An 8-year-old girl presented with severe muscular weakness, peripheral neuropathy, ataxia, fever and macrocytic anaemia. Clinically, vitamin B(12) (cobalamin) deficiency was considered. Despite the lack of pre-treatment laboratory confirmation of the diagnosis, a therapeutic trial of hydroxocobalamin injections was begun. After several days, a partial clinical response was seen. Within 5 months all symptoms had resolved. After treatment was initiated, laboratory analysis of pre-treatment blood samples confirmed the presence of vitamin B(12) deficiency. Auto-antibodies to intrinsic factor and p

Methylmalonic acidemia is characterized by the inability to convert L-methylmalonyl-CoA to succinylCoA, resulting in the accumulation of methylmalonic acid in the blood and/or urine. In its chronic form, it can cause cardiomyopathy with impaired cardiac function. Its management is based on several pillars; the most important is intramuscular hydroxycobalamin supplementation, which serves as a cofactor for methylmalonyl-CoA mutase. We present the case of a 6-month-old girl who was admitted to the emergency department with signs of heart failure. A multidisciplinary evaluation was performed to d

Vasoplegia is a condition characterized by persistent low systemic vascular resistance despite a normal or high cardiac index, resulting in profound and uncontrolled vasodilation. Vasoplegia may occur due to various conditions, including cardiac failure, sepsis, and post-cardiac surgery. In the cardiac cohort, multiple risk factors for vasoplegia have been identified. Several factors contribute to the pathophysiology of this condition, and various mechanisms have been proposed, including nitric oxide, adenosine, prostanoids, endothelins, the renin-angiotensin-aldosterone system, and hydrogen s

Intoxications with sodium azide are rare and in almost all cases lethal in doses above 700 mg or 10mg/kg. We report a case of a patient who ingested 2 grams of sodium azide as a suicide attempt. Sodium azide irreversible blocks cytochrome C oxidase by inhibiting oxidative phosphorylation leading to cell death. There is currently no antidote available. Our patient was treated with a range of therapies, on site, in the emergency department and in the intensive care unit, such as sodium thiosulphate, methylene blue, intralipid, extensive gastric lavage, whole bowel irrigation combined with pro-ki

Nitrous oxide (NO) is an inhalant that has become increasingly popular as a recreational drug. While it is presumed to be harmless, a number of adverse effects of NO have been described. We discuss the case of a 24-year-old man with no medical history, who initially presented to the emergency department with progressive polyneuropathy caused by vitamin B12 deficiency after NO abuse. Two days after being discharged with hydroxocobalamin supplementation, the patient returned with a severe headache, blurry vision and slurred speech. Imaging revealed cerebral venous sinus thrombosis. Hypercoagulab

In most cases megaloblastic anaemias are the sequel of a deficiency of vitamin B12, more infrequently of a deficiency of folic acid of different etiology. Oriented to frequency and anamnesis the diagnostics follows the leading symptoms of pernicious anaemia (straw colour, glossitis, achlorhydria) and on the basis of special findings in the peripheral blood (hyperchromacia, megalocytosis, much decreased number of reticulocytes, increased iron and bilirubin level) it leads to the proved suspicion of a megaloblastic anaemia. This suspicion is ascertained by the investigation of the bone-marrow, w

Cyanide poisoning can present in multiple ways, given its widespread industrial use, presence in combustion products, multiple physical forms, and chemical structures. The primary target of toxicity is mitochondrial cytochrome oxidase. The onset and severity of poisoning depend on the route, dose, physicochemical structure and other variables. Common poisoning features include dyspnea, altered respiratory patterns, abnormal vital signs, altered mental status, seizures, and lactic acidosis. Our present knowledge supports cyanide poisoning treatment based on excellent supportive care with adjunc

Vitamin B12 deficiency is a common and reversible cause of megaloblastic anemia, but in severe cases, it may mimic thrombotic microangiopathy (TMA), a presentation known as pseudothrombotic microangiopathy (pseudo-TMA). This rare entity is frequently underrecognized and may lead to inappropriate therapies if not promptly identified. We report the case of a 62-year-old man who presented with abdominal pain, distension, and constipation, initially raising concern for intestinal obstruction or malignancy. Laboratory evaluation revealed macrocytic anemia, thrombocytopenia, elevated lactate dehydro

Introduction: cobalamin C (Cbl C) deficiency is the most common defect in intracellular cobalamin metabolism, associated with methylmalonic acidemia and homocystinuria. Its late clinical presentation is heterogeneous and may lead to a diagnostic delay. Case report: we report the case of a 45-year-old man with a 20-year history of chronic kidney disease and recently diagnosed spastic paraparesis, both of unknown origin. Metabolic studies revealed elevated levels of homocysteine and methylmalonic acid in the blood and urine. A genetic study confirmed cobalamin C deficiency. Treatment with hydrox

Vasoplegic syndrome, characterized by low systemic vascular resistance and hypotension in the presence of normal or supranormal cardiac function, is a frequent complication of cardiovascular surgery. It is associated with a diffuse systemic inflammatory response and is mediated largely through cellular hyperpolarization, high levels of inducible nitric oxide, and a relative vasopressin deficiency. Cardiopulmonary bypass is a particularly strong precipitant of the vasoplegic syndrome, largely due to its association with nitric oxide production and severe vasopressin deficiency. Postoperative va

BACKGROUND: Remethylation defects are rare inherited disorders in which impaired remethylation of homocysteine to methionine leads to accumulation of homocysteine and perturbation of numerous methylation reactions.
OBJECTIVE: To summarise clinical and biochemical characteristics of these severe disorders and to provide guidelines on diagnosis and management.
DATA SOURCES: Review, evaluation and discussion of the medical literature (Medline, Cochrane databases) by a panel of experts on these rare diseases following the GRADE approach.
KEY RECOMMENDATIONS: We strongly recommend measuring plas

A comparison of the degradation of cyanocobalamin (vitamin B(12)) solutions on exposure to sunlight and artificial light has been made using two concentrations, i.e. 950 mug/ml (corresponding to parenteral solutions) and 95 mu/ml (1:10 dilution) at pH 4.0, 5.5 and 7.0. Cyanocobalamin and the photoproduct, hydroxocobalamin (vitamin B(12b)), have been determined by a two component spectrophotometric method at 550 and 525 nm. Cyanocobalamin loss (950 mug/ml and 95 mug/ml) on exposure to sunlight for 2 hours amounts to 14.7-21.0% and 56.3-81.5% respectively indicating that light intensity is the r

INTRODUCTION: Vasoplegic syndrome (VS), a common complication of patients undergoing cardiac surgery with cardiopulmonary bypass (CPB), can be defined as an abnormally low systemic vascular resistance presenting as profound hypotension and vasodilatory shock despite adequate cardiac output. Multiple risk factors for the development of VS post-CPB have been explored. The purpose of this study is to identify additional patient-specific risk factors associated with VS, including angiotensin-converting enzyme (ACE) inhibitors, angiotensin II receptor blockers, angiotensin receptor/neprilysin inhib
