Diffuse axonal injury and early intracranial sequelae in severe head injury.

The importance of diffuse axonal injury (DAI) and early intracranial sequelae was studied in 107 patients with diffuse and focal brain injuries. Comprehensive neuropathological study was also undertaken in 24 fatal patients. The mortality rate was clearly the highest in traumatic subarachnoid hemorrhage, followed by acute subdural hematoma, cerebral contusion with delayed hematoma formation, traumatic intracerebral hematoma, diffuse cerebral swelling, DAI with classical features, and finally nearly normal on computed tomographic scans. The mean flow velocities in the middle cerebral artery rec

Inner Ear Symptoms Are Prevalent in Patients with High Head Abbreviated Injury Scale Scores after Blunt Head Trauma.

INTRODUCTION: The purpose of this article was to determine the prevalence of inner ear symptoms in patients with blunt head trauma and to explore whether the severity of head trauma was associated with the incidence of such symptoms. METHODS: We performed a retrospective review of 56 patients admitted with blunt head trauma who underwent audiovestibular evaluation within 1 month after injury. Two scales were used to measure the severity of trauma; these were the Glasgow Coma Scale (GCS) and the Head Abbreviated Injury Scale (H-AIS). Patients with sensorineural-type hearing loss, or dizziness …

Admission Neurological Status, Radiological Mass Effect, and In-Hospital Mortality Among Surgically Treated Patients with Traumatic Acute Subdural Hematoma: A Single-Center Experience from Türkiye.

BACKGROUND: Traumatic acute subdural hematoma (ASDH) carries high morbidity and mortality despite surgery. We evaluated associations of admi … rwent 1000-resample bootstrap validation. RESULTS: In-hospital mortality was 44%. Higher admission Glasgow Coma Scale (GCS) score was associated with lower odds of mortality (adjusted OR: 0.72; 95% CI: 0.57-0.91), whereas greater midline shift (adjusted OR: 1.14; 95% CI: 1.04-1.25) and abnormal pupillary response (adjusted OR: 3.18; 95% CI: 1.09-9.28) were associat …

Predictors of early neurological deterioration and 6-month outcome in older adults with moderate traumatic brain injury.

INTRODUCTION: To analyze the epidemiologic characteristics of older adults with moderate traumatic brain injury (moderate TBI) and identify independent predictors for early neurological deterioration and unfavorable outcomes. METHODS: Clinical data of 93 consecutive older adults (age ≥65 years) with moderate TBI (Glasgow Coma Scale [GCS] 9-12) admitted to Tangdu Hospital (2015-2020) were retrospectively analyzed. Multivariable logistic regression was performed to determine factors associated with early neurological deterioration (GCS decrease ≥2 points) and unfavorable outcome (Extended Glasg…

Application of Serum NADPH Oxidase 2 Levels for Predicting 180-Day Clinical Outcomes Following Severe Traumatic Brain Injury: A Prospective Cohort Analysis.

OBJECTIVES: Nicotinamide adenine dinucleotide phosphate oxidase 2 (NOX2) affects oxidative response to acute brain injury. We set out to determine if there are connections between serum NOX2 levels, severity, and subsequent clinical outcomes of severe traumatic brain injury (sTBI). METHODS: In this prospective cohort study, serum NOX2 levels were measured in 123 patients and 123 controls. The Glasgow Coma Scale (GCS) scores and Rotterdam computed tomography (CT) classifications were applied for assessing injury severity. A poor prognosis was considered if the Glasgow Outcome Scale Extended (G…

Surgery for contralateral acute epidural hematoma following acute subdural hematoma evacuation: five new cases and a short literature review.

BACKGROUND: The occurrence of a contralateral acute epidural hematoma (AEDH) following removal of an acute subdural hematoma (ASDH) is a rare but nearly devastating postoperative complication. Here, we describe a series of five patients with contralateral AEDH and provide a review of the literature to elucidate the characteristics and improve management of these patients. METHODS: A total of 386 patients underwent ASDH evacuations in our hospital between August 2008 and July 2011. Five of these patients (1.3 %) developed AEDH that required surgery. Thirty-two additional patients were identifi

Effect of cisternostomy in managing cerebral swelling: A clinical study.

Traumatic brain injury (TBI) often leads to cerebral edema, resulting in elevated intracranial pressure (ICP) and poor neurological outcomes. Therefore, it is of interest to evaluate the efficacy of cisternostomy in managing cerebral edema, showing a significant reduction in ICP and improved neurological outcomes, including Glasgow Coma Scale (GCS) and Glasgow Outcome Scale (GOS) scores. The procedure offers a less invasive alternative to decompressive craniectomy with fewer complications and favorable recovery. Data from 200 patients suggest its potential for improving long-term functional re…

The median Glasgow Coma Scale of all patients on arrival at hospital was 6 (IQR: 3-10), and 53.3% of the patients who tested positive for gamma-hydroxybutyrate presented with a Glasgow Coma Scale of 3.
