Successful treatment of one case of pemphigoid gestationis complicating with polycystic ovary syndrome.

Pemphigoid gestationis (PG) is a rare, autoimmune, subepidermal bullous dermatosis associated with pregnancy. It is mainly caused by autoantibodies against hemidesmosomal proteins-bullous pemphigoid 180, and usually presents in the second or third trimester of pregnancy sometimes exacerbates spontaneously after delivery. Here we reported a case of PG with polycystic ovary syndrome (PCOS). This patient received assisted reproduction technology and suffered PG in her third trimester of pregnancy. To the best of our knowledge, there is no similar case has been reported.

Plectin gene defects lead to various forms of epidermolysis bullosa simplex.

Plectin is an important organizer of the keratin filament cytoskeleton in basal keratinocytes. It is essential for anchoring these filaments to the extracellular matrix via hemidesmosomal integrins. Loss of plectin or incorrect function of the protein due to mutations in its gene can lead to various forms of the skin blistering disease, epidermolysis bullosa simplex. Severity and subtype of the disease is dependent on the specific mutation and can be associated with (late-onset) muscular dystrophy or pyloric atresia. Mouse models mimicking the human phenotypes allow detailed study of plectin f

Revisiting Type VII Collagen Mislocalisation in Dystrophic Epidermolysis Bullosa Using High-Resolution Imaging.

Type VII collagen (COL7), a major component of anchoring fibrils, is essential for dermal-epidermal adhesion and pathogenic variants in COL7A1, encoding COL7, cause dystrophic epidermolysis bullosa (DEB). In normal skin, COL7 is localised just beneath the lamina densa. In contrast, previous immunoelectron microscopy (IEM) studies of DEB skin with residual COL7 expression demonstrated aberrant localisation of COL7 above the lamina densa and hemidesmosomes of epidermal basal keratinocytes, possibly reflecting impaired secretion of defective COL7. However, conventional microscopy techniques lack …

Bullous pemphigoid and cicatricial pemphigoid autoantibodies react with ultrastructurally separable epitopes on the BP180 ectodomain: evidence that BP180 spans the lamina lucida.

The BP180 antigen is a hemidesmosomal glycoprotein that is recognized by autoantibodies associated with three autoimmune disorders, bullous pemphigoid (BP), herpes gestationis (HG), and cicatricial pemphigoid (CP). BP and HG sera have been shown to recognize a common extracellular site located near the membrane-spanning domain of this protein, whereas CP sera react predominantly with a distinct site near the C terminus. In the current study, the main immunogenic sites on the BP180 ectodomain were ultrastructurally localized using six BP sera, four CP sera, and two rabbit antisera. The immunolo

In situ-bound antibodies eluted from the skin of patients with bullous pemphigoid are preferentially directed against the 230-kD bullous pemphigoid antigen.

Two bullous pemphigoid antigens (BPAs) have been identified: a 230-kD intracellular hemidesmosome-associated molecule and a 180-kD transmembrane hemidesmosome-associated molecule. Although patients with bullous pemphigoid (BP) have been shown to have circulating antibodies directed against one or both BPAs, the antigenic specificity of tissue-bound BP autoantibodies has not been studied. Because these skin-bound antibodies may play an important role in disease initiation, we sought to determine their antigenic specificity. In situ-bound BP antibodies were eluted with glycine from salt-split pe

Identification of a second protein product of the gene encoding a human epidermal autoantigen.

A 230 kDa polypeptide component of the hemidesmosome, an epithelial-cell-connective-tissue attachment device, is thought to be involved in cytoskeleton-cell-surface anchorage. This 230 kDa polypeptide is recognized by bullous pemphigoid auto-antibodies and for this reason is generally termed the bullous pemphigoid antigen (BPA). We have identified two distinct mRNA products of the single BPA gene by RACE (rapid amplification of cDNA ends)/PCR techniques. The first of these mRNAs encodes the 230 kDa protein component of the hemidesmosome. A second mRNA lacks over 1800 bases that encode the C-te

Bullous pemphigoid and herpes gestationis autoantibodies recognize a common non-collagenous site on the BP180 ectodomain.

Bullous pemphigoid (BP) and herpes gestationis (HG) are skin diseases characterized by subepidermal blisters and autoantibodies against two hemidesmosomal Ag, i.e., BP230 and BP180. Based on sequence analysis the BP180 Ag was predicted to be a transmembrane protein with a long extracellular collagenous domain. In the present investigation fusion proteins encompassing various segments of the BP180 Ag were expressed in a prokaryotic system and assayed by immunoblotting and immunoadsorption against a panel of BP, HG and control sera. One antigenic site, comprising 14 amino acids of the BP180 nonc

Plectin in Skin Fragility Disorders.

Plectin is a multi-faceted, 500 kDa-large protein, which due to its expression in different isoforms and distinct organs acts diversely as a cytoskeletal crosslinker and signaling scaffold. It functions as a mediator of keratinocyte mechanical stability in the skin, primarily through linking intermediate filaments to hemidesmosomes. Skin fragility may occur through the presence of mutations in the gene encoding for plectin, PLEC, or through the presence of autoantibodies against the molecule. Below, we review the cutaneous manifestations of plectinopathies as well as their systemic involvement…

Collagen XVII: a shared antigen in neurodermatological interactions?

Collagen XVII is a nonfibril-forming transmembrane collagen, which functions as both a matrix protein and a cell-surface receptor. It is particularly copious in the skin, where it is known to be a structural component of hemidesmosomes. In addition, collagen XVII has been found to be present in the central nervous system, thus offering an explanation for the statistical association between bullous pemphigoid, in which autoimmunity is directed against dermal collagen XVII, and neurological diseases. In support of the hypothesis that collagen XVII serves as a shared antigen mediating an immune r…

Collagen XVII is expressed in human CNS neurons.

Type XVII collagen (collagen XVII) is a component of hemidesmosomes, which connect epithelial cells to the underlying basement membrane. Previously, an association has been suggested between neurological disorders and the skin disease bullous pemphigoid, where autoimmunity is directed against collagen XVII. Furthermore, the lack of alpha6 integrin, a ligand of collagen XVII, has been implicated in defects of cortical organization in the mouse brain. In this study, we demonstrate for the first time the presence of collagen XVII in neurons of the human brain by in situ hybridisation, immunohisto…

Human melanocytic naevi. III. Immunofluorescence and immunoelectron microscopy of the basement membrane zone.

Serum pemphigoid antibody (BPA) was used to demonstrate the intraepidermal location of human pigmented naevi. Immunofluorescence was predominant along the epidermal-dermal junction and around the naevus nests seemingly located in the dermis. Serial sections showed that these naevus nests were intraepidermal. The immuno-gold labelling technique for electron microscopy, was improved by using uranyl acetate with no osmium tetroxide or lead citrate. Hemidesmosomes were absent from the epidermal-dermal junction apposed to naevus nests. Gold labelling occurred in patches of particles along the lamin…

Complement-independent blistering mechanisms in bullous pemphigoid.

Bullous pemphigoid (BP) is an autoimmune subepidermal blistering disease that clinically demonstrates tense blisters with widespread erythema, histologically demonstrates subepidermal blistering and immunologically demonstrates the presence of circulating autoantibodies against hemidesmosomal molecules. Complement activation has long been regarded as necessary for the generation of the BP. However, certain evidence has recently come to support non-complemental blistering mechanisms. The story of BP blistering mechanisms is a complicated one. This review mainly focuses on a specific blistering

Linear IgA disease: cylindroma demonstrates heterogeneity of the target antigens.

Serum from 58 patients with linear IgA disease was tested by indirect immunofluorescence on the novel substrate cylindroma, which produces an abundance of basement membrane components. The use of split skin and of cylindroma has identified 2 target antigens. The majority of patients have a target antigen associated with epidermal cells and similar in distribution on cylindroma to hemidesmosome proteins. A minority have a dermal antigen with the distribution of collagen VII on cylindroma. These data support the hypothesis that linear IgA disease is a heterogeneous disease with regard to the tar

Collagen XVII.

Collagen XVII has been identified as having a role in inherited junctional epidermolysis bullosa non-Herlitz (JEB-other, MIM #226650). The role of collagen XVII in both autoimmune and genetic blistering disorders demonstrates its relevance to dermal-epidermal adhesion. Collagen XVII is a major structural component of the hemidesmosome (HD), a highly specialized multiprotein complex that mediates the anchorage of basal epithelial cells to the underlying basement membrane in stratified, pseudostratified, and transitional epithelia. This article examines the genetic and pathological features of c

Epidermolysis Bullosa-A Different Genetic Approach in Correlation with Genetic Heterogeneity.

Epidermolysis bullosa is a heterogeneous group of rare genetic disorders characterized by mucocutaneous fragility and blister formation after minor friction or trauma. There are four major epidermolysis bullosa types based on the ultrastructural level of tissue cleavage: simplex, junctional, dystrophic, and Kindler epidermolysis bullosa. They are caused by mutations in genes that encode the proteins that are part of the hemidesmosomes and focal adhesion complex. Some of these disorders can be associated with extracutaneous manifestations, which are sometimes fatal. They are inherited in an aut…

Two dogs with juvenile-onset skin diseases with involvement of extremities.

Two dogs of juvenile-onset skin diseases with involvement of extremities were examined by histopathological, immunohistochemical and ultrastructural analyses. Clinically, both cases showed alopecia and crusts on the face and extremities. Case 1 showed histopathology of dermo-epidermal separation. Ultrastructural analysis revealed that the clefts were recognized between hemidesmosomes and lamina densa. In case 2, histopathology showed follicular atrophy, vacuolar degeneration at lower epidermis and masseter muscle degeneration, without remarkable ultrastructural abnormalities in basement membra…

Autoreactive T cell responses in pemphigus and pemphigoid.

Pemphigus and pemphigoid are cutaneous autoimmune diseases characterised by autoantibodies directed against specific adhesion proteins of the epidermis and dermal-epidermal junction. These proteins are usually associated with desmosomes or hemidesmosomes. Binding of antibodies to their targets leads to the loss of cell-cell or cell-matrix adhesion and subsequently to blister formation. The humoral aspects of the autoimmune responses in pemphigus and pemphigoid have been extensively studied in the past. More recently, the cellular interactions resulting in the formation of autoantibodies and th…
