Urinary cytokine profiles in unilateral congenital hydronephrosis.

BACKGROUND: We aimed to evaluate possible clinical application of urinary monocyte chemotactic protein-1 (MCP-1), osteopontin (OPN), and regulated upon activation, normal T-cell expressed and secreted (RANTES) chemokine as useful noninvasive markers in children with congenital hydronephrosis (HN) caused by ureteropelvic junction obstruction (UPJO). METHODS: The study cohort consisted of surgical cases (study group 1), comprising 15 children with severe HN who required surgery (median age 1.03 years), conservative cases (study group 2), comprising 21 patients with mild, nonobstructive HN, and …

Hydronephrosis after radical hysterectomy: a prospective study.

One part of the operative procedure of radical hysterectomy (RH) is the dissection of the ureter from its overlying tissue and this may result in injury to the ureteric adventitia. This might induce ureteric obstruction and consequently produce hydronephrosis. The objective of this prospective study was to evaluate the incidence of hydronephrosis after RH in patients with early stage cervical cancer. From July 2006 through March 2007, 77 patients with IA2-IIA cervical cancer who planned to undergo radical hysterectomy and pelvic lymphadenectomy (RHPL) received urinary tract ultrasonography 5 t…

Penicillin G-induced hemorrhagic cystitis with hydronephrosis.

Irritable urological symptoms with gross hematuria and bilateral lumbar pain developed when the patient received penicillin G for endocarditis. These symptoms were followed by renal insufficiency. A contrast-enhanced abdominal computed tomography (CT) scan revealed a thickened bladder wall, bilateral hydroureter and hydronephrosis, suggesting hemorrhagic cystitis complicated with urinary tract obstruction. Urine culture was negative. After discontinuation of penicillin G, all symptoms subsided and renal function recovered; hence, penicillin G seems to have been associated with hemorrhagic cyst…

Ultrasonographic unilateral hydronephrosis with bladder outlet obstruction.

Renal ultrasonography plays an important role in the evaluation of azotemic patients. Knowledge of its limitations is crucial for the proper evaluation of obstructive uropathy. Two cases are presented in which screening ultrasonography detected unilateral hydronephrosis despite lower urinary tract obstruction. Both patients had markedly asymmetrical renal function, with the hydronephrosis found in the better functioning kidney. The importance of searching for asymmetry of renal function in the azotemic patient with atypical ultrasonogram findings is emphasized. Other clinical factors which wou…

Persistent maternal hydronephrosis after intra-abdominal pregnancy.

Pregnancy presents a wide range of difficult urological management problems. We report a unique case of a term intra-abdominal pregnancy complicated by persistent hydronephrosis after cesarean section. Intra-abdominal placenta degeneration was monitored by serial serum beta-human chorionic gonadotropin values. Hydronephrosis ultimately resolved with conservative management.

Fetal hydronephrosis and renal artery blood velocity.

The renal artery blood velocity waveform was recorded by Doppler ultrasound in 17 fetuses with hydronephrosis in the third trimester of pregnancy. The waveform was analyzed for pulsatility index and related to the degree of dilatation of the renal pelvis. The pulsatility index was found to be above the renal reference range in only five out of the total of 31 kidneys examined. No relationship was found between the degree of dilatation and pulsatility index. A slight relationship was, however, found between abnormal pulsatility index and the need for operative intervention in the neonatal perio…

[Congenital hydronephrosis: pyeloureteral junction syndrome. Surgical treatment].

This paper reviews the surgical procedure performed in 119 kidneys (114 children), diagnosed with congenital hydronephrosis due to pyeloureteral juncture stenosis (PUJS). Approaches used for diagnosis and choice of surgical techniques are commented. Puncture/nephrostomy with functional kidney assessment has been the method selected in extreme cases. Nephrectomy was the chosen option in 8 cases (6.7%) while conservative surgery was indicated in the others. Anderson-Hynes dismembered pyeloureteral plasty was the technique used in 110 cases with a 97.28% rate of success. Post-operative occurrence…

Urinary tract infections in children with prenatal hydronephrosis: A risk assessment from the Society for Fetal Urology Hydronephrosis Registry.

INTRODUCTION: Risk factors for urinary tract infection (UTI) in children with prenatal hydronephrosis (PNH) are not clearly defined. Our study aim was to describe incidence and identify factors associated with UTI among a cohort of children diagnosed with PNH. MATERIAL AND METHODS: Patients with confirmed PNH from four medical centers were prospectively enrolled in the Society for Fetal Urology (SFU) hydronephrosis registry between 9/2008 and 10/2015. Exclusion criteria included enrollment because of UTI, associated congenital anomalies, and less than 1-month follow-up. Univariate analysis wa…

Continuous antibiotic prophylaxis in isolated prenatal hydronephrosis.

BACKGROUND: Prenatal hydronephrosis (PNH) is one of the most common congenital anomalies and can increase the risk of developing a urinary tract infection (UTI) in the first two years of life. Continuous antibiotic prophylaxis (CAP) has been recommended empirically to prevent UTI in children with PNH, but its use has been controversial. OBJECTIVE: We describe the incidence of UTI in children with isolated PNH of the renal pelvis without ureteral dilation. Our objective was to compare patients receiving and not receiving CAP and determine whether CAP is beneficial at preventing UTI in children…

Ovarian Follicular Lymphoma Diagnosed Due to Hydronephrosis.

A 74-year-old woman presented with left lateral abdominal pain. Abdominal echography revealed left hydronephrosis and a pelvic mass. The patient underwent left adnexal resection of a suspected left ovarian tumor and was diagnosed with follicular lymphoma (FL) of clinical stage IIIA, grade 2. The patient was treated with rituximab-combined chemotherapy and achieved complete remission. The most common histological types of ovarian lymphoma are diffuse large B-cell lymphoma and Burkitt lymphoma, with FL being an extremely rare variant. We herein report a case of ovarian FL diagnosed as hydronephr…

Giant hydronephrosis complicated by multiple uroepithelial carcinomas.

Giant hydronephrosis is uncommon, and malignant hydronephrosis of unknown origin is even rarer. We report a case of a 43-year-old male patient with giant hydronephrosis without painless carnivorous hematuria. Preoperative imaging and urinary search for exfoliated cells revealed no evidence of renal pelvic cancer. The patient underwent simple nephrectomy and pathology confirmed the diagnosis of multifocal high-grade papillary uroepithelial carcinoma of the renal pelvis.

ACR Appropriateness Criteria® Antenatal Hydronephrosis-Infant.

Antenatal hydronephrosis is the most frequent urinary tract anomaly detected on prenatal ultrasonography. It occurs approximately twice as often in males as in females. Most antenatal hydronephrosis is transient with little long-term significance, and few children with antenatal hydronephrosis will have significant obstruction, develop symptoms or complications, and require surgery. Some children will be diagnosed with more serious conditions, such as posterior urethral valves. Early detection of obstructive uropathy is necessary to mitigate the potential morbidity from loss of renal function.…

[Case of cystitis glandularis causing bilateral hydronephrosis].

A 38 year-old male was referred to our hospital due to bilateral hydronephrosis. Cystoscopy revealed multiple edematous papillary tumors that exist on the prostatic urethra, bladder neck, trigone, and lateral wall on both sides. He underwent a transurethral resection of bladder tumors. The pathological diagnosis was cystitis glandularis. He was given steroid orally for 6 months and had no sign of recurrence after 2 years.

Spontaneous rupture of fetal hydronephrosis: case report.

Hydronephrosis is the most common congenital anomaly observed with prenatal ultrasonography. Ureteropelvic junction obstruction (UPJO) is the most common cause of prenatal hydronephrosis. Spontaneous rupture has been reported in adults with severe hydronephrosis. There is no reported spontaneous rupture case in the fetus in the literature. A spontaneous ureteral rupture due to severe UPJO was reported in this case report. Prenatal ultrasound at 33 week gestation in a 21-year-old pregnant woman, revealed a female fetus with grade IV hydronephrosis of the right kidney, suggestive of a UPJO. Duri…

[Fetal hydronephrosis. Case report and literature review].

Fetal hydronephrosis is the most common congenital anomaly detected by routine prenatal ultrasound. It is the abnormal dilatation of the urinary canal, and its most frequent causes are physiologic and transitory hydronephrosis, with spontaneous resolution. Pelvi-ureteric junction obstruction represents its first pathological cause. Renal function worsening is its major complication. During pre and post-natal periods ultrasound is the election diagnostic method for this entity. The two most accepted criteria to defining hydronephrosis are Society for Fetal Urology grade and anterior-posterior d…
