National Public Health Institutes (NPHIs) are pivotal to health security, yet their performance could be constrained by complex, interlinked barriers. This paper examines these operational challenges, with a focus on the Eastern Mediterranean Region (EMR), and argues that strengthening NPHIs requires context-specific, cross-domain policy action rather than isolated technical fixes. Drawing on global and EMR-specific experiences, we undertook an evidence-informed narrative synthesis of selectively identified peer-reviewed literature, documented case studies, and regional reports, to map barrier

BACKGROUND: The Africa Health Security and Sovereignty (AHSS) Agenda of the Africa Centres for Disease Control and Prevention (Africa CDC) provides a continental framework for strengthening health security and sovereignty. As implementation begins, no document-based readiness assessment instrument exists to monitor progress against AHSS pillars and goals, limiting efforts to identify bottlenecks and target assistance.
OBJECTIVE(S): To develop and apply a document-based scorecard for assessing policy readiness for AHSS implementation across selected African countries.
METHODS: We conducted a

Water, sanitation, hygiene, waste management, and other environmental health services are critical for safe health systems, but global access is lacking. Adequate financing is a key barrier, and understanding resource allocation can help identify solutions in resource-limited contexts. We conducted a qualitative case examining resource allocation in rural Niger. Our objectives were to understand resource allocation processes, key actors and their roles, and contextual factors that influenced resource allocation. We interviewed thirty-three healthcare workers, community leaders, and government

BACKGROUND: Low- and middle-income countries (LMICs) face persistent challenges in achieving universal health coverage (UHC) due to limited financing and weak health systems, and the broader contribution of health taxes—despite their recognised public health and fiscal benefits—remains insufficiently synthesised.
OBJECTIVE: This scoping review examines the role of health taxes in advancing UHC in LMICs, including their effects across UHC dimensions, implementation challenges and policy implications.
ELIGIBILITY CRITERIA: Peer-reviewed and official grey literature published in English between

BACKGROUND: Childhood cancers remain a major cause of morbidity and mortality in low- and middle-income countries (LMICs), where nearly 90% of the world's children live. In 2018, the World Health Organization (WHO) and St Jude Children's Research Hospital launched the Global Initiative for Childhood Cancer (GICC) with the goal of increasing global survival to at least 60% by 2030. Achieving this requires a systematic approach to understanding country-level contexts to guide national planning.
METHODS: The Pediatric Oncology Facility Integrated Local Evaluation (PrOFILE) tool was implemented i

Increasing opiate use among women of reproductive age has led to a rise in adverse pregnancy outcomes, including neonatal abstinence syndrome (NAS). Recent studies have documented the increased incidence of NAS, but subsequent impact on the chain of organizations within the social service system remains unexplored. In this article, we begin to estimate the reach of this issue by assessing the labor costs of caring for NAS infants within the Massachusetts Department of Children and Families (MA DCF). Based on a process map of services, we modeled social service hours using encounter-level hospi

CONTEXT: The number of new vaccines recommended for children and adolescents has nearly doubled during the past 5 years, and the cost of fully vaccinating a child has increased dramatically in the past decade. Anecdotal reports from state policy makers and clinicians suggest that new gaps have arisen in financial coverage of vaccines for children who are underinsured (ie, have private insurance that does not cover all recommended vaccines). In 2000, approximately 14% of children were underinsured for vaccines in the United States.
OBJECTIVES: To describe variation among states in the provisio

Objectives. To examine the impacts of a government-implemented cash plus program on violence experiences and perpetration among Tanzanian adolescents. Methods. We used data from a cluster randomized controlled trial (n = 130 communities) conducted in the Mbeya and Iringa regions of Tanzania to isolate impacts of the "plus" components of the cash plus intervention. The panel sample comprised 904 adolescents aged 14 to 19 years living in households receiving a government cash transfer. We estimated intent-to-treat impacts on violence experiences, violence perpetration, and pathways of impact. Re

BACKGROUND: The purpose of this article was to describe methods that sexually transmitted disease (STD) programs can use to estimate the potential effects of changes in their budgets in terms of disease burden and direct medical costs.
METHODS: We proposed 2 distinct approaches to estimate the potential effect of changes in funding on subsequent STD burden, one based on an analysis of state-level STD prevention funding and gonorrhea case rates and one based on analyses of the effect of Disease Intervention Specialist (DIS) activities on gonorrhea case rates. We also illustrated how programs c

In the last 20 years, demographic development in Bulgaria has been characterized by population decline, a low crude birth rate, a low fertility rate, a high mortality rate and an ageing population. A stabilizing political situation since the early 2000s and an economic upsurge since the mid-2000s were important factors in the slight increase of the birth and fertility rates and the slight decrease in standardized death rates. In general, Bulgaria lags behind European Union (EU) averages in most mortality and morbidity indicators. Life expectancy at birth reached 73.3 years in 2008 with the mai

This policy-oriented commentary aims to interpret the drivers of maternal and neonatal mortality in Somalia through the Three Delays Model and the World Health Organization health-system building-block framework and to propose a Somalia-specific policy framework for strengthening the continuum of care. It draws on purposively selected peer-reviewed literature, national and international reports, and health-system evidence. Its distinctive contribution is to frame maternal and neonatal mortality as both a women's health and reproductive-health equity crisis and a priority for health-system refo

BACKGROUND: Social grants have been found to have an impact on health and wellbeing in multiple settings. Who receives the grant, however, has been the subject of discussion with regards to how the money is spent and who benefits from the grant.
METHODS: Using survey data from 1214 young women who were in the intervention arm and completed at least one annual visit in the HPTN 068 trial, and qualitative interview data from a subset of 38 participants, we examined spending of a cash transfer provided to young women conditioned on school attendance.
RESULTS: We found that spending was largely

BACKGROUND: It is unclear why rates of homelessness claims in England have risen since 2010. We used variations in rates across local authorities to test the impact of economic downturns and budget cuts.
METHODS: Using cross-area fixed effects models of data from 323 UK local authorities between 2004 and 2012, we evaluated associations of changes in statutory homelessness rates with economic activity (Gross Value Added per capita), unemployment, and local and central government expenditure.
RESULTS: Each 10% fall in economic activity was associated with an increase of 0.45 homelessness claim

On November 2, 2004 California voters approved Proposition 71, the California Stem Cell Research and Cures Act ("Act"), a $3 billion bond measure to advance stem cell research over the next ten years. The Act also created the California Institute for Regenerative Medicine (CIRM) and the Independent Citizens Oversight Committee (ICOC) which will govern the Institute, and make grants and loans for stem cell research and research facilities. This article will cover the history of the "Act", its potential impact on NIH's current stem cell program, and what are other states doing to compete with Ca

BACKGROUND: In the context of accelerating digital transformation, the alignment between digital transformation initiatives and sustainable supply chain finance has become an important issue for firms seeking to improve operational coordination and financial efficiency. This alignment, however, is influenced not only by technological adoption but also by organizational and contextual conditions such as governance structures, strategic orientation, and resource availability. Existing studies have not sufficiently examined how these contextual factors jointly shape the integration of digital tra

Begun in 2014, the Sustainable Financing Initiative (SFI) was dedicated to mobilizing domestic resources for the HIV response. Among its three programmatic pillars was a focus on ensuring financial protection for people living with HIV (PLHIV). SFI's activities were founded on a strong understanding of and alignment with partner government priorities, as well as costing and actuarial analyses, which allowed for the development of context-specific approaches for improving financial protection for PLHIV. SFI implemented financial protection activities in a total of nine countries; the five count
