adults with FASDs that had an increased incidence of metabolic abnormalities, including type 2 diabetes, low HDL, high triglycerides, and female-specific overweight and obesity. Using a zebrafish model for PAE, we performed population studies to elucidate the metabolic disease seen in the clinical cohort. E…
