Electrophysiological alterations in heart failure.

Electrophysiological defects are common and diverse features of heart failure. Cardiac arrhythmias may be precipitated by mechanical factors, catecholamine release, ischaemia drug therapy or drug-induced hypokalaemia. There is a high prevalence of ventricular and supraventricular arrhythmias, and chronic atrial fibrillation is also common. Ventricular arrhythmias are characterized by multiform, bigeminal and paired beats. There is also a high incidence of non-sustained ventricular tachycardia. These abnormalities are particularly prevalent amongst patients in New York Heart Association (NYHA) …

Epidemiology of congestive heart failure.

The incidence of congestive heart failure (CHF) is influenced by a variety of factors, including availability of medical care, socioeconomic status, geography, nutrition and race. The etiology of CHF in China, England, Botswana and Sweden will be examined and compared with prospective findings from Boston and Detroit. Because the therapy used in the treatment of CHF varies with the underlying causes, which may be as diverse as rheumatic fever, systemic hypertension and viral infection, the importance of fully determining its pathogenesis is emphasized.

Calcium antagonists in heart failure.

Clinical experience with calcium antagonists in congestive heart failure has, to date, been mainly restricted to the use of nifedipine but there is either no or only a limited extent of information available on diltiazem and verapamil. In patients with acute and chronic congestive heart failure, single-dose administration of nifedipine was seen to lead to a decrease in systemic vascular resistance, left ventricular filling pressure and ventricular volumes as well as to an increase in stroke volume, ejection fraction and mean velocity of circumferential fiber shortening. These favorable effects…

Bumetanide in congestive heart failure.

Bumetanide, a new diuretic exerting its major effect on the ascending limb of the loop of Henle, was evaluated in 20 patients with congestive heart failure. Dosage ranged from 1 mg to 3 mg daily depending on the patient's condition. The results after 3 and 8 days' treatment showed that bumetanide caused a significant diuresis, an increased excretion of sodium, potassium and chloride, and a comparable fall in the serum levels of these electrolytes. Changes in electrolyte levels were directly related to the dose of the drug. The resultant hypochloraemia was accompanied by a slight metabolic alka…

Heart failure and neonatal hypocalcaemia.

Six infants had congestive heart failure for which no cause could be found apart from a low serum calcium. All except one recovered completely and had normal chest x-ray appearances and E.C.G.s on discharge from hospital. Necropsy on the infant who died shoed engorgement and dilation of the right side of the heart and a small ductus but no other abnormalities. It is suggested that hypocalcaemia should be considered as a possible cause of heart failure in infants, especially when oedema is prominent.

Associations of ANGPTL3/4/8 proteins and complexes with heart failure and heart failure mortality.

BACKGROUND: Angiopoietin-like 3/4/8 (ANGPTL3/4/8) proteins play critical roles in modulating lipid metabolism through calorically sensitive and tissue-specific regulation of lipoprotein lipase (LPL) activity via formation of ANGPTL3/8 and ANGPTL4/8 complexes. ANGPTL3/4/8 proteins are also associated with cardiovascular risk. Circulating levels of ANGPTL3, ANGPTL4/8, and C-terminal domain-containing ANGPTL4 (CD-ANGPTL4) have been associated with overall cardiovascular mortality. In this study, we investigated the associations of ANGPTL3/4/8 proteins and complexes with heart failure (HF) and HF …

Korean Society of Heart Failure Guidelines for the Palliative Care and Hospice for Heart Failure Patients.

Heart failure (HF) is a leading cause of mortality and morbidity in South Korea and imposes substantial physical, emotional, and economic burdens on patients, families, and society. Despite the high symptom burden and complex care needs of patients with HF, palliative care and hospice services remain underutilized owing to cultural, institutional, and knowledge-related barriers. This guideline from the Korean Society of Heart Failure (KSHF) provides evidence-based recommendations to integrate palliative and hospice care across the HF disease trajectory, with the aim of improving quality of lif…

Sex differences in mortality and hospitalisation in heart failure patients: sex differences in heart failure.

OBJECTIVES: We aimed to investigate the clinical and demographic characteristics, and mortality differences between male and female patients with heart failure (HF) with reduced ejection fraction (HFrEF) admitted to our hospital. METHOD: The files of patients who were referred to the HF outpatient clinic of our hospital in Antalya and the surrounding provinces between 2014 and 2019 were analysed retrospectively. RESULTS: 875 patients were included in the study (63.75 ± 13.26 years; 73.1% male). The women were older than the men (67 vs. 64 years, p = 0.002), with higher body mass indexes (27.…

Effect of heart failure pharmacotherapies in patients with heart failure with mildly reduced ejection fraction.

AIMS: The study sought to comprehensively investigate the effect of heart failure (HF) pharmacotherapies in patients with HF with mildly reduced ejection fraction (HFmrEF). In the absence of randomized controlled trials, guideline recommendations concerning HF-related therapies in patients with HFmrEF are limited. METHODS AND RESULTS: Consecutive patients hospitalized with HFmrEF were retrospectively included at one institution from 2016 to 2022. The prognostic value of treatment with beta-blockers (BB), angiotensin-converting enzyme inhibitors, receptor blockers, or receptor-neprilysin inhib…

Iron Deficiency in Heart Failure: A Scientific Statement from the Heart Failure Society of America.

Iron deficiency is present in approximately 50% of patients with symptomatic heart failure and is independently associated with worse functional capacity, lower quality of, life and increased mortality. The purpose of this document is to summarize current knowledge of how iron deficiency is defined in heart failure and its epidemiology and pathophysiology, as well as pharmacological considerations for repletion strategies. This document also summarizes the rapidly expanding array of clinical trial evidence informing when, how, and in whom to consider iron repletion.

Safety and Efficacy of Traditional Heart Failure Therapies in Patients With Cardiac Amyloidosis and Heart Failure.

Randomized controlled trials have demonstrated mortality benefits for several medication classes in patients with heart failure (HF), especially with reduced ejection fraction (EF). However, the benefit of these traditional HF therapies in patients with HF from cardiac amyloidosis is unclear. our study aimed to evaluate the safety and efficacy of traditional HF therapies in patients with cardiac amyloidosis and HF with reduced EF or HF with mid-range EF (HFmrEF). We conducted a single-center retrospective study. Patients were included if they were diagnosed with cardiac amyloidosis and HF with…
