Targeting deubiquitinating enzymes and ubiquitin pathway modulators to enhance host defense against bacterial infections.

The rise of antibiotic-resistant bacterial pathogens poses a critical global health challenge, necessitating innovative therapeutic strategies. This study explores host-targeted therapies by focusing on deubiquitinating enzymes (DUBs), key regulators of the ubiquitin-proteasome system (UPS) that mediate host-pathogen interactions. Using Salmonella-infected macrophages, we screened a UPS-targeted compound library and identified several compounds that enhanced bacterial clearance without affecting host cell viability. Among these, the dual USP25/USP28 inhibitor AZ-1 emerged as one of the top can

Lessons learned and concepts formed from study of the pathogenesis of the two negative-strand viruses lymphocytic choriomeningitis and influenza.

Viruses have unique lifestyles. To describe the pathogenesis and significance of viral infection in terms of host responses, resultant injury, and therapy, we focused on two RNA viruses: lymphocytic choriomeningitis (LCMV) and influenza (Flu). Many of the currently established concepts and consequences about viruses and immunologic tolerance, virus-induced immunosuppression, virus-induced autoimmunity, immune complex disease, and virus-lymphocyte and virus-dendritic cell interactions evolved through studies of LCMV in its natural murine host. Similarly, the mechanisms, aftermath, and treatment

Chromoblastomycosis Treated With Posaconazole and Adjunctive Imiquimod: Lending Innate Immunity a Helping Hand.

Chromoblastomycosis (CBM) is a difficult-to-treat, chronic fungal infection of the skin and subcutaneous tissue. The evidence base for treatment is scarce, with no standardized therapeutic approach. Chronicity of CBM infection is postulated to be due in part to a failure of host cell-mediated immunity to generate a proinflammatory response sufficient for fungal clearance. We present a case of a chronic chromoblastomycosis lesion of the hand present for nearly 4 decades, previously refractory to itraconazole monotherapy, that was successfully treated with a combination of posaconazole and adjun

Host-targeted oral avian vaccine virus demonstrates broad antiviral activity and safety in patients.

The absence of an immediately deployable, broad-spectrum antiviral remains a critical vulnerability in global pandemic preparedness. Host-directed agents that activate innate immunity offer a pathogen-agnostic strategy, yet no such therapy is currently stockpiled or authorized for emergency use. Infectious Bursal Disease Virus (IBDV)-a non-replicating avian dsRNA vaccine virus with a 60-year safety record in poultry-induces robust interferon responses in mammals and has been administered orally in marmosets and more than 50 human patients with hepatitis A, B, C, SARS-CoV-2, and herpes zoster i

Overcoming Treatment Challenges in HIV-Associated Mycobacterial Diseases: New Therapeutic Frontiers.

For drug-susceptible TB, the WHO-endorsed first-line regimen (isoniazid, rifampicin, ethambutol, pyrazinamide) remains the global reference. Therapy must always be tailored to drug susceptibility, especially in MDR- and XDR-TB. HIV-associated mycobacterial infections-including Mycobacterium tuberculosis (TB), disseminated Mycobacterium avium complex (MAC), and Mycobacterium leprae (M. leprae)-remain leading causes of morbidity and mortality in people living with HIV (PLWH). TB continues to account for the highest burden of AIDS-related deaths worldwide, while MAC and leprosy complicate care in

PLK2 facilitates Mycobacterium tuberculosis clearance via p38 MAPK-mediated ROS production.

BACKGROUND: Traditional anti-tuberculosis (TB) treatments have limitations. Host-directed therapy (HDT), which boosts the immune response, presents a new treatment approach. This study aimed to identify a new host defense factor against tuberculosis, understand its mechanism, and evaluate its potential as an HDT target. RESULTS: Polo-like kinase 2 (PLK2) was discovered as a novel host restriction factor that increases during Mycobacterium tuberculosis infection. It limited bacterial survival by activating the p38 MAPK pathway and enhancing reactive oxygen species (ROS) production. Additionall

Oligometastases: history of a hypothesis.

The term oligometastases represents a clinical state of metastatic disease that is limited in the number of metastatic sites and extent of disease, and amenable to metastasis-directed surgical or ablative therapy. While metastasis-directed approaches are used for palliation, the primary goal of such treatment for patients with oligometastases is to prolong survival and the duration of cancer control. Metastasis-directed therapy, for patients with limited number of metastatic sites, has been practiced for decades, dating back to the era before chemotherapy was widely used. Systemic therapy has

Biofilm-Host Immune Crosstalk at the Diabetic Foot Ulcer Interface: Molecular Mechanisms, Immune Evasion, and Next-Generation Anti-Biofilm Strategies.

Diabetic foot ulcers (DFUs) are chronic wounds in which microbial persistence and defective host defense interact to impair healing. This review examines DFU through the biofilm-host immune interface rather than viewing biofilm as a purely microbiological problem. We summarize how the diabetic wound milieu, including hyperglycemia, impaired perfusion, neuropathy, and polymicrobial community structure, favors persistent biofilm infection, and how DFU-relevant biofilms evade clearance through matrix shielding, altered innate recognition, virulence-associated host modulation, and intracellular St

Integrative transcriptome-based drug repurposing in tuberculosis.

Tuberculosis (TB) remains the second leading cause of infectious disease mortality worldwide, killing over one million people annually. Rising antibiotic resistance has created an urgent need for host-directed therapeutics (HDTs) - preferably by repurposing existing approved drugs - that modulate host immune responses rather than directly targeting the pathogen. Repurposed therapeutics have been successfully identified for cancer and COVID-19 by finding drugs that reverse disease gene expression patterns (an approach called 'connectivity scoring'), but this approach remains largely unexplored

The quest for host targets to combat dengue virus infections.

Dengue virus (DENV) is a human threat of increasing importance. Although a tetravalent vaccine has been recently approved, owing to limited efficacy there is still an urgent need for antiviral drugs to prevent or treat DENV infections. Traditionally, antiviral drug discovery has focused on molecules targeting viral factors. However, thus far the identification of direct-acting antiviral drugs with potent DENV pan-serotypic activity has been problematic. An alternative are host-targeting antiviral drugs that hold great promise for broad-spectrum activity. In this review, we summarize cellular f

Myeloid immune reprogramming in bone metastatic cancer: Mechanistic crosstalk, therapeutic targets and unresolved clinical challenges.

Bone metastasis is a major clinical challenge in advanced solid tumors, driving skeletal-related events, chronic pain, and therapeutic resistance. Increasing evidence suggests that the bone metastatic niche is predominantly shaped by reprogrammed myeloid cells, including tumor-associated macrophages (TAMs), myeloid-derived suppressor cells (MDSCs), neutrophils, and tolerogenic dendritic cells. These cells interact with osteoclasts and stromal components through RANKL, TGF-β, CSF-1/CSF-1R, CCL2/CCR2, and Siglec sialoglycan pathways to promote osteolysis, immune suppression, and metastatic expan

Host-Pathogen Dual Targeting With Repurposed Drugs Identifies a Synergistic Therapy for Intracellular Staphylococcus aureus.

Staphylococcus aureus is a major cause of severe infections, including pneumonia and sepsis, partly due to its ability to survive within host cells where many antibiotics are ineffective. Drug repurposing offers a rapid strategy to identify compounds that enhance intracellular antibacterial activity by modulating host pathways. Here, a high-throughput screen of 6297 clinically approved compounds in S. aureus-infected A549 cells identified 5-fluoro-2'-deoxycytidine (5-FdC) as an effective intracellular inhibitor. When combined with rifapentine (5FR), 5-FdC displayed synergistic activity across

Mycobacterium tuberculosis-induced PCBP1 degradation drives macrophage ferroptosis to promote infection: a lung-macrophage-targeted RNAa nanotherapy.

BACKGROUND: Immune evasion by Mycobacterium tuberculosis (Mtb) complicates tuberculosis (TB) therapy. Ferroptosis, an iron-dependent form of regulated cell death, is increasingly recognized as a critical process in host-pathogen interactions. We aimed to define the role of poly(C)-binding protein 1 (PCBP1) in macrophage ferroptosis during Mtb infection and to develop a targeted RNA activation (RNAa) nanotherapy to exploit this pathway. METHODS: We analyzed clinical samples from TB patients and investigated Mtb-host interactions in macrophage models using molecular and biochemical assays. Mann

Mycobacterium tuberculosis infection drives osteoclast overactivation via α2,3-Sialylation to promote pathological bone destruction.

INTRODUCTION: Bone tuberculosis is characterized by severe bone destruction driven by aberrant osteoclast overactivation. However, the direct mechanism by which Mycobacterium tuberculosis (Mtb) mediates this pathological process remains unclear. Understanding the molecular basis of pathogen-driven osteoclast dysregulation is essential for developing effective host-directed therapeutic strategies. METHODS: Transcriptomic profiling was performed to identify differentially expressed sialylation-related genes and activated signaling pathways in Mtb-infected cells. Murine bone-tuberculosis models

Intracellular Salmonella hijacks the mitochondrial citrate carrier to evade host oxidative defenses.

Intracellular vacuolar pathogens replicate within membrane-bound compartments known as pathogen-containing vacuoles (PCVs). Maintaining the integrity of these vacuoles is essential for creating a permissive niche that supports pathogen survival and proliferation. In this study, we show that Salmonella enterica serovar Typhimurium co-opts the host mitochondrial citrate carrier (CIC) to promote its intracellular replication by detoxifying the Salmonella-containing vacuole (SCV). Loss of CIC significantly impairs Salmonella growth within host cells, as CIC recruitment to SCVs regulates local citr

Host-directed antileishmanial interventions: Harvesting unripe fruits to reach fruition.

Leishmaniasis is an exemplary paradigm of immune evasion, fraught with the perils of limited clinical assistance, escalating costs of treatment and made worse with the lack of suitable vaccine. While drugs remain central to large-scale disease control, the growing emergence of parasite resistance necessitates the need for combination therapy involving host-directed immunological agents. Also, since prolonged disease progression is associated with strong immune suppression of the host, augmentation of host immunity via restoration of the immunoregulatory circuit involving antigen-presenting cel

Immunotherapy targeting drug-tolerant Mycobacterium tuberculosis persisters accelerates tuberculosis cure in preclinical models.

Mycobacterium tuberculosis remains a global health crisis, ranking among the deadliest infectious diseases worldwide. In response to the WHO's call for therapeutic vaccines to complement antibiotic regimens and reduce tuberculosis (TB) treatment duration, we developed an intranasal DNA vaccine fusing the M. tuberculosis stringent response gene relMtb with the gene encoding the DC-targeting chemokine Mip3a (also known as CCL20). Administered alongside the first-line regimen, this vaccine accelerated a stable cure in immunocompetent murine TB models, reducing lung inflammation and eliciting robu

Neutrophil progenitor cell therapy rescues host defense against Staphylococcus aureus in murine chronic granulomatous disease.

Despite advances in engineered adaptive immune cell therapies, current options for innate immune cell therapies are sparse. In this work, we demonstrate the utility of a neutrophil progenitor (NP)-based cell therapy. Murine conditionally immortalized NPs overcome some of the hurdles of alternative cell therapies, such as granulocyte transfusion, by engrafting in the unconditioned host and undergoing substantial expansion in vivo. Here, we demonstrate the therapeutic value of NPs using a murine model of the primary immunodeficiency chronic granulomatous disease (CGD). Those with CGD are highly

Towards precision antileishmanial drug discovery: Integrating multi-omics, functional genomics, artificial intelligence and host-directed therapeutics.

Leishmaniasis remains one of the most neglected tropical diseases, affecting millions of people worldwide and posing a substantial public health burden in endemic regions. Current treatment options are limited by toxicity, high costs, prolonged treatment regimens, treatment failure, and the increasing emergence of drug-resistant parasites. Moreover, the remarkable genomic plasticity and adaptive capacity of Leishmania species, driven by mosaic aneuploidy and copy number variation, complicate durable target identification and underscore the need for precision-oriented therapeutic approaches. Will It Be Possible in the Future to Target Innate Immunity Against Staphylococcus aureus Using Resident Microbiota and Plant Extracts?

Staphylococcus aureus remains a major cause of hospital- and community-acquired infections, successfully evading host defense mechanisms and escaping proposed vaccination strategies. The effective response of professional phagocytes is crucial in limiting the spread, tissue invasion, and subsequent infection by S. aureus. However, direct stimulation of innate immunity with S. aureus antigens is too risky due to their strong proinflammatory properties. Therefore, this study aimed to test the hypothesis that plant extracts trigger the gentle release of staphylococcal bioactive components that ca
