In no situation is perioperative patient advocacy more important than when a smoothly running case is interrupted by an episode of malignant hyperthermia (MH). An understanding of the demographics, manifestations, and clinical interventions can enable the perioperative nurse to respond in an appropriate manner.

The incidence, etiology, clinical manifestations, and management of malignant hyperthermia (MH) are reviewed. The syndrome of MH is recognized as one of the causes of anesthesia-related deaths. It is considered pharmacogenetic because both an abnormal gene and precipitating environmental factors are necessary to produce an acute reaction. Metabolic defects, involving a derangement of calcium dynamics, appear to be the common characteristic of susceptible individuals. Calcium release and uptake from the sarcoplasmic reticulum is altered when an individual with MH is exposed to certain anestheti…

Humans maintain core body temperature via a complicated system of physiologic mechanisms that counteract heat/cold fluctuations from metabolism, exertion, and the environment. Overextension of these mechanisms or disruption of body temperature homeostasis leads to bodily dysfunction, culminating in a syndrome analogous to exertional heat stroke (EHS). The inability of this thermoregulatory process to maintain the body temperature is caused by either thermal stress or certain drugs. EHS is a syndrome characterized by hyperthermia and the activation of systemic inflammation. Several drug-induced…

OBJECTIVES: This study was conducted to describe the characteristics and outcomes of patients who presented to the emergency department (ED) with presumed environmental hyperthermia.
METHODS: A retrospective chart review was performed in 2 institutions with patients who were seen in the ED and had a discharge diagnosis of hyperthermia, heat stroke, heat exhaustion, or heat cramps. Exclusion criteria were an alternative diagnosis potentially explaining the hyperthermia (pneumonia, etc). Research assistants, who were blinded to the purpose of the study, performed a systematic chart review after…

MH is a life-threatening crisis that can be controlled if promptly recognized and appropriately treated. Identification of patients at risk for MH is an ongoing crusade. Preparedness with needed supplies available at hand facilitates optimal treatment. All perioperative personnel should remain current in their knowledge of MH management and treatment to ensure the best possible outcome for their patients.

PURPOSE: To describe two cases of hyperthermia occurring after cardiac surgery.
CLINICAL FEATURES: At the end of cardiopulmonary bypass, protamine was injected to reverse heparin. Following protamine, hypotension, pulmonary hypertension and hypoxia developed, but protamine induced hypotension recovered after administration of methyl predonisolone and catecholamines. Rectal temperature increased to 41 degrees C in the first case and 40 degrees C in the second after recovery from protamine induced hypotension. Arterial blood gas analysis, breathing 100% oxygen, showed severe acidosis and hyperc…
