Fulminant infections of odontogenic origin.

The dentition is a common source of infection in the head and neck and most odontogenic infections respond uneventfully to proper dental therapy. Some more fulminant odontogenic infections can produce complications including airway obstruction, necrotizing fasciitis, and extension of the infection to the orbits, intracranial structures, and thorax. Six such cases treated by the authors are presented and recommendations for management including aggressive antimicrobial, therapy-based bacteriology and surgical drainage and debridement are made.

Bacteremia in childhood.

Review of the bacteriology records of a University Hospital pediatric service for a 30-month period revealed 42 patients with Hemophilus influenzae type b bacteremia and 30 patients with Streptococcus pneumoniae bacteremia, all under age 10. Eighty-eight percent of the Hemophilus bacteremias and 7% of the pneumococcal bacteremias occurred in children less than 2 years of age. Hemophilus bacteremia was seen mot frequently in the first year of life, in contrast to pneumococcal bacteremia which was seen evenly throughout the first and second years of life. In all but one of the Hemophilus infecti

Pediatric Odontogenic Infections.

Odontogenic infections are a broad group of head and neck conditions that arise from the teeth and surrounding periodontium. These largely preventable infections disproportionately affect members of ethnic and racial minorities and low-income/uninsured groups, and result in significant costs to our health care system. Left untreated, odontogenic infections can spread to deep spaces of the head and neck and can result in life-threatening complications. The mainstay of treatment includes timely treatment of the affected teeth. These infections are a global public health concern that could be dim

Periodontal disease epidemiology - learned and unlearned?

The notion of periodontal disease being the major cause of tooth loss among adults was rooted in the focal infection paradigm that dominated the first half of the 20th century. This paradigm was established largely by personal opinions, and it was not until the development of periodontal indices in the mid-1950s that periodontal epidemiology gained momentum. Unfortunately, the indices used suffered from a number of flaws, whereby the interpretation of the research results took the form of circular reasoning. It was under this paradigm that therapeutic and preventive intervention for periodonta…

Pustulosis palmaris et plantaris: the value of routine radiographic examinations in the detection of an infectious focus.

A three-year clinical material of 163 patients with pustulosis palmaris et plantaris is presented. The study was designed to assess the value of routine radiographic examinations of teeth, nasal sinuses and chest in the detection of an asymptomatic infectious focus. 66 patients underwent 151 examinations. Infectious lesions were found in the lungs of 0 of 53 examinations, inflammation in the nasal sinuses in 3 of 52 examinations (6%) , and abscesses at the teeth roots in 8 of 46 examinations (17%). The 3 patients with sinuitis had symptoms leading to the diagnosis. The root abscesses were all

Is periodontal disease a risk factor for coronary artery disease (CAD)?

Coronary artery disease (CAD) remains the principal cause of death in most developed countries, despite significant preventive and therapeutic advances. Current epidemiological data imply that recent reductions in the prevalence of this disease are unlikely to be sustained until those at high risk are more precisely targeted. Although dental (especially periodontal) infections have been recently identified as independent risk factors for CAD, current evidence is insufficient to justify treatment of such infections to arrest or reverse CAD or other systemic conditions (e.g., diabetes mellitus,

Periodontal medicine in the next millennium.

Dentistry and medicine have, to a great extent, been somewhat separated during the last 160 years, despite the fact that they have the same patients in common. We have now reached a period in our history when research is bringing medicine and dentistry closer together with data that are cogent to physicians and dentists and, most importantly, to the patient. A new paradigm is emerging with regard to susceptibility to periodontal disease, its etiology, and pathogenesis. Definite relationships between the oral disease and systemic conditions show that some risk factors are a concern for periodon

[Cost-effectiveness analysis of integrated control strategy of parasitic diseases in demonstration plots].

OBJECTIVE: To study the cost-effectiveness of different deworming schemes in demonstration plots of integrated control of parasitic diseases. METHODS: The cost-effectiveness was analyzed between mass drug administration and drug administration to focal population according to the different infection rates of parasites. RESULTS: In the demonstration plots of soil-born nematodes control, the costs for reducing one infected case and the cost for reducing 1% infection rate per ten thousand people in mass drug administration groups (Group 1 and Group 2) and drug administration to focal population
