Barriers and facilitators to accessing sexual and reproductive health services for people with severe mental illness: a systematic review.

PURPOSE: Despite increased prevalence of sexual and reproductive health problems among people with severe mental illness (SMI), uptake of sexual and reproductive healthcare in this group is poor. The reasons for this are unclear. Therefore, this review aimed to identify the barriers and facilitators to accessing sexual and reproductive health services from a service user perspective. METHODS: Three electronic databases were searched using key words for "sexual health" and "SMI". Data were screened and extracted by two independent reviewers. The Joanna Briggs Institute Critical Appraisal Tools

Facilitators and barriers for the implementation of a transmural fall-prevention care pathway for older adults in the emergency department.

BACKGROUND: Older adults at the emergency department (ED) with fall-related injuries are at risk of repeated falls. National guidelines state that the ED is responsible for initiating fall preventive care. A transmural fall-prevention care pathway (TFCP) at the ED can guide patients to tailored interventions. In this study, we investigated the facilitators and barriers for the implementation of the TFCP for older adults following a fall-related ED visit from the perspectives of patients and healthcare professionals. METHODS: In this qualitative study, we used semi-structured interviews with t

An interpretative phenomenological analysis of the lived experience of people with multimorbidity in low- and middle-income countries.

People living with multimorbidity (PLWMM) have multiple needs and require long-term personalised care, which necessitates an integrated people-centred approach to healthcare. However, people-centred care may risk being a buzzword in global health and cannot be achieved unless we consider and prioritise the lived experience of the people themselves. This study captures the lived experiences of PLWMM in low- and middle-income countries (LMICs) by exploring their perspectives, experiences, and aspirations. We analysed 50 semi-structured interview responses from 10 LMICs across three regions-South

Evaluation of cost-effective strategies for rabies post-exposure vaccination in low-income countries.

BACKGROUND: Prompt post-exposure prophylaxis (PEP) is essential in preventing the fatal onset of disease in persons exposed to rabies. Unfortunately, life-saving rabies vaccines and biologicals are often neither accessible nor affordable, particularly to the poorest sectors of society who are most at risk and upon whom the largest burden of rabies falls. Increasing accessibility, reducing costs and preventing delays in delivery of PEP should therefore be prioritized. METHODOLOGY/PRINCIPAL FINDINGS: We analyzed different PEP vaccination regimens and evaluated their relative costs and benefits

'Stressed, uncomfortable, vulnerable, neglected': a qualitative study of the psychological and social impact of the COVID-19 pandemic on UK frontline keyworkers.

OBJECTIVES: Non-healthcare keyworkers face distinct occupational vulnerabilities that have received little consideration within broader debates about 'essential' work and psychological distress during the COVID-19 pandemic. The aim of this study was therefore to explore the impact of the pandemic on the working lives and mental health and well-being of non-healthcare keyworkers in the UK. DESIGN: In-depth, qualitative interviews, analysed using a reflexive thematic analysis. SETTING: Telephone or video call interviews, conducted in the UK between September 2020 and January 2021. PARTICIPANT

Video teleconsultation service: who is needed to do what, to get it implemented in daily care?

INTRODUCTION: In telemedicine, technology is used to deliver services. Because of this, it is expected that various actors other than those involved in traditional care are involved in and need to cooperate, to deliver these services. The aim of this study was to establish a clear understanding of these actors and their roles and interrelationships in the delivery of telemedicine. A video teleconsultation service is used as a study case. METHODS: A business modeling approach as described in the Freeband Business Blueprint Method was used. The method brings together the four domains that make

Globalization of healthcare: case management in a 21st-century world.

PURPOSE: This article explains the current state of the global healthcare market with respect to international medical travel (medical tourism) and worldwide provider sourcing. Emphasis is placed on the traditional twin pillars of oversight: program accreditation and branding affiliation. These are discussed for their main strength, which is their ability to operate at a system-strata level. This strength also represents a primary weakness from the international patient's perspective, which is the functional gap between systemic oversight and bedside surveillance. International case management

Financial and healthcare tradeoffs associated with cystic fibrosis care in the United States: A cross-sectional study.

BACKGROUND: Financial challenges are common among people with cystic fibrosis (PwCF). We explored the financial and healthcare tradeoffs that PwCF and their care partners make to meet healthcare needs, and how these tradeoffs vary across people from different income levels. METHODS: Adults (≥18 years) with cystic fibrosis and parents/guardians/spouses of PwCF completed the Cystic fibrosis Outcomes, Social needs and Tradeoffs due to Coverage and Financial burden (COST-CF) survey between February and May 2024. We compared financial and healthcare tradeoffs and desired support over the prior 12

Womens' experience on barriers and facilitators for implementation with Kangaroo Mother Care at Mbarara Regional Referral Hospital, Uganda: a qualitative study.

BACKGROUND: Kangaroo Mother Care (KMC) is a proven cost-effective intervention for preterm and low‑birth‑weight infants, to provide warmth and secondary health effects. In Uganda, despite a high burden of prematurity, resource constraints limit the delivery of cost-effective interventions. OBJECTIVE(S): This study aimed to identify barriers and enablers for implementing KMC, explore mothers' experience of KMC and apply the i-PARIHS framework to understand why implementation was, or was not, successful at Mbarara Regional Referral Hospital. METHODS: This study was conducted in a rural referra

Leading improvement at scale in healthcare: Sir John Oldham.

BACKGROUND: When I became senior partner at my general practice I undertook a part time MBA at Manchester Business School in the late 90s to learn the things medical school hadn't taught me; organizational design and behaviour, finance, strategic management and human resources (as it was then called). My dissertation was on quality methods in other industries that could be applied in healthcare. JOURNEY: We tried out total quality management methods in my practice and began to demonstrably improve care, services and the use of resources. I titled this in presentations in the early noughties t

Effects of health insurance on out-of-pocket payments: a coarsened exact matching analysis using Kenya's 2022 demographic and health survey.

OBJECTIVES: Out-of-pocket payments remain a major source of healthcare financing in Kenya, yet it is unclear whether health insurance coverage is associated with lower out-of-pocket spending due to non-random enrolment. This study uses coarsened exact matching (CEM) to compare out-of-pocket costs (OOPCs) between insured and uninsured individuals. DESIGN, SETTING AND PARTICIPANTS: This study used cross-sectional data from the 2022 Kenya Demographic and Health Survey which covered 80 780 household members. Participants with missing or incomplete information on health insurance status and OOPC w…

A Clinical and Operational Preparedness Framework for Dengue in Colombia: Integrating Surveillance, Clinical Response, Prevention, and Health System Planning.

Dengue is an arboviral disease of major public health relevance in Colombia, where recurrent endemic-epidemic transmission intersects with climatic variability, urbanization, population mobility, co-circulation of viral serotypes and lineages, seasonal healthcare pressure, and substantial direct and indirect costs. Although dengue prevention and response in Colombia include epidemiological surveillance, clinical diagnosis, management of warning signs, vector-control activities, outbreak response, and assessment of preventive interventions, these components may be implemented unevenly across te

Marginalized two part model for analyzing multilevel semicontinuous medical costs in Iranian households.

Medical costs (MCs) represent a significant burden on household finances and often lead to economic challenges. This study analyzed the data of 8,993 Iranian households from 2021 collected by the Iranian Statistical Center. Using a marginalized two-part model (MTP) with lognormal and gamma distributions, the relationship between MCs and factors such as age, gender, education, and household characteristics was examined. A two-level structure was applied to account for heterogeneity across provinces, with analyzes performed using R software. The mean annual MC was $180 with high variability (SD
