A Case of Nonunion with Ballooning Deformity after Periprosthetic Humeral Fracture.

INTRODUCTION: We report a very rare case of nonunion with ballooning deformity of the humeral bone after a periprosthetic humeral fracture nonunion. CASE REPORT: A 79-year-old woman underwent hemiarthroplasty 19 years ago for her proximal humerus fracture. She injured her right humeral diaphysis (stem distal end fracture) 6 years ago. She underwent revision hemiarthroplasty with long stem, but bony union was not obtained, and her right upper limb function was subsequently abolished due to extreme instability and pain in her right upper arm. She was then referred to our hospital for further tr

Nonunion of conservatively treated humeral shaft fractures is not associated with anatomic location and fracture pattern.

INTRODUCTION: Humeral shaft fractures make up 1-3% of all fractures and are most often treated nonoperatively; rates of union have been suggested to be greater than 85%. It has been postulated that proximal third fractures are more susceptible to nonunion development; however, current evidence is conflicting and presented in small cohorts. It is our hypothesis that anatomic site of fracture and fracture pattern are not associated with development of nonunion. MATERIALS AND METHODS: In a retrospective cohort study, 147 consecutive patients treated nonoperatively for a humeral shaft fracture we

Bilateral surgery of humeral intracondylar fissures and humeral condylar fractures is associated with increased complication rates.

OBJECTIVES: To compare the postoperative complication rates and outcomes of client-owned dogs treated surgically for humeral intracondylar fissures and/or humeral condylar fractures either as unilateral or bilateral simultaneous procedures. MATERIALS AND METHODS: Hospital records from a single referral centre in the United Kingdom were reviewed retrospectively. Dogs were categorised into the unilateral and bilateral groups and then further subdivided according to the surgical procedure performed. Clinical follow-up and a client evaluation scheme were used to identify complications and assess

Biomechanics of Upper Extremity Double Plating.

Double plating (DP) is an established treatment modality in traumatology. For certain indications, such as open reduction and internal fixation of bicondylar tibial plateau fractures and distal humeral fractures, DP is standard of care in the clinical routine. However, the principles of DP can be transferred to other anatomic regions and indications. In the past years, DP has been brought more and more into focus. For revision cases, such as the treatment of fracture non-unions, DP seems generally useful. In anatomical locations with high torsional stress and bending forces, but also in region

[Total elbow arthroplasty in an older adult patient, as a therapeutic option for the management of pseudoarthrosis in distal humerus fracture].

INTRODUCTION: Distal humeral fractures in the elderly patient are a therapeutic challenge for orthopaedists, because of ages characteristic physiological conditions and bone changes. CASE REPORT: We present the case of an 80-year-old woman with osteopenia, who had distal humeral fracture due to a fall from her own height; she was initially treated conservatively, but by the reason of a non-union, we decided to perform a total elbow arthroplasty, achieving a positive functional result in the four years of postoperative follow-up. DISCUSSION: Nonsurgical management is an option for treat humer

Vascularity of the humeral head after proximal humeral fractures. An anatomical cadaver study.

We studied the arterial anatomy and the effect of four-part fractures on the vascularity of the humeral head, using barium sulphate perfusion of 16 cadaver shoulders. The main arterial supply to the humeral head was via the ascending branch of the anterior humeral circumflex artery and its intraosseous continuation, the arcuate artery. There were significant intraosseous anastomoses between the arcuate artery and: 1) the posterior humeral circumflex artery through vessels entering the posteromedial aspect of the proximal humerus; 2) metaphyseal vessels; and 3) the vessels of the greater and le

A new modular nail for the diaphyseal fractures of the humerus.

Over a period of 5 years, 63 traumatic and eight pathological diaphyseal humeral fractures were treated with a new modular humeral nail. The nail is cannulated, square in shape--with concave sides--and has two different extensions that can be used with either the antegrade or the retrograde approach. Adequate rotational and axial stability is provided without the need for distal locking screws in the majority of fractures, while the need for proximal locking screws during the antegrade procedure is abolished. This study aims to present the 'Garnavos' nail and the results of its use, along with

Proximal fracture line extension in humeral shaft fractures.

INTRODUCTION: Humeral shaft fractures are common injuries treated by orthopaedic surgeons. The purpose of this study is to evaluate displaced diaphyseal humerus fractures and describe the incidence and characteristics associated with non or minimally displaced fracture line extension into the proximal metadiaphyseal region of the humerus. METHODS AND MATERIALS: All adult patients with diaphyseal humeral shaft fractures located within the distal two-thirds of the humeral shaft, treated at a single level I trauma institution between 2007 and 2020, were retrospectively identified. 202 patients w

[Intramedullary fixation of humerus shaft fractures. An analysis of complications of 2 implants with special reference to outcome after management with the unreamed humerus interlocking nail].

INTRODUCTION: Operative treatment of humeral shaft fractures has gained a new impetus with the development of intramedullary interlocking systems for the humerus. Because of the anatomical structure of the humeral marrow cavity, a regular jamming of nail into bone is not to be expected, so that the stability of these systems needs to be achieved through an interlocking mechanism. Among the commercially available interlocking nailing systems the unreamed humeral nail (UHN) and the Seidel-nail (SHN) can be regarded as the standard implants. DESIGN: In a retrospective study comparing two groups

Comparison of open and closed reduction and percutaneous pinning for pediatric lateral humeral condyle fractures: A systematic review and meta-analysis.

BACKGROUND: Open reduction and percutaneous pinning (ORPP) is commonly regarded as the primary treatment option for serious displaced lateral condyle fractures of the humerus (LCFs) in children. However, some authors have suggested that closed reduction and percutaneous pinning (CRPP) may be an appropriate method for treating LCFs. This meta-analysis aims to compare the outcomes of these 2 fixation techniques. METHODS: Our study conducted a search of the Pubmed, Embase, and Cochrane Library databases for published research up to October 1, 2022. Our analysis comprehensively compared the opera

Radial nerve palsy following pediatric humeral shaft fractures: a systematic review and pooled analysis.

Pediatric humeral shaft fractures associated with radial nerve palsy are rare, with an incidence of 4-4.35%. There exists one study on this topic; this systematic review and pooled analysis aim to shift the focus from incidence to recovery, apply patient-level analyses, reassess previously reported associations, identify discrepancies to inform clinical decision-making, and elucidate possible factors associated with poor recovery using exploratory and hypothesis-generating analyses. PubMed, Cochrane Central, and Google Scholar were searched using the following terms: (radial nerve palsy) AND (

Dual plate fixation on distal third diaphyseal fracture of the humerus.

INTRODUCTION: Displaced unstable distal third fractures of the humeral diaphysis are treated surgically through open reduction and internal fixation. Conventionally, surgeons prefer using long plates for secure fixation; however, we performed short plate dual plating for robust fixation that required a smaller incision and less dissection through an anterior approach. In this study, we report the results of dual plating of fractures of the humeral shaft, with radiographic evidence and clinical analysis. METHODS: This retrospective study included 29 patients with distal third diaphyseal fractu

[Articular Fractures in the Elderly: to Fix or to Replace? - Distal Humerus].

Distal humeral fractures in adults are rare. They are routinely treated by open reduction and internal fixation in an attempt to retain a painless, stable and functional joint. However, even with advances in plate technology, the results of fixation still depend on screw purchase and bone quality. Even if functional results appear to be favourable, complication rates remain quite high. This is particularly true in elderly patients, in whom multifragmentary intraarticular fractures are mostly associated with diminished bone quality. Recent results support the use of primary total elbow arthropl

[Treatment of humeral shaft fracture with intramedullary procedures (Seidel nail, Marchetti-Vicenzi nail, Prevot pins)].

Humeral shaft fractures can be treated either conservatively or operatively. Plating of the humerus is the established operative method, but recently interest has also been focussed on intramedullary nailing. Fifty-nine cases of humeral fractures treated with intramedullary nailing (Seidel/Marchetti-Vicenzi/Prévot) from January 1991 to December 1995 (44 fractures after trauma, 11 pathological fractures, 3 pseudarthroses, 1 re-fracture). Closed reduction in 55/59 cases. One infection (soft tissue); 2/48 pseudarthrosis (indication for nailing: pseudarthrosis!). No iatrogenic palsy of the radial

Clinical outcomes following Triceps-Reflecting Anconeus pedicle (TRAP) approach for treating intercondylar distal humerus fractures: A systematic review.

PURPOSE: Distal humerus fractures (DHFs) constitute 2% of all fractures, with open reduction and internal fixation (ORIF) often indicated for treatment. Within ORIF literature exists the triceps-reflecting anconeus pedicle (TRAP) approach: an alternative that offers adequate exposure while preserving key soft-tissue structures hypothesised to improve outcomes. Despite this, TRAP-related findings remain poorly collated and lacking consensus. Thus, this systematic review aims to synthesise TRAP-utilising intercondylar DHF ORIF outcomes to guide and inform surgical decision-making. METHODS: A PR

Complications of Seidel intramedullary nailing of narrow diameter humeral diaphyseal fractures.

Forty acute blunt fractures of the humeral diaphysis were treated with Seidel nails (9-mm diameter) between April 1988 and August 1992. Follow up was available for 36 patients. Average Injury Severity Score (ISS) was 22. Forty-eight pelvic and lower extremity fractures necessitated humeral weight bearing in 23 patients. Patients were grouped by canal diameter measured at the point of distal humeral fixation: < or = 9 mm or > or = 10 mm. Data were analyzed with regard to complications, fracture pattern, and time to union. Due to difficulties reaming the humeral canal, five different reaming sys

Outcomes and complications of metallic vs. bioabsorbable screw fixation for displaced medial epicondylar fractures in pediatric and adolescent patients.

PURPOSE: The use of bioabsorbable screws in orthopedics and trauma surgery is increasing. The major advantage of not having to remove the implant is of particular interest in pediatric surgery. Due to drawbacks, most notably implant fracture, bioabsorbable screw osteosynthesis has not yet surpassed the gold standard of metallic screw osteosynthesis. This single-centre study compares metallic screw and bioabsorbable screw osteosynthesis of medial humeral epicondyle fractures in children and adolescents over a two-year period. MATERIALS AND METHODS: Retrospective analysis of surgically treated
