Neuro-ophthalmologic manifestations of psychogenic disease.

From a neuro-ophthalmologic standpoint, five areas may be affected by psychogenic disease: (1) vision, including visual acuity and visual field; (2) ocular motility and alignment; (3) pupillary size and reactivity; (4) eyelid position and function; and (5) corneal and facial sensation. The physician faced with a patient complaining of decreased vision or some other disturbance related to the afferent or efferent visual systems for which there is no apparent biologic explanation has three responsibilities. First, the physician must ascertain that an organic disorder is not present. Second, the

Ocular prevalence versus ocular dominance.

UNLABELLED: Ocular dominance manifests itself in tests that contain stereo-objects with a disparity beyond Panum's area, e.g. in pointing a finger. These tests force subjects to decide in favour of one or the other eye. In contrast, ocular prevalence is determined using stereo-targets imaged within Panum's areas. These tests allow a graded quantification of the balance between the eyes. Here we present the computer-based Freiburg Ocular Prevalence Test in which stereo-disparate targets have to be aligned, and compare it with the Haase Stereo-balance Test that requires an estimation of the hori

Subjective vergence error at near during active head rotation.

There exists some controversy regarding the effects of active horizontal head rotation on vergence error. We used a conventional, Ogle-type, subjective fixation disparity apparatus to measure vergence error at near over a full range of horizontal head-rotation frequencies. Change in vergence error rarely exceeded 10 min arc. This finding demonstrates the adequacy of the vergence system to maintain relatively accurate bifixation at near and to allow for continuous maintenance of binocular sensory-motor integrative ability, even in the presence of system perturbations.

Ablation design in relation to spatial frequency, depth-of-focus, and age.

PURPOSE: To consider factors relevant to the design of wavefront-aberration-based customized ablations. METHODS: Review. RESULTS: Ablations that seek to eliminate all wavefront aberrations, both second- and higher-order, may not be optimal for all patients. This is particularly the case for presbyopes. Their main requirement will normally be for extended binocular depth-of-focus to yield adequate distance and near vision with good retinal contrast at lower spatial frequencies, rather than the highest levels of acuity and modulation transfer function at a single distance. For many presbyopes,

Imposed retinal image size changes--do they provide a cue to the sign of lens-induced defocus in chick?

BACKGROUND: Young chicks can adjust their eye growth to compensate for both imposed hyperopia and myopia (using negative and positive spectacle lenses); the rate of eye elongation increases in the former and slows in the latter case. This emmetropizing behavior implies that the eye can distinguish the sign and magnitude of defocus, although the identity of the cue(s) involved is unknown. As the spectacle lenses used in these studies generally introduce significant retinal image size differences that are in opposite directions for negative and positive lenses (minification vs. magnification), w

The variable clinical characteristics and course of early infantile esotropia.

A study of 75 patients with early infantile esotropia treated by one of the authors (RMR) between 1965 and 1980 revealed a broad range of clinical findings and treatment modalities. The study group included nine children born prematurely and 16 with neurologic impairment; the balance had no apparent illness other than strabismus. Only 21 patients had spontaneously alternating fixation; the rest required some form of occlusion therapy. Glasses were prescribed for 75% of the patients sometime during their management, and changes in refractive error were common. One or more operations were perfor

Congenital third nerve palsy with amblyopia of the contralateral eye.

A child with congenital third nerve palsy using the palsied eye for fixation is presented. Apparently, amblyopia developed in the secondarily deviating eye. This is the first case report of which we are aware with such a disturbance, ie, primary ocular motor palsy in one eye and sensory amblyopia in the other.

Abnormal optokinetic and perceptual span parameters in cerebellar-vestibular dysfunction and learning disabilities or dyslexia.

To measure the ocular fixation and sequential scanning dysfunction assumed responsible for the visual reading symptoms which characterize dyslexia or learning disabilities, an optokinetically based tracking method was devised. This method quantitatively demonstrated significantly reduced fixation, tracking, and perceptual or visual-span scores as well as "movement illusions" for 70 cerebellar-vestibular dysfunctioning persons with learning disabilities vs 70 controls. Such data tended to validate the hypothesis that cerebellar-vestibular-determined fixation and tracking mechanisms predispose d

Design and clinical evaluation of a virtual reality-based system for automated ocular deviation measurement.

Accurate measurement of ocular deviation is essential for strabismus assessment and treatment monitoring. Conventional methods such as the prism alternating cover test require trained examiners and substantial patient cooperation, which may introduce inter-examiner variability and limit measurement standardization. We developed a virtual reality (VR)-based system for automated horizontal ocular deviation measurement. The system simulates a prism alternating cover test in a controlled VR environment to guide fixation and record eye movement videos under standardized visual conditions. Recorded

[Pharmacologically-caused eye movement disorders--differential diagnosis and mechanisms of action].

The variety of drug induced patterns of pathological eye movements is reviewed with emphasis on hydantoins, barbiturates, carbamazepine, benzodiazepines, amitriptyline and alcohol. These different substances may be analogous or distinct in their site of action within the labyrinths, brain stem and cerebellum. Ocular oscillations are described in correlation to drug uptake or serum levels such as positional, downbeat, gaze evoked, periodic alternating nystagmus as well as saccadic pursuit, slowing down of saccades, alteration of VOR gain or suppression by fixation, internuclear and complete oph

Familial, congenital paralysis of horizontal gaze.

Eye movements were studied in a sister and brother with familial, congenital paralysis of horizontal gaze. Horizontal and vertical eye movements were recorded with DC electro-oculography and analyzed with a laboratory computer. All horizontal, conjugate eye movements were absent (saccades, pursuit, optokinetic nystagmus, vestibulo-ocular response, and visual-vestibular responses). Voluntary vergence eye movements were preserved and were used to track visual targets. An involuntary, horizontal, pendular nystagmus was found to represent disconjugate, smooth, vergence eye movements. Vertical sacc

An optimized workflow for transcriptomic analysis from archival paraformaldehyde-fixed retinal tissues collected by laser capture microdissection.

RNA sequencing (RNA-seq) coupled with laser capture microdissection (LCM) is a powerful tool for transcriptomic analysis in unfixed fresh-frozen tissues. Fixation of ocular tissues for immunohistochemistry commonly involves the use of paraformaldehyde (PFA) followed by embedding in Optimal Cutting Temperature (OCT) medium for long-term cryopreservation. However, the quality of RNA derived from such archival PFA-fixed/OCT-embedded samples is often compromised, limiting its suitability for transcriptomic studies. In this study, we aimed to develop a methodology to extract high-quality RNA from P

[The advantages and disadvantages of bifocal lenses].

The bifocal glasses present three main disadvantages: the jump of the image when the visual axis passes from the far vision glass to the reading segment, the prismatic effect on the near vision point that entails an apparent displacement of the fixed object as well as a degradation of the quality of its image, and the absence of intermediate vision in total presbyopia. Bifocals however keep their indications, even after the availability of the progressive glasses, by people accustomed with this kind of correction, and by others who are not supporting deformations in the lateral vision of the p

The influence of refractive correction upon disorders of vergence and accommodation.

Clinical care routinely includes prescription of lenses that compensate for the distance refractive error. Indeed, refractive correction is so commonly prescribed that we often neglect its potential effects on disorders of binocular vision. We report improvement of binocular function that resulted 1 or more months after prescription of an initial spectacle correction for 143 nonstrabismic patients who had a refractive error and either a vergence anomaly (28%), an accommodative anomaly (8%), or both (64%). Refractive correction was estimated objectively with an autorefractor and subjectively re

Comparison of iris fixation and anterior chamber intraocular lens implants.

The visual results and complications with iris fixation and anterior chamber lenses were compared by studying 289 consecutive implantations, most performed by intracapsular techniques. The visual results were equivalent with the two types of lens, but cystoid macular edema, corneal edema, uveitis, steroid-induced ocular hypertension and lens dislocation were more frequent among the 176 eyes with iris fixation lenses. The rates of other complications were approximately equal in the two groups of eyes. Among the patients with cystoid macular edema 21.2% of those with iris fixation lenses but non
