Hip-spine syndrome.

The incidence of symptomatic osteoarthritis of the hip and degenerative lumbar spinal stenosis is increasing in our aging population. Because the subjective complaints can be similar, it is often difficult to differentiate intra- and extra-articular hip pathology from degenerative lumbar spinal stenosis. These conditions can present concurrently, which makes it challenging to determine the predominant underlying pain generator. A thorough history and physical examination, coupled with selective diagnostic testing, can be performed to differentiate between these clinical entities and help prior…

Bilateral hip fractures.

BACKGROUND: In the past, patients admitted for hip fractures had often sustained a contralateral hip fracture. The incidence of bilateral hip fractures has been reported to be 5%-10%, and in the future the number of bilateral hip fractures is expected to increase with the increase of unilateral hip fractures. We believe that bilateral hip fractures can be efficiently prevented if a fracture on the other side could be predicted after unilateral hip fracture. Therefore, this retrospective study of 835 patients with hip fractures investigated the incidence, prognosis, and risk factors of bilatera…

Arthroscopic hip surgery.

Many pathological entities that previously could be accessed only through large, open incisions now can be treated arthroscopically. Furthermore, arthroscopy has allowed certain conditions to be diagnosed that previously were unrecognized. Arthroscopic hip surgery has become an accepted surgical procedure with well-defined indications and expected outcomes, primarily because of recent advances in surgical instrumentation and techniques. Hip arthroscopy is performed as a same-day procedure using a fracture table to apply distraction to the hip joint. Coordination among surgeons and perioperativ…

Hip arthroscopy update.

The management of hip injuries in the athlete has evolved significantly in the past few years with the advancement of arthroscopic techniques. The application of minimally invasive surgical techniques has facilitated relatively rapid returns to sporting activity in recreational and elite athletes alike. Recent advancements in both hip arthroscopy and magnetic resonance imaging have elucidated several sources of intraarticular pathology that result in chronic and disabling hip symptoms. Many of these conditions were previously unrecognized and thus, left untreated. Current indications for hip a…

Neonatal hip screening.

CONTEXT: A "missed" case of congenital hip dislocation (CDH) can be a disaster for the patient and the outcome may be poor. Considerable resources are expended on screening programmes to identify appropriate cases early but a recent change in terminology to developmental dysplasia of the hip (DDH) and a realisation that neonatal hip maturation is poorly understood has made it difficult to know who should be screened and why. STARTING POINT: Neonatal hip screening is well established although some experts feel that the effectiveness of clinical let alone ultrasound screening programmes is unpr…

Habitual hip dislocation.

A snapping hip is a common problem in children and young adults. The common etiologies of the syndrome have been well described. We believe that habitual dislocation may be a more frequent cause of the snapping hip syndrome than has been previously described. We report two cases of this rarely reported clinical entity presenting as a snapping hip.

Total hip replacement.

Total hip replacement in the dog has proved to be a highly successful method of restoring normal, pain-free motion to the coxofemoral joint. In a prospective study done at The Ohio State University College of Veterinary Medicine and the Berwyn Veterinary Associates Hospital, the indications and contraindications, the most effective surgical technique, the complication rate, and the long-term functional evaluation have been established for canine total hip replacements. This study has established that a 95 per cent success rate for function of the hip joint can be achieved if the established gu…

Total hip arthroplasty.

A review of 333 consecutive Charnley total hip arthroplasties four to seven years after operation showed that twenty-eight patients had died, and three deaths were directly related to the surgery. The survivors had an average improvement of 46.6 points in their evaluation scores to a level of 91.3 points. Four deep infections occurred for which removal of the prosthesis was required. Thirteen hips dislocated postoperatively, and five of them required additional surgery. Three were four sciatic-nerve injuries and all but one resolved. Wear of the acetabular component was evident in seventeen hi…
