Eucapnia and hypercapnia in patients with chronic airflow limitation. The role of the upper airway.

In this study, we examined two groups of patients with chronic airflow limitation (CAL) separated according to their awake, stable arterial CO2 level. The aim was to identify factors that may contribute to the development of chronic hypercapnic respiratory failure. Patients with obstructive sleep apnea were excluded from the study. Detailed lifetime histories of smoking, alcohol, and snoring were obtained from all patients together with measurements of lung function and of upper airway size. Thirty-three patients with FEV1 less than 1.5 L were studied, of whom 19 were eucapnic and 14 were hype

Carotid body chemoreceptor and ventilatory responses to sustained hypoxia and hypercapnia in the cat.

To understand the role of carotid chemoreceptor activity in the ventilatory responses to sustained hypoxia (30 min) the following measurements were made in cats anesthetized with alpha-chloralose: (1) carotid chemoreceptor and ventilatory responses to isocapnic hypoxia and to hypercapnia during hyperoxia; (2) carotid chemoreceptor responses to isocapnic hypoxia after dopamine receptor blockade; and (3) ventilatory responses to hypoxia after bilateral section of carotid sinus nerves (CSN). Transition to hypoxia (PaO2 approximately equal to 52 Torr) from hyperoxia gradually increased carotid che

Regulation of Na/H exchange in renal microvillus vesicles in chronic hypercapnia.

Several disturbances of acid-base balance, including chronic metabolic and respiratory acidoses and metabolic alkalosis, are associated with enhanced proximal tubule bicarbonate reabsorption. To determine whether augmented brush border Na/H exchange might mediate enhanced proximal tubule bicarbonate reabsorption in these disorders, we measured Na/H exchange activity in cortical brush border membrane vesicles (BBMV) prepared from rats and rabbits adapted to hypercapnia and other chronic acid-base disturbances. BBMV prepared from control animals and animals with chronic acid-base disturbances we

Effects of pulmonary stretch receptor blockade on laryngeal responses to hypercapnia and hypoxia.

Electromyographic patterns of the diaphragm and a posterior cricoarytenoid (PCA) muscle were evaluated in anesthetized rabbits with and without reversible blockade of pulmonary stretch receptors by inhalation of 200 ppm sulfur dioxide (SO2). Inspiratory activity of both muscles was increased by hypercapnia and by isocapnic hypoxia, with or without receptor blockade. Expiratory diaphragmatic activity was diminished by these stimuli and was likewise not qualitatively affected by SO2 exposure. Expiratory PCA activity was increased by hypercapnia and hypoxia prior to SO2 exposure, but decreased by

Effect of hypercapnia on hypoxic ventilatory drive in carotid body-resected man.

Steplike end-tidal hypoxic drives (PETCO2 = 53 Torr) lasting for 5 min were generated in a group of normal subjects and a group of carotid body-resected subjects when end-tidal CO2 was maintained constant under eucapnic (PETCO2 = 39 Torr) and hypercapnic (PETCO2 = 49 Torr) conditions. The hypoxic ventilatory response of the normal subjects was prompt and significant in eucapnia and was enhanced in the hypercapnic state, evidencing CO2-O2 interaction. In contrast, the carotid body-resected subjects did not respond to eucapnic hypoxia but did demonstrate a small but significant ventilatory respo

Post-Thrombectomy Mild Hypercapnia State Prevents Poor Outcome by Reducing Infarct Progression.

OBJECTIVE: In endovascular therapy (EVT) for stroke, futile recanalization contributes to poor prognosis. Mild hypercapnia may enhance cerebral blood flow and prevent ischemia, but its impact on prognosis in successfully recanalized EVT patients is unclear. METHODS: We retrospectively analyzed 237 patients from the INSPIRE database who underwent successful recanalization at our center (October 2018-March 2022). Patients were grouped by post-EVT PaCO2 levels: high (40-50 mmHg) and low (<40 mmHg). Significant infarct expansion (SIE) was defined as a decrease in ASPECTS or pc-ASPECTS by ≥2 from

Serial monitoring of CO2 reactivity following sport concussion using hypocapnia and hypercapnia.

PRIMARY OBJECTIVE: This study examined the effects of mild traumatic brain injury (mTBI) on cerebrovascular reactivity (CVR). RESEARCH DESIGN: A repeated measures design was used to examine serial changes in CVR. METHODS AND PROCEDURES: Twenty subjects who recently suffered a mTBI were subjected to a respiratory challenge consisting of repeated 20 s breath-holds (BH) and hyperventilations (HV). Testing occurred on days 2 (D2), 4 (D4) and 8 (D8) post-injury as well as a baseline (BASE) assessment (after return-to-play). Transcranial Doppler was used to assess mean cerebral blood velocity (vMC

Neuronal death during combined intermittent hypoxia/hypercapnia is due to mitochondrial dysfunction.

Breathing-disordered states, such as in obstructive sleep apnea, which are cyclical in nature, have been postulated to induce neurocognitive morbidity in both pediatric and adult populations. The oscillatory nature of intermittent hypoxia, especially when chronic, may mimic the paradigm of ischemia-reperfusion in that tissues and cells are exposed to episodes of low and high O(2) and this may lead to oxidant stress. Therefore, we decided to explore the potential contribution of oxidant stress in our intermittent hypoxia/hypercapnia animal model and the role that mitochondria might play in this

Cerebral blood flow and dynamic cerebral autoregulation during ethanol intoxication and hypercapnia.

More than one-third of patients diagnosed with head injury are intoxicated with ethanol. Most clinical and animal studies have shown alcohol to have a deleterious impact in the setting of cerebrovascular trauma; however, there are also data showing neuroprotective effects in low ethanol doses. Human studies using imaging modalities suggest that small doses of alcohol produce cerebral vasodilatation and higher doses cerebral vasoconstriction. The aim of this study was to investigate the effect of ethanol intake on dynamic cerebral autoregulation and velocities in the middle cerebral arteries, a

Does hypercapnia-induced cerebral vasodilation modulate the hemodynamic response to neural activation?

Increases in cerebral blood flow produced by vasoactive agents will increase blood oxygen level-dependent (BOLD) MRI signal intensity. The effects of such vasodilation on activation-related signal changes are incompletely characterized. The two signal changes may be simply additive or there may be more a complex interaction. To investigate this, BOLD MRI was performed in four normal male subjects using T2*-weighted echo planar imaging; brain volumes were acquired every 6.2 s, using a Siemens VISION scanner operating at 2 Tesla; each volume consisted of 64 sequential transverse slices (64 x 64

The Effects of Targeted Mild Hypercapnia on Right Ventricular Function After Out-of-Hospital Cardiac Arrest: A Substudy of the Targeted Therapeutic Mild Hypercapnia After Resuscitated Cardiac Arrest Trial.

BACKGROUND: Targeting hypercapnia during invasive mechanical ventilation with subsequent respiratory acidosis may impair right ventricular (RV) function and cause RV failure. RV dysfunction is common after cardiac arrest and may be associated with poor outcomes. RESEARCH QUESTION: Does targeting mild hypercapnia after out-of-hospital cardiac arrest (OHCA) adversely affect RV function, and is RV failure after OHCA associated with increased mortality? STUDY DESIGN AND METHODS: In this single-center, preplanned substudy of the Targeted Therapeutic Mild Hypercapnia After Resuscitated Cardiac Arr

Hypercapnia dissociates neuronal and hemodynamic responses impairing neurovascular coupling and functional brain connectivity.

Neurovascular coupling (NVC) underpins the interpretation of hemodynamic signals as proxies for neural activity, yet its response to metabolic perturbations remains poorly understood. Here, we leverage concurrent fluorescence calcium imaging and functional magnetic resonance imaging in mice expressing genetically encoded calcium indicators to dissect how elevated CO₂ levels reshape the interplay between neural and vascular responses. Our findings indicate that hypercapnia induces opposing trends in calcium and blood-oxygen-level-dependent (BOLD) responses, accompanied by global desynchronizati

Oxidative stress suppresses internal carotid artery dilation to hypercapnia in healthy older adults.

Cerebrovascular disease and dementia risk increases with age, and lifetime risk is greater in women. Cerebrovascular dysfunction likely precedes cerebrovascular disease and dementia but the mechanisms are incompletely understood. We hypothesized that oxidative stress mediates cerebrovascular dysfunction with human aging. Internal carotid artery dilation (ICACO2 dilation) and middle cerebral artery cerebrovascular reactivity (MCA CVRCO2) in response to hypercapnia (5% CO2) were measured in 20 young [10 F/10 M; age 23 ± 3 yr (means ± SD)] and 21 older (11 F/10 M; age 69 ± 9 yr) adults during int

[Non-invasive positive pressure ventilation for residual OSAHS with hypercapnia: a case report].

This case report outlines the treatment of an 11-year-old female who underwent adenotonsillectomy six years ago for snoring but experienced postoperative inefficacy. Her symptoms worsened two weeks before readmission, with increased snoring and sleep apnea, disabling her from lying down to sleep. She was readmitted on December 1, 2023, and diagnosed with severe obstructive sleep apnea hypopnea syndrome and hypercapnia. Automatic BiPAP alleviated her symptoms, with sleep breathing parameters normalizing during treatment. Follow-up at one month showed significant acceleration in her growth and r

Efficacy of High-Flow Nasal Cannula Oxygen Therapy in Patients with Mild Hypercapnia.

OBJECTIVES: To investigate the indications of high-flow nasal cannula (HFNC) oxygen therapy among patients with mild hypercapnia and to explore the predictors of intubation when HFNC fails. METHODS: This retrospective study was conducted based on the Medical Information Mart for Intensive Care IV (MIMIC-IV) database. Adult patients with mild hypercapnia (45 < PaCO2 ≤ 60 mmHg) received either HFNC or non-invasive ventilation (NIV) oxygen therapy. Propensity score matching (PSM) was implemented to increase between-group comparability. The Kaplan-Meier method was used to estimate overall surviva

Hypercapnia versus normocapnia for emergence from desflurane anaesthesia: Single-blinded randomised controlled study.

BACKGROUND: Rapid emergence from general anaesthesia is desirable only if safety is not sacrificed. Mechanical hyperventilation during hypercapnia produced by carbon dioxide infusion into the inspired gas mixture or by rebreathing was reported to shorten emergence time from inhalation anaesthesia. OBJECTIVES: To test the hypothesis that hypercapnia produced by hypoventilation before desflurane cessation shortens emergence time from general anaesthesia (primary hypothesis) and reduces undesirable cardiorespiratory events. DESIGN: A single-blinded randomised controlled study. SETTING: A singl

Hypercapnia impaired cognitive and memory functions in obese patients with obstructive sleep apnoea.

Obstructive sleep apnoea (OSA) is a sleep disorder involving repeated nocturnal desaturation and sleep fragmentation. OSA can result in decreased daytime alertness and neurocognitive dysfunction. Hypercapnia status is also related to neurocognitive dysfunction in patients with pulmonary diseases. We evaluated the effects of hypercapnia on cognitive performance and memory function in a prospective case-controlled study. We enrolled thirty-nine obese patients with OSA and collected their arterial blood samples. All the participants provided arterial blood samples, and completed two questionnaire

Behavioural responses to environmental hypercapnia in two eusocial species of African mole rats.

Damaraland and naked mole rat are eusocial mammals that live in crowded burrows in which CO2 is elevated. These species are thought to be highly tolerant of CO2 but their behavioural responses to hypercapnia are poorly understood. We hypothesized that Damaraland and naked mole rats would exhibit blunted behavioural responses to hypercapnia and predicted that their activity levels would be unaffected at low to moderate (2-5%) CO2 but increased at > 7% CO2. To test this, we exposed Damaraland and naked mole rats to stepwise increases in environmental CO2 (0-10%) and measured activity, explorator
