Hemoglobin trends and transfusion practices after cardiac arrest: a cohort study.

BACKGROUND: Hemoglobin targets are absent from post-cardiac arrest guidelines. New trials suggest that patients with brain and myocardial injury, injuries common post-arrest, may benefit from higher hemoglobin targets. Trends in post-arrest hemoglobin and real-world practices remain poorly characterized. METHODS: We performed a retrospective cohort study of consecutive adult cardiac arrests at three academic hospitals. We screened in-hospital and out-of-hospital arrests with resuscitation efforts ongoing at hospital arrival for inclusion. Patients who survived the initial resuscitation and ha…

Ergothioneine exhibits selective, capacity-limited localization within a hemoglobin-associated microenvironment.

Ergothioneine (EGT) is a diet-derived sulfur-containing small molecule that accumulates in red blood cells and has been reported to protect hemoglobin (Hb). However, the concentration of Hb in erythrocytes far exceeds that of EGT, raising the question of how low-abundance EGT can effectively protect Hb. Here, we show that EGT is selectively recovered with Hb under trichloroacetic acid (TCA) precipitation conditions, whereas this behavior was not observed in plasma or non-globin proteins. This selective recovery was reproduced with purified Hb and myoglobin. Recombinant cytoglobin and neuroglob

Hemoglobin San Diego: A Case Report and Review of the Literature.

BACKGROUND High-oxygen-affinity hemoglobinopathies are rare autosomal dominant disorders caused by missense mutations in globin genes. Such variants decrease oxygen delivery to peripheral tissues, resulting in erythrocytosis. Patients are often asymptomatic, and erythrocytosis is frequently detected incidentally. These hemoglobin variants should be considered in the differential diagnosis, particularly when there is a strong family history of erythrocytosis or when other causes of primary and secondary erythrocytosis have been excluded. Although hemoglobin variants are typically benign, they m

Hemoglobin Trajectory Phenotypes and Neurological Outcomes in a Neurosurgical ICU Cohort.

Background/Objectives: Current hemoglobin management in neurocritical care relies on static transfusion thresholds, which fail to capture the dynamic nature of hemoglobin changes during ICU care. We aimed to determine whether distinct longitudinal hemoglobin trajectory phenotypes exist among neurosurgical ICU patients and whether specific trajectory patterns independently predict unfavorable neurological outcomes. Methods: In this retrospective observational cohort study, we analyzed 8517 patients admitted to the neurosurgical ICU of a tertiary academic medical center between January 2015 and

Development of a Plasma Free Hemoglobin-Adsorbing Device for Extracorporeal Therapies.

In extracorporeal therapies such as cardiopulmonary bypass (CPB) or extracorporeal membrane oxygenation (ECMO), the transport of blood through artificial circulation frequently results in the rupture of erythrocytes, releasing hemoglobin (Hb) into the plasma (fHb), and exhausting the body's supply of the natural fHb scavenger-haptoglobin (Hp). This complication, known as mechanical hemolysis, results in the release of fHb and its downstream heme-containing degradation products and is associated with multi-organ dysfunction and adverse outcomes. Current strategies for mitigating hemolysis in ex

Hemoglobin Trajectory During Pregnancy and Postpartum Hemorrhage: A Retrospective Cohort Study.

BACKGROUND: Studies on the association between anemia and postpartum hemorrhage (PPH) mostly use the hemoglobin (Hb) levels in a single trimester, thereby failing to capture the dynamic changes of Hb levels during pregnancy. This study aims to assess the trajectories of Hb during pregnancy and evaluate their association with PPH. METHODS: The study population consisted of 4296 women who gave birth in a tertiary hospital after 28 weeks of gestation between January 2024 and February 2025. Group-based trajectory modelling was used to identify hemoglobin trajectories. Logistic regression models w

Liver sinusoidal endothelial cells constitute a major route for hemoglobin clearance.

Mild rupture of aged erythrocytes occurs in the spleen, resulting in hemoglobin (Hb) release, whereas pathological hemolysis characterizes several diseases. Hb detoxification is attributed to macrophages, but other routes of Hb clearance remain elusive. Here, we uncover that Hb uptake is chiefly executed by liver sinusoidal endothelial cells (LSECs) via macropinocytosis. Consistently, LSECs display proteomic signatures indicative of heme catabolism, ferritin iron storage, antioxidant defense, and macropinocytic capacity, alongside high iron content and expression of the iron exporter ferroport

Hemoglobin oxygen saturation in myopic eyes: a scanning laser ophthalmoscope study.

AIM: To evaluate retinal hemoglobin oxygen saturation in myopic eyes by scanning laser ophthalmoscope (SLO) and to assess its correlations with different severity of myopia. METHODS: Sixty-one eyes from 61 patients were included and subdivided into three groups according to their refractive errors: high myopia group [20 eyes, spherical equivalent (SE)≤-6 D]; low and moderate myopia (22 eyes, -6.0<SE≤-0.5 D); normal (19 eyes, -0.5<SE<+0.5 D). All subjects underwent SLO imaging with dual lasers (532 nm and 633 nm). The oxygen saturations of hemoglobin in arteries (SO2A) and veins (SO2V), and th

A J-shaped relationship between hemoglobin levels and chronic kidney disease.

To evaluate the connection between hemoglobin and chronic kidney disease in the adult population at high altitudes. 380 people who were at least 18 years old were invited to participate in this cross-sectional study, and they underwent interviews and examination for albuminuria, hematuria, estimated glomerular filtration rate (eGFR), hemoglobin, and several other essential variables from April 2021 to May 2021. The total prevalence of chronic kidney disease was 15%. During logistic regression analysis, we found that participants with elevated hemoglobin per 10 g/L had a 27% greater risk of chr…

RapTB: a lung-derived hemoglobin fragment with activity against Mycobacterium tuberculosis.

Tuberculosis (TB) remains difficult to treat due to the need for prolonged multidrug therapy and the global rise of drug-resistant Mycobacterium tuberculosis (Mtb) strains. Endogenous antimicrobial peptides (AMPs) have emerged as promising candidates for host-directed therapies. Given the pulmonary nature of TB, we hypothesized that human lung tissue contains peptides with intrinsic antimycobacterial activity. We screened a peptide library derived from human lung tissue and identified a 39-amino-acid C-terminal fragment of β-hemoglobin (HBB(112-147)), referred to as RapTB, with potent activity

A Hemoglobinopathy That Produces an Array of Different Hemoglobin A1c Values.

Hemoglobin A1c (HbA1c) refers to non-enzymatically glycated hemoglobin and reflects the patient's glycemic status over approximately 3 months. An elevated HbA1c over 6.5% National Glycohemoglobin Standardization Program (NGSP) (48 mmol/mol the International Federation of Clinical Chemistry and Laboratory Medicine (IFCC)) can be used to diagnose diabetes mellitus. In our laboratory, HbA1c is determined by ion-exchange chromatography which has the advantage of detecting common Hb variants such as Hb S, C, E and D without adversely affecting the HbA1c determination. Certain homozygous or compound…

Spontaneous Acute Mesenteric Thrombosis in a Patient With Hemoglobin E Thalassemia.

Acute mesenteric vein thrombosis (MVT) is an uncommon cause of mesenteric ischemia and portal hypertension. The common risk factors for the development of MVT include surgery, acute-intraabdominal inflammatory disorders, malignancies, and other prothrombotic states. However, MVT can occur in the absence of these risk factors. We describe the case of a 40-year-old man with a new diagnosis of hemoglobin E thalassemia and MVT and discuss the relationship between the hemoglobinopathy and thrombosis based on evidence from existing literature. The case emphasizes the importance of a high index of su

Unexpectedly low hemoglobin A1c in a patient with chronic lymphocytic leukemia.

Glycated hemoglobin (HbA1c) assays are currently utilized to monitor patients with diabetes mellitus type 2 (T2DM). These assays employ various methods, some of which are more prone to interference than others. Commonly recognized causes of interference include hemoglobin variants and conditions that result in reduced red blood cell survival such as hemolytic anemia and certain medications. Enzymatic assays represent one of the predominant methods for HbA1c testing for practical reasons. Herein, we describe a potentially novel interference in an enzymatic HbA1c assay by neoplastic lymphocytes

Congenital Methemoglobinemia and Unstable Hemoglobin Variant in a Child With Cyanosis.

Methemoglobinemia (Meth) is a rare hemoglobin (Hb) disorder with distinguished clinical features and complex pathophysiology. We present a three-year-old female who was diagnosed with congenital methemoglobinemia when she presented with peri-oral cyanosis and profound oxygen desaturation in the 20-30% range. This patient also had elevated deoxyhemoglobin (HHb) not explained by methemoglobinemia alone; the low pulse oximetry (SpO2) reading suggested a rightward-shift oxyhemoglobin (O2Hb) dissociation curve, which is the opposite of that expected in methemoglobinemia. This, along with evidence o

Hemoglobin S identification in blood donors: A cross section of prevalence.

INTRODUCTION: In Brazil, the sickle cell trait (SCT) has an average prevalence of 4% in the general population and 6-10% among Afro-descendants. Although SCT is highly prevalent, a large segment of the population ignores their status. The Therapeutic Guidelines prohibit the transfusion of SCT red blood cells into patients with hemoglobin disorders or severe acidosis and newborns. METHODS: This was a cross-sectional study with data from 37,310 blood donation candidates. The study included only eligible first-time donors qualified to be tested for the presence of hemoglobin S (HbS) at the Funda

Does Lowering Hemoglobin A1c Reduce Penile Prosthesis Infection: A Systematic Review.

INTRODUCTION: Hemoglobin A1c (HbA1c), a glycated form of hemoglobin, develops when glucose is elevated in the blood. It is used as a marker of how well a diabetic patient has been controlling their blood sugar over the previous 3-4 months. Some use HbA1c as a predictor of infection risk during prosthetic surgery, and many surgeons require patients to lower it preoperatively. OBJECTIVE: This study was designed to comprehensively review the literature relating HbA1c and penile prosthesis (PP). METHODS: A PubMed search of English-language articles identified studies that investigate the relatio…

Hemoglobin Sunshine Seth: A Case Report of Low-Oxygen-Affinity Hemoglobinopathy.

Pulse oximetry is routinely used in the newborn nursery for clinical monitoring and to detect critical congenital heart disease. The differential diagnoses for reduced peripheral oxygen saturation in an infant include congenital heart disease, respiratory distress syndrome, transient tachypnea of the newborn, persistent pulmonary hypertension of the newborn, meconium aspiration syndrome, pneumonia, pneumothorax, and sepsis. The diagnostic evaluation for neonatal hypoxemia can be invasive and expensive. When this evaluation is unrevealing, other interventions may be tried without clear benefit

First trimester glycosylated hemoglobin as a predictor of gestational diabetes mellitus.

OBJECTIVE: To evaluate the potential role of glycosylated haemoglobin (HbA1C) as an early biomarker for gestational diabetes. METHODS: In a retrospective analysis, healthy women who went on to give birth to a singleton newborn in a tertiary medical center in Petach Tikva, Israel, underwent measurement of HbA1C in the first trimester (up to 12 weeks of pregnancy) between August 1, 2007, and December 31, 2014. Women with type 1 or type 2 diabetes, HbA1C ≥6.5%, and/or fasting plasma glucose ≥126 mg/dL were excluded, as were women whose glucose levels had already been tested before 24 weeks of pr

Characterization of Spbhp-37, a Hemoglobin-Binding Protein of Streptococcus pneumoniae.

Streptococcus pneumoniae is a Gram-positive microorganism that is the cause of bacterial pneumonia, sinusitis and otitis media. This human pathogen also can cause invasive diseases such as meningitis, bacteremia and septicemia. Hemoglobin (Hb) and haem can support the growth and viability of S. pneumoniae as sole iron sources. Unfortunately, the acquisition mechanism of Hb and haem in this bacterium has been poorly studied. Previously we identified two proteins of 37 and 22 kDa as putative Hb- and haem-binding proteins (Spbhp-37 and Spbhp-22, respectively). The sequence of Spbhp-37 protein was
