The effects of mechanical forces on fetal lung growth.

Abnormalities of fetal lung growth, especially pulmonary hypoplasia, are important causes of neonatal morbidity and mortality. For most clinical conditions associated with abnormal lung growth, studies in experimental animals indicate that these conditions affect the mechanical forces acting on the fetal lung. The major stimulus to growth in the fetal lung is stretch, which is due either to (1) to the constant distending pressure during periods when fetal breathing movements are absent, or (2) the intermittent, repetitive stretch induced by changes in thoracic shape during fetal breathing move

Fetal seizures: a case study.

BACKGROUND: A review of the literature and an extensive Medline search revealed that this is the first case report of fetal seizures diagnosed by ultrasound. CASE: A 23-year-old woman was found to have a fetus with bilateral choroid plexus cysts during a second-trimester ultrasound examination. Karyotype and alpha-fetoprotein level were normal. The cysts resolved, but idiopathic polyhydramnios was noted. Fetal testing beginning at 32 weeks revealed abnormal fetal behavior patterns on ultrasound and documented fetal seizures with rapid, repetitive limb movements noted on several occasions and

Fetal and maternal surveillance in high-risk pregnancy: tools, timing, and trends.

According to WHO statistics, stillbirths represent an incompletely elucidated, partially neglected problem, resulting in millions of pregnan … rent status of fetal and maternal surveillance in high-risk pregnancies and the role of identifying fetal movements associated with the risk of stillbirth. Recommendations for routine monitoring of fetal movement are warranted in high-risk pregnancies, particularly those with placental pathology or small for gestational age (SGA)/FGR (fetal growth restriction) a …

Prenatal diagnosis of fetal seizure: a case report.

A 35-yr-old woman carrying a 17-week-old fetus presented with right hydronephrosis and a single umbilical artery. Karyotyping was normal and targeted ultrasonography showed an otherwise normal fetus. After 28 weeks of gestation, the mother felt rapid, repetitive fetal movement and an ultrasound at 30 weeks of gestation revealed tonic clonic movements of the fetal trunk and extremities. At 36 weeks of gestation, an emergency repeat Cesarean section was performed because of a premature rupture of the membranes and a male infant weighing 4,295 gm was delivered. After birth, the infant continued t…

Magnetoencephalography as a Novel Tool for Prenatal Diagnosis of Fetal Seizures.

Fetal seizures represent one of the rarest phenomena in prenatal medicine, with fewer than 50 documented cases. Diagnosis has traditionally relied on maternal reports and ultrasound visualization of abnormal movements, lacking the neurophysiologic confirmation available in neonatal care. This case report presents the first neurophysiologic assessment of fetal seizures using fetal magnetoencephalography (fMEG), demonstrating a novel diagnostic approach for prenatal neurologic evaluation. A 22-year-old primigravida with controlled epilepsy presented with a fetus showing severe growth restriction

IoT assisted fetal health classification using mother optimization algorithm with deep learning approach on cardiotocogram data.

The adoption of the Internet of Things (IoT) for the application of smart health is an effective method for distributed and intelligent automated diagnosis systems. Fetal movement is a basic index of fetal well being. IoT based fetal health classification leverages IoT technology to remotely assess and monitor fetal well being in real time. Continuous data streams including uterine contractions, fetal heart rate (FHR), and movement patterns can be gathered and analyzed by incorporating sensors with cloud machine learning (ML) and computing algorithms. This allows prompt diagnosis of distress i…

Exome-First Approach in Fetal Akinesia Reveals Chromosome 1p36 Deletion Syndrome.

BACKGROUND: Fetal akinesia refers to a broad spectrum of disorders with reduced or absent fetal movements. There is no established approach for prenatal diagnosis of the cause of fetal akinesia. Chromosome 1p36 deletion syndrome is the most common subtelomeric terminal deletion syndrome, recognized postnatally from typical craniofacial features. However, the influence of chromosome 1p36 deletion on fetal movements remains unknown. CASE REPORT: A 32-week-old fetus with akinesia showed multiple abnormalities, including fetal growth restriction, congenital cardiac defects, and ventriculomegaly. …

Treatment options in fetomaternal hemorrhage: four case studies.

BACKGROUND: Significant fetomaternal hemorrhage (FMH) is an uncommon event that places the fetus at risk of severe morbidity and mortality. Symptoms and signs at presentation are subtle and, if promptly recognized, appropriate management may permit the fetus to escape serious injury. CASES: Four cases of significant FMH were diagnosed in the high-risk obstetrical unit at Mount Sinai Hospital, Toronto, during 2003. Three of the women complained of reduced fetal movements and were investigated initially with a non-stress test, a Kleihauer-Betke test, and ultrasound, including Doppler of the mid…

Arthrogryposis (multiple congenital contractures): diagnostic approach to etiology, classification, genetics, and general principles.

Arthrogryposis has been the term used to describe multiple congenital contractures for over a century. It is a descriptive term and present in over 400 specific conditions. Responsible gene abnormalities have been found for more than 150 specific types of arthrogryposis. Decreased fetal movement is present in all affected individuals which leads to a variety of secondary deformations. Decreased fetal movement (fetal akinesia) is associated with increased connective tissue around the immobilized joint, skin dimpling overlying the immobilized joint, disuse atrophy of the muscles that mobilize th…

Prenatal Diagnosis of a Segmental Small Bowel Volvulus with Threatened Premature Labor.

Fetal primary small bowel volvulus is extremely rare but represents a serious life-threatening condition needing emergency neonatal surgical management to avoid severe digestive consequences. We report a case of primary small bowel volvulus with meconium peritonitis prenatally diagnosed at 27 weeks and 4 days of gestation during threatened premature labor with reduced fetal movements. Ultrasound showed a small bowel mildly dilated with thickened and hyperechogenic intestinal wall, with a typical whirlpool configuration. Normal fetal development allowed continuation of pregnancy with ultrasound…

Diagnosis of Spontaneous Massive Fetomaternal Hemorrhage: A Case Report and Literature Review.

Fetomaternal hemorrhage (FMH) is the passage of fetal blood through the maternal circulatory system before or during delivery. Due to the nonspecificity of clinical manifestations, FMH is rarely diagnosed. When it occurs rapidly, it could have catastrophic consequences. Decreased fetal movements and a sinusoidal cardiotocographic (CTG) for more than 30 min are characteristic findings. This paper presents a case report of a 34-year-old woman at 38 + 4 weeks with reduced fetal movements and a sinusoidal CTG. An emergency cesarean section was performed, delivering a pale newborn with a hemoglobin…

Chorioamnionitis caused by Listeria monocytogenes: a case report of ultrasound features of fetal infection.

Maternal listeriosis is often associated with mild symptoms for the patient, but fetal infection can lead to severe adverse perinatal outcome. The most described antenatal symptoms are reduced fetal movements and an abnormal fetal heart rate trace. We present a case of fetal listeriosis suspected by ultrasound findings of fetal gastrointestinal compromise, neonatal diagnosis and outcome.

Ultrasound evaluation of fetal brain dysfunction based on behavioral patterns.

To identify fetuses at high risk of poor neurological outcomes using a novel ultrasound evaluation system. We assessed an ultrasound evaluation system based on our previous findings, consisting of screening for decreased or lack of fetal movements, abnormal patterns of fetal heart rate, congenital CNS malformations, polyhydramnios of unknown cause, and a "brief ultrasound evaluation" of fetal brain functions, including movement of extremities, breathing movements, ultradian rhythm, REM period, and NREM period. We then assessed the correlation between fetal brain functions and neurological outc…

Intrauterine fetal demise after negative oxytocin challenge tests.

The oxytocin challenge test was used to evaluate the fetoplacental unit in 572 patients over a 5-year period. Four fetuses died in utero within 7 days of a negative test. None had periodic fetal heart rate decelerations of any type. All 4 showed periodic accelerations of the fetal heart rate (FHR) in association with fetal movement. Two had baseline changes in the fetal heart rate during the oxytocin challenge test.
