Effective education for patient focused staff. To be successful, you must fill the wide "gaps" between today's capabilities and tomorrow's needs.

In summary, your staff's Patient Focused education must be realigned to their unique clinical, non-clinical and multidisciplinary needs if a restructuring program is be successful. Without this realignment, traditional education services will be inadequate to meet these needs and your program may eventually fail. Perhaps Naisbitt and Auburdene said it best: "By identifying the forces pushing the future, rather than those that have contained the past, you possess the power to engage with your reality." Filling the wide "gaps" between today's educational capabilities and tomorrow's educational n

Establishing a high level of knowledge regarding bioterrorist threats in emergency department physicians: methodology and the results of a national bio-preparedness project.

INTRODUCTION: Medical systems worldwide are facing the new threat of morbidity associated with the deliberate dispersal of microbiological agents by terrorists. Rapid diagnosis and containment of this type of unannounced attack is based on the knowledge and capabilities of medical staff. In 2004, the knowledge of emergency department physicians of anthrax was tested. The average test score was 58%. Consequently, a national project on bioterrorism preparedness was developed. The aim of this article is to present the project in which medical knowledge was enhanced regarding a variety of bioterro

Variation in hospital caesarean section rates for women with at least one previous caesarean section: a population based cohort study.

BACKGROUND: Internationally, repeat caesarean sections make the largest contribution to overall caesarean section rates and inter-hospital variation has been reported. The aim of this study was to determine if casemix and hospital factors explain variation in hospital rates of repeat caesarean sections and whether these rates are associated with maternal and neonatal morbidity. METHODS: This population-based record linkage study utilised data from New South Wales, Australia between 2007 and 2011. The study population included maternities with any previous caesarean section(s) and were singlet

Association of Computer-Assisted Virtual Preoperative Planning With Postoperative Mortality and Complications in Older Patients With Intertrochanteric Hip Fracture.

IMPORTANCE: The outcomes of surgical treatment in patients with intertrochanteric hip fractures are unsatisfactory. Computer-assisted virtual preoperative planning may provide an opportunity to solve this treatment dilemma. Virtual preoperative planning is a technique based on dynamic 3-dimensional computed tomographic imaging, which allows precise evaluation of fracture details and simulation of reduction of fracture and internal fixation procedures before surgery is performed. OBJECTIVE: To evaluate the association of computer-assisted virtual preoperative planning with the risk of 90-day a

The use of paramedics in family planning services in Iran.

In the Family Planning Department of Farah Maternity Hospital, from 1968 to 1976, paramedics were trained to administer all reversible methods of family planning and pregnancy termination up to ten weeks and to perform a limited number of abdominal and vaginal tubectomies (1). From 1974 to 1976, all the above methods, except tubal ligation, were performed satisfactorily on 460 417 acceptors at a clinic not located in the hospital. The complication rate of 1%-5% was the same whether the procedures were performed by paramedics or by physicians. The project has been economical. It has been accept

The use of 3D planning in facial surgery: preliminary observations.

Three-dimensional (3D) planning is becoming a more commonly used tool in maxillofacial surgery. At first used only virtually, 3D planning now also enables the creation of useful intraoperative aids such as cutting guides, which decrease the operative difficulty. In our center, we have used 3D planning in various domains of facial surgery and have investigated the advantages of this technique. We have also addressed the difficulties associated with its use. 3D planning increases the accuracy of reconstructive surgery, decreases operating time, whilst maintaining excellent esthetic results. Preparation for the next major incident: are we ready? A 12-year update.

OBJECTIVES: A major incident is any emergency that requires special arrangements by the emergency services and generally involves a large number of people. Recent such events in England have included the Manchester Arena bombing and the Grenfell Tower disaster. Hospitals are required by law to keep a major incident plan (MIP) outlining the response to such an event. In a survey conducted in 2006 we found a substantial knowledge gap among key individuals that would be expected to respond to the enactment of an MIP. We set out to repeat this survey study and assess any improvement since our orig

Psychiatric consultations in pediatric inpatients.

Emotional problems are not uncommon in hospitalized children. The authors reviewed psychiatric consultations on a pediatric ward in a medical center and compared the findings with a non-pediatric group and the Western reports. Forty consultations were requested in a three-year period which represented 0.35% of all admissions to pediatric ward and 1.0% of all psychiatric consultations in that period. The consultation rate of the pediatric ward is significantly lower than that of the non-pediatric wards. The most common reason for the referrals was for the evaluation of possible psychopathogenes

Concurrent quality assurance for a nutrition-support service.

A pharmacy-based quality assurance program for a nutrition support service (NSS) is described. The NSS, located in a university teaching hospital, is consulted to provide nutritional therapies, primarily total parenteral nutrition (TPN). A planning group from the NSS developed a quality assurance program to monitor specific activities of the service. Four categories of indicators were selected: discretionary, including the indication for TPN and length of TPN therapy; complications, including metabolic, septic, and mechanical; nutritional outcomes, such as nitrogen balance determinations and v

Impact of the fall 2009 influenza A(H1N1)pdm09 pandemic on US hospitals.

BACKGROUND: Understanding how hospitals functioned during the 2009 influenza A(H1N1)pdm09 pandemic may improve future public health emergency response, but information about its impact on US hospitals remains largely unknown. RESEARCH DESIGN: We matched hospital and emergency department (ED) discharge data from the Agency for Healthcare Research and Quality (AHRQ) Healthcare Cost and Utilization Project with community-level influenza-like illness activity during each hospital's pandemic period in fall 2009 compared with a corresponding calendar baseline period. We compared inpatient mortality

The 4 S's of Disaster Management Framework: A Case Study of the 2022 Pediatric Tripledemic Response in a Community Hospital.

Most children in the United States present to community hospitals for emergency department (ED) care. Those who are acutely ill and require critical care are stabilized and transferred to a tertiary pediatric hospital with intensive care capabilities. During the fall of 2022 "tripledemic," with a marked increase in viral burden, there was a nationwide surge in pediatric ED patient volume. This caused ED crowding and decreased availability of pediatric hospital intensive care beds across the United States. As a result, there was an inability to transfer patients who were critically ill out, and

Hospital-based Patient-specific Templates for Total Knee Arthroplasty: A Proof of Concept Clinical Study.

INTRODUCTION: All available patient-specific instruments or patient-specific templates (PSTs) are controlled by implant companies. Most of these companies outsource some of the steps of the PST such as imaging, preoperative planning, manufacturing of PST, and packing/sterilization. This is a proof of concept clinical study on the hospital-based PST system for total knee arthroplasty (TKA). METHODS: A total number of 257 TKA procedures were performed on the basis of a new concept of hospital-based PSTs. All 5 steps of the PST [ie, imaging (computed tomographic scanning), planning, PST producti

Planned Home Births in the United States Have Outcomes Comparable to Planned Birth Center Births for Low-Risk Birthing Individuals.

OBJECTIVE: There are lingering concerns in the United States about home birth. We used 2 large (n = 50,043; n = 62,984), national community birth registries to compare maternal and neonatal outcomes for planned home versus planned birth center births. METHODS: To compare outcomes by intended birth site, we used logistic regressions, controlling for demographic and pregnancy risk variables. Maternal outcomes included intrapartum or postpartum transfer to hospital, hospitalization, cesarean, and hemorrhage; neonatal outcomes included neonatal transfer, hospitalization, neonatal intensive care u
