Association of Clinical Hypoperfusion Variables With Lactate Clearance and Hospital Mortality.

BACKGROUND: Lactate has shown utility in assessing the prognosis of patients admitted to the hospital with confirmed or suspected shock. Some findings of the physical examination may replace it as screening tool. We have determined the correlation and association between clinical perfusion parameters and lactate at the time of admission; the correlation between the change in clinical parameters and lactate clearance after 6 and 24 h of resuscitation; and the association between clinical parameters, lactate, and mortality. METHODS: Prospective cohort study of adult patients hospitalized in the

Cause-specific mortality patterns among hospital deaths in Tanzania, 2006-2015.

BACKGROUND: Understanding the causes of inpatient mortality in hospitals is important for monitoring the population health and evidence-based planning for curative and public health care. Dearth of information on causes and trends of hospital mortality in most countries of Sub-Saharan Africa has resulted to wide use of model-based estimation methods which are characterized by estimation errors. This retrospective analysis used primary data to determine the cause-specific mortality patterns among inpatient hospital deaths in Tanzania from 2006-2015. MATERIALS AND METHODS: The analysis was carr…

A multiparameter model predicting in-hospital mortality in malignant cerebral infarction.

The early identification of patients with large hemisphere infarctions (LHIs) at risk of fatal brain edema may result in better outcomes. A quantitative model using parameters obtained at admission may be a predictor of in-hospital mortality from LHI.This prospective study enrolled all patients with LHI involving >50% of the middle cerebral artery (MCA) admitted to our neurological intensive care unit within 48 hours of symptom onset. Early clinical and radiographic parameters and the baseline CHADS2 score (congestive heart failure, hypertension, age ≥ 75 years, diabetes mellitus, stroke [doub…

Variability in Very Preterm Stillbirth and In-Hospital Mortality Across Europe.

BACKGROUND AND OBJECTIVE: Stillbirth and in-hospital mortality rates associated with very preterm births (VPT) vary widely across Europe. International comparisons are complicated by a lack of standardized data collection and differences in definitions, registration, and reporting. This study aims to determine what proportion of the variation in stillbirth and in-hospital VPT mortality rates persists after adjusting for population demographics, case-mix, and timing of death. METHODS: Standardized data collection for a geographically defined prospective cohort of VPTs (22+0-31+6 weeks gestatio…

Out-of-hospital mortality among patients receiving methadone for noncancer pain.

IMPORTANCE: Growing methadone use in pain management has raised concerns regarding its safety relative to other long-acting opioids. Methado … mg/d and 40 mg/d, respectively. MAIN OUTCOMES AND MEASURES: The primary study end point was out-of-hospital mortality, given that opioid-related deaths typically occur outside the hospital. RESULTS: There were 477 deaths during 28 699 person-years of follow-up (ie, 166 deaths per 10 000 person-years). After control for study covariates, patients receiving methadon …

[Risk factors and causes of hospital mortality after Bentall-deBono's surgery].

It was done an evaluation of surgical treatment results of 71 patients with aortic arch aneurysm and aortic insufficiency, who were operated routinely. According to data of correlation and multiple regression analysis, chronic obstructive pulmonary disease and kidney disease before operation are significant factors predisposing to death in postoperative period. The authors consider that normalization of lungs and kidneys function before operation improves surgical treatment results in patients of this group.

Discharge status and in-hospital mortality in posterior reversible encephalopathy syndrome.

BACKGROUND: Posterior reversible encephalopathy syndrome is a serious and increasingly recognized disorder, but data from observational stud … uded. Clinical charts, paraclinical and brain imaging data at onset as well as available data on in-hospital mortality and discharge status were analyzed. RESULTS: A total of 103 patients were included. Five (4.8%) patients died during hospital stay, 27 (26.2%) remained hospitalized after discharge. In univariate analyses, significant differences were found between …

Neonatal morbidity and mortality in Calabar, Nigeria: a hospital-based study.

BACKGROUND: The morbidity and mortality pattern amongst neonates admitted into the University of Calabar Teaching Hospital were reviewed from 1st June 2003 to 30th November 2004. RESULTS: The major indications for admission for inborn babies were infections (27.4%), jaundice (21%) and low birth weight (LBW) (18.4%). The out born babies were admitted largely for sepsis (26.8%), jaundice (17.7%), tetanus (13.9%) and low birth weight (11.2%). Staphylococcus aureus (61.2%) and unclassified coliforms (21.9%) were the dominant isolates of septicaemia. The overall mortality rate of 19.3% was largely

Risk factors affecting in-hospital mortality in patients with nosocomial infections.

BACKGROUND/PURPOSE: Nosocomial infection (NI)-associated death is an important issue for both patients and clinicians, and is of emerging importance in public health. This study investigated the factors associated with in-hospital deaths among patients with NI. METHODS: Between July 1, 2002 and June 30, 2003, a total of 1574 patients with NI at National Taiwan University Hospital were enrolled to investigate the factors associated with fatal outcome. Host factors, hospital services, surgical and medical interventions, microbial factors, infection sites, and the treatment and complications of

New diagnostic criteria for acute myocardial infarction and in-hospital mortality.

BACKGROUND: The recent introduction of new diagnostic criteria for acute myocardial infarction (AMI), with troponin measurement, has increas … schemia, with normal enzymes). We evaluated whether this change in criteria had any influence on in-hospital mortality. RESULTS: The new criteria significantly (by 11.9 %) increased the total number of patients admitted with AMI. This was due to an increase in AMI without ST elevation (p < 0.001) and a decrease in patients with UA (p < 0.001), with no changes in AM …

Associations between perinatal interventions and hospital stillbirth rates and neonatal mortality.

BACKGROUND: Previous studies suggest that high risk and low birthweight babies have better outcomes if born in hospitals with level III neonatal intensive care units. Relations between obstetric care, particularly intrapartum interventions and perinatal outcomes, are less well understood, however. OBJECTIVE: To investigate effects of obstetric, paediatric, and demographic factors on rates of hospital stillbirths and neonatal mortality. METHODS: Cross sectional data on all 65 maternity units in all Thames Regions, 1994-1996, covering 540 834 live births and stillbirths. Hospital level analyse

Hospital volume and post-operative mortality after resection for gastric cancer.

AIMS: In low-volume hospitals, expertise in gastric surgery is difficult to maintain because of the decreasing incidence of gastric cancer and the fall of surgery for ulcer disease. We evaluated the prognostic impact of hospital volume on post-operative mortality (POM) in a consecutive series of 1978 patients. METHODS: Information on patients undergoing resection for gastric cancer in the period 1987-97 was retrieved from the Rotterdam Cancer Registry. The relationship between hospital volume and POM was analysed by logistic regression, adjusting for other prognostic factors. RESULTS: POM wa

Impact of hospital volume on operative mortality for major cancer surgery.

CONTEXT: Hospitals that treat a relatively high volume of patients for selected surgical oncology procedures report lower surgical in-hospital mortality rates than hospitals with a low volume of the procedures, but the reports do not take into account length of stay or adjust for case mix. OBJECTIVE: To determine whether hospital volume was inversely associated with 30-day operative mortality, after adjusting for case mix. DESIGN AND SETTING: Retrospective cohort study using the Surveillance, Epidemiology, and End Results (SEER)-Medicare linked database in which the hypothesis was prospectiv…

Tricuspid valve replacement in rheumatic disease: preoperative predictors of hospital mortality.

BACKGROUND AND AIM OF THE STUDY: The study analyzes the possible predictive power of different clinical and hemodynamic parameters with regard to hospital mortality after the first tricuspid valve replacement. METHODS: A retrospective study of 62 consecutive patients undergoing tricuspid valve replacement from 1974 to 1994 (7.2% of all tricuspid surgery performed at our institution in this period) was completed. Hospital mortality was 37%. Twenty-three patients died in hospital after the first tricuspid valve replacement (group I) and 39 patients (group II) survived this procedure. Tricuspid …

HIV-1 seropositivity and mortality at University Hospital, Kinshasa, Zaire, 1987.

This study examines the impact of HIV-1 infection and AIDS on 500 of 563 consecutive deaths at University Hospital, Kinshasa, Zaire, in late 1987. HIV-1 seroprevalence was 31% for the entire population and 43% for the 247 adults. Forty-two (38%) of the 110 HIV-1-seropositive adult deaths occurred in those between the ages of 25 and 34 years. The mean age of death for seropositives was 36 years, 7.5 years less than seronegative deaths. AIDS and AIDS-associated diagnoses such as cryptococcal meningitis, chronic diarrhea and pneumonia accounted for 42% of all adult deaths and 74% of all HIV-1-ser

In-hospital morbidity and mortality in patients undergoing elective coronary angioplasty.

We prospectively recorded all in-hospital complications of the first 3500 consecutive patients to undergo elective coronary angioplasty (PTCA) at Emory University Hospitals from July 14, 1980, to August 28, 1984, by three operators. PTCA was attempted in a total of 3933 lesions, with a primary success rate of 91%. Multiple-lesion PTCA was performed in 401 patients, and PTCA of saphenous vein grafts was attempted in 172. No complications were recorded in 3116 (89%) cases, isolated minor complications occurred in 241 (6.9%), and major complications (emergency surgery, myocardial infarction, deat
