A 70-year-old female presented with the clinical triad of normal pressure hydrocephalus (NPH) and senile tremor. Neuroimaging disclosed findings of both NPH and empty sella (ES). A ventriculoperitoneal shunt did not modify the clinical course except for a mild and transient improvement, and shunt malfunction occurred later on. The association of NPH and ES may result from a common underlying mechanism such as transient increases in intracranial pressure.

The regional cerebral metabolic rate for glucose (rCMRglu) has never been investigated in large consecutive groups of patients with normal pressure hydrocephalus (NPH), a potentially treatable form of dementia with an unpredictable outcome after shunt surgery. Using PET and 18F-2-fluorodeoxyglucose, rCMRglu was studied in 18 patients who fulfilled hydrodynamic criteria for NPH and in whom a biopsy of the frontal cortex was obtained. When compared with an age matched group of 11 healthy subjects, the patients with NPH showed a significant rCMRglu reduction in all cortical and subcortical region

Seven patients with psychiatric and behavioural manifestations of normal-pressure hydrocephalus are described. There is little disagreement about the classical triad of symptoms. However, it requires a high index of suspicion when a patient presents with non-specific psychiatric and behavioural symptoms. Clinicians must be alert to the possibility when positive findings point to the diagnosis of normal-pressure hydrocephalus, namely gait disturbance, incontinence, soft neurological signs, history of hypertension and no previous psychiatric history.

Normal pressure hydrocephalus (NPH) in elderly patients is reviewed. Since the clinical signs that characterize NPH--incontinence, difficulty walking, and dementia--occur frequently in the elderly in association with a wide variety of disorders, attention is paid to the differential diagnosis of NPH on the basis of both clinical findings and laboratory tests. Success rates for treatment of NPH using surgical shunting are discussed, as are mortality rates and complication rates for this type of surgery in the elderly. Factors that may be of value in predicting which NPH patients are most likely

The evaluation of dementia usually includes a consideration of normal pressure hydrocephalus, which may be a treatable aspect of the patient's cognitive dysfunction. This article outlines clinical syndromes, standard radiologic evaluation, and newer diagnostic tests that may suggest cerebrospinal fluid (CSF) shunting. In general, patients who present with dementia alone will not respond well to CSF shunting because of cerebral atrophy and the lack of tension within the cerebral ventricles. However, normal pressure hydrocephalus remains a diagnostic consideration, and improved evaluation may al

Normal pressure hydrocephalus (NPH) is generally considered to be a disorder of adult and geriatric patients. We report four patients who are children or young adults with chronic neurological disorders, recent deterioration of their levels of function, normal cerebrospinal fluid (CSF) pressures, and ventricular enlargement. All four patients improved after the placement or revision of a ventriculoperitoneal shunt. Frequent symptoms and signs included irritability (three patients), vomiting (three patients), and abnormal limb posturing (two patients). Correct diagnosis was hampered by two fact

Although senile dementia of Alzheimer type (SDAT) is the commonest cause of dementia in the elderly, it is important to recognise potentially reversible causes such as normal pressure hydrocephalus. This has been reported to occur in association with Paget's disease of the skull, but would appear to be an extremely rare complication of this disease. To date 4 cases of successful surgical intervention in normal pressure hydrocephalus complicating Paget's disease of skull have been reported. We report 3 cases which have presented to one Department of Geriatric Medicine during a 3-year period. Tw

Two observations of intrinsic third ventricle craniopharyngiomas, both involving males in the fifth decade, are reported. Histologically, the tumours, one entirely solid and the other one chiefly cystic, were composed of squamous epithelium with microcysts and no calcifications. In the literature primary third ventricle craniopharyngiomas are considered to be exceedingly rare. However, if a more precise preoperative diagnosis, using computerised axial tomography, were made as a matter of routine, to verify third ventricular masses, it might demonstrate that these tumours are more common than p

After reviewing the literature on psychic disturbance in normal pressure hydrocephalus, the author reports on 26 cases of CSF drainage. Impairment of memory and concentration were always present in normal pressure hydrocephalus; behaviour disorders were very frequent and characterized by both types of frontal behaviour; the mental picture also comprised difficulties of calculation, writing, visuoconstructive praxis and, less frequently, impaired speech and reading ability. CSF drainage brought clinical improvement in 73% of the cases. Disorders of consciousness, mental function and behaviour r

5 patients are described in which normal pressure hydrocephalus was diagnosed. The criteria on which the diagnosis was based were a cerebrospinal fluid perfusion test and an air study. Pathological verification was additionally obtained in 2 patients. The clinical picture in all patients was characterized by dementia. 4 patients were incontinent and 3 were paraspastic.

The authors report ten cases of normal pressure hydrocephalus treated by ventricular drainage. They pay particular attention to the neuropsychological and psychometric aspects and endeavour to identify the most characteristic nosological features by comparing their own clinical observations with those reported in the literature.

A 61-year-old woman presented with a 10-month history of gait disturbance and a 7-month history of urinary incontinence. The Hasegawa dementia scale-revised score indicated cognitive impairment. Brain magnetic resonance imaging (MRI) indicated hydrocephalus with disproportionately enlarged subarachnoid space. This is usually considered a characteristic finding in idiopathic normal pressure hydrocephalus (iNPH). Ventriculo-peritoneal shunting improved the patient's symptoms. Neurosarcoidosis was suspected as a cause of the hydrocephalus because of the abnormalities in the cerebrospinal fluid an

The indications for shunt operation in patients with idiopathic normal pressure hydrocephalus accompanied by brain atrophy (atypical idiopathic normal pressure hydrocephalus: AINPH) were investigated in 25 patients who satisfied the diagnostic criteria and underwent ventriculoperitoneal (VP) shunting. All patients had no apparent history of intra- or extracranial disease; dementia and gait disturbance as the main complaints; moderate to severe cerebral atrophy and ventricular dilatation and at least periventricular low density around the anterior horn on computed tomography; normal cerebrospin
