[Therapy of hypertonic dehydration].

Hypertonic dehydration developed in 30 patients (age ranged from 17 days to 28 months) with acute gastroenteritis. Rehydration was achieved with i.v. solutions, early potassium replacement, and especially with oral solutions (WHO-solution: 90 mval/l Na+ content). The average amount of sodium and fluids was 23.2 mval/kg/d and 270 ml/kg/d respectively to cause an average drop in sodium of 0.6 mval/h. There were no CNS-complications or deaths.

Morphology of sickle cells produced in solutions of varying osmolarities.

The effect of varying osmolarities (0.6% to 1.5% NaCl solutions, 213 to 492 mOsm/kg H2O) on the morphology of deoxygenated sickle cells was … were formed in hypotonic and isotonic solutions, deformation was slower in these solutions than in hypertonic solutions. These results indicate that the shape and the degree of deformation of deoxygenated sickle cells are highly dependent on the osmolarity of the suspending medium and that the rate of deformation is inversely related to osmolarity. The relationship b …

[Etiology of hypertonic dehydration (author's transl)].

A revision of the ethiology in hypertonic dehydration of 80 infants admitted in the Clínica Infantil "La Paz", between 1972--1973 is made. The most important ethiological factors were: hyperconcentrated food (86% in neonates), vomiting, diarrhoea, increase of insensible water loss (respiratory, fever). The etiopathogenic and physiopathologic bases of hyperconcentrated diet in the development of hypertonic dehydration in accordance with "solute overload" to neonatal kidney are discussed. The autoperpetuation mechanisms of hypertonic dehydration are also discussed.

Stimulation of guinea-pig tracheal afferent fibres by non-isosmotic and low-chloride stimuli and the effect of frusemide.

1. Inhalation of low-chloride or non-isosmotic solutions evokes cough or reflex bronchoconstriction in humans that is inhibited by frusemide (furosemide), whilst capsaicin-evoked cough is unaffected. Here we have examined the responses of single vagal afferent fibres innervating the isolated guinea-pig trachea to these stimuli, and tested the effect of frusemide on fibre responses. 2. Both distilled water and hypertonic saline applied for 30 s onto identified receptive fields produced marked excitation of all A delta and C fibres tested. Isotonic glucose, a low-chloride solution, was a less po

Hypertonic cryohemolysis of pathologic red blood cells.

Human erythrocytes suspended in hypertonic solutions undergo hemolysis when the temperature of the suspension is changed from 37 degrees C toward 0-4 degrees C. It has been suggested that the hypertonic environment causes some proteins of the skeletal network to be changed in such a way that their normal adaptation to temperature changes is prevented, thus resulting in cryohemolysis. In the present study, we compared the cryohemolysis of some pathologic red blood cells in hypertonic sucrose and NaCl to normal cells. Erythrocytes of hereditary spherocytosis (HS) were found to be significantly m…

Pulmonary oedema during treatment of acute water intoxication.

Acute water intoxication with deepening coma and uncontrolled epileptiform seizures in a 25-year-old previously fit male schizophrenic was treated with hypertonic (2 N) saline and a 20% mannitol solution. This improved his neurological state but precipitated severe pulmonary oedema. Intravenous frusemide increased his urinary output sufficiently to clear the pulmonary oedema. In acute water intoxication the use of hypertonic solutions may thus precipitate left heart failure by expanding the intra-pulmonary blood volume beyond the capacity of even a healthy left ventricle to compensate. Simple …

Centrally administered losartan inhibits the reduction in plasma renin concentration caused by intracerebroventricular hypertonic saline in Na-depleted sheep.

Infusions (at 1 ml/h) of two different hypertonic solutions were made into the lateral cerebral ventricle of Na-depleted sheep and the effect on plasma renin concentration (PRC) was investigated. Intracerebroventricular (i.c.v.) infusion of 0.75 mol/l NaCl caused a large reduction in plasma renin concentration whereas i.c.v. infusions of hypertonic 1.2 mol/l sorbitol in 0.15 mol/l NaCl or isotonic artificial cerebrospinal fluid did not. An i.c.v. infusion of the angiotensin AT1 receptor antagonist losartan (1 mg/ml per h for 2.5 h) commencing 1 h prior to i.c.v. 0.75 mol/l NaCl prevented the r…

Effect of hypertonicity on augmentation and potentiation and on corresponding quantal parameters of transmitter release.

Augmentation and (posttetanic) potentiation are two of the four components comprising the enhanced release of transmitter following repetitive nerve stimulation. To examine the quantal basis of these components under isotonic and hypertonic conditions, we recorded miniature endplate potentials (MEPPs) from isolated frog (Rana pipiens) cutaneous pectoris muscles, before and after repetitive nerve stimulation (40 s at 80 Hz). Continuous recordings were made in low Ca2+ high Mg2+ isotonic Ringer solution, in Ringer that was made hypertonic with 100 mM sucrose, and in wash solution. Estimates were

Severe Hypernatremia During Hydatid Cyst Surgery: An Anusual Cause Of Acute Abdomen.

Treatment of liver hydatid cysts is still in most cases surgical. To avoid the recurrence of hydatid cysts injection of scolicidal products inside the cystic cavity is an important step in the surgical procedure. Many scolicidal solutions are used. Hypertonic Saline Solution (HSS) is widely used by surgeons; however, there is a risk of hypertonic saline resorption and acute hypernatremia. Iatrogenic hypernatremia can be life-threatening. We report three cases of hypernatremia secondary to HSS injection for hydatid cyst disease treatment. The objective of this study was to discuss the clinical

Effects of intraocular miotics on cultured bovine corneal endothelium.

Two cases of severe corneal oedema occurred after the use of intraocular pilocarpine. Experimental investigations were conducted with cultured bovine corneal endothelial cells exposed for 5 minutes to 1% pilocarpine solutions of varying composition. Cells were destroyed in solutions not isotonic with aqueous humour, and calcium-free ionic solutions caused loss of cell adhesion without loss of viability. Low pH or the presence of 1% pilocarpine had no detectable effects; 1% acetylcholine chloride in 5% mannitol (Miochol) also caused cell destruction, and this preparation was found to be conside

Hypertonic saline in critical illness - A systematic review.

INTRODUCTION: The optimal approach to fluid management in critically ill patients is highly debated. Fluid resuscitation using hypertonic saline was used in the past for more than thirty years, but has recently disappeared from clinical practice. Here we provide an overview on the currently available literature on effects of hypertonic saline infusion for fluid resuscitation in the critically ill. METHODS: Systematic analysis of reports of clinical trials comparing effects of hypertonic saline as resuscitation fluid to other available crystalloid solutions. A literature search of MEDLINE and

Regulatory volume increase in epithelial cells isolated from the mouse fourth ventricle choroid plexus involves Na(+)-H(+) exchange but not Na(+)-K(+)-2Cl(-) cotransport.

The aim of this study was to determine the ability of choroid plexus epithelial cells to volume regulate when exposed to hypertonic solutions, and furthermore to identify the ion transporters involved in any volume regulation. Experiments were performed on cells freshly isolated, using the enzyme dispase, from the mouse fourth ventricle choroid plexus. Cell volume was measured using a video-imaging method. Cells used in this study were all of a similar morphology and had a mean volume of 0.71pl. Cells shrank when superfused with hypertonic solutions to a minimum relative cell volume of 0.84+/-…

Clinical Use of Hypertonic Lactate-Current Evidence and Emerging Perspectives.

CONTEXT: Lactate has traditionally been viewed as a metabolic byproduct associated with metabolic acidosis, exercise-induced fatigue, and cancer metabolism. However, accumulating evidence highlights its valuable role as an alternative fuel and a key signaling molecule. Hypertonic sodium lactate infusions have gained interest for their potential therapeutic applications, particularly in neurology and cardiology. This review examines the established and potential clinical uses of exogenous hypertonic lactate treatment. EVIDENCE ACQUISITION: A literature search was conducted in PubMed and Medlin

Intravital imaging of mucus transport in asthmatic mice using microscopic optical coherence tomography.

Asthma is one of the most common chronic diseases. Mucus overproduction is consistently linked to asthma morbidity and mortality. Despite the knowledge of the importance of mucus, little data exist on how mucus is transported in asthma and the immediate effects of therapeutic intervention. We therefore used microscopic optical coherence tomography (mOCT) to study spontaneous and induced mucus transport in an interleukin-13 (IL-13)-induced asthma mouse model and examined the effects of isotonic (0.9% NaCl) and hypertonic saline (7% NaCl), which are used to induce mucus transport in cystic fibro

[Treatment of severe hypovolemia with hypertonic saline].

Hypertonic saline (HS) has been used since 1980 in the treatment of traumatised, hypotensive patients. Studies show more pronounced and stable increments in blood pressure and reduced mortality in various subgroups of patients treated with HS compared to isotonic fluids. When infused in the setting of hypovolaemia HS instantaneously expands plasma volume and improves central haemodynamics through increased filling pressures and direct myocardial stimulation. HS, as compared to isotonic fluids, causes precapillary vasodilation, reduction of cell oedema and decreased haematocrit and hence improv

Resuscitation of pulmonary contusion: hypertonic saline is not beneficial.

We postulated that hypertonic solutions could minimize the accumulation of lung water and subsequent respiratory derangements that occur after pulmonary contusion. Anesthetized pigs underwent contusion of the right chest at baseline and then were hemorrhaged (30 cc/kg) over 20 min. They were resuscitated with either 7.5% NaCl (4 cc/kg) or .9% saline (90 cc/kg) for 20 min and observed for 4 h. Gravimetric lung weights and spiral computed tomography scans were used to quantitate lung water. The hemodynamic response to contusion and hemorrhage was similar in both resuscitation groups. Arterial ox…

Urea-prostaglandin versus hypertonic saline for instillation abortion.

Authorities have suggested use of a combination of hyperosmolar urea and low-dose prostaglandin F2 alpha as a second-trimester intra-amniotic abortifacient to avoid the disadvantages of hypertonic saline solution. To examine the safety and efficacy of urea-prostaglandin compared with the instillation of saline solution, we analyzed data from a prospective multicenter study conducted in the United States between 1975 and 1978. Both agents were highly effective in producing an abortion. However, urea-prostaglandin had a significantly lower rate of serious complications when compared with saline
