Asphyxia due to laryngeal spasm as a severe complication of awake deep brain stimulation for Parkinson's disease: a case report.

BACKGROUND: In accordance with German neurosurgical and neurological consensus recommendations, lead placements for deep brain stimulation (DBS) in patients with Parkinson's disease (PD) are usually performed with the patient awake and in "medication off" state. This allows for optimal lead position adjustment according to the clinical response to intraoperative test stimulation. However, exacerbation of Parkinsonian symptoms after withdrawal of dopaminergic medication may endanger the patient by inducing severe "off" state motor phenomena. In particular, this can be a problem in awake craniot

Minocycline-induced black bone disease encountered during total knee arthroplasty.

Finding discolored bone intraoperatively can be confusing and concerning to orthopedic surgeons. Multiple causes of pigmented bone exist, including ochronosis, metabolic bone diseases, metal deposits, sequestrum, metastatic disease, and minocycline use. Bone quality is an important consideration in intraoperative decision making with respect to components and fixation options in total joint arthroplasty. Abnormal bone encountered in routine arthroplasty can raise concerns over the integrity and healing potential of the bone when the etiology is uncertain.Minocycline is a drug routinely used fo

[Huge falx meningioma resected en bloc following acute brain swelling: a case report].

Surgery of a meningioma is composed of four essential consecutive steps; devascularization, detachment, debulking, and dissection. However, this is not the case with a huge meningioma in which circumferential devascularization and detachment may be difficult to complete before debulking is attempted. We report a case of a 37-year-old female presented headache, memory disturbance, and character change and sustaining a huge falx meningioma, with hypervascular appearance. Intraoperatively her blood pressure decreased to 45/30 mmHg due to profuse bleeding caused by "premature debulking" followed b

Rare Variant of the Radial Artery Causing Compression of the Superficial Radial Nerve: A Case Report.

CASE: This case report describes a patient with paresthesia in the distribution of the superficial sensory branch of the radial nerve that was treated with surgery. Intraoperatively, there was a unique cause of internal compression by a rare superficial radial artery variant running adjacent to it. The nerve was mobilized from the artery with fascial releases. The patient had symptom resolution postoperatively. CONCLUSION: To our knowledge, this cause of compression has not been described before and should be considered in a differential diagnosis. In addition, clinicians should be aware of t

Aberrant hepatic arterial anatomy and the whipple procedure: lessons learned.

Appreciation and study of hepatic arterial anatomical variability is essential to the performance of a pancreaticoduodenectomy to avoid surgical complications such as bleeding, hepatic ischemia/failure, and anastomotic leak/stricture. Awareness of this variability permits the surgeon to adapt the surgical technique to deal with anomalies identified preoperatively or intraoperatively thereby preventing unnecessary surgical morbidity and mortality. The objective of our study is to provide a comprehensive review of the anatomic arterial anomalies and discuss surgical strategies that will equip th

Coronary vasospasm during anesthesia induction: awareness, recognition, possible mechanisms, anesthetic factors, and treatment.

We describe a patient who developed severe ST-segment elevation, pulseless ventricular tachycardia, and cardiac arrest during induction of general anesthesia. This transient ST-segment elevation may have been the result of coronary artery spasm. Awareness of this variant of coronary artery vasospasm--Prinzmetal angina--in patients without clear symptoms or established ischemic heart disease, is the key to successful outcome. Recommended preparations and treatments are preoperative calcium channel blockers, nitroglycerin available intraoperatively, reduction of endogenous or exogenous catechola

High incidence of intraoperative calcar fractures with the cementless CLS Spotorno stem.

INTRODUCTION: This study reports on the incidence of intraoperative calcar fractures with the cementless Spotorno (CLS) stem, and the potential role of a learning curve and implant positioning is investigated. METHODS: After introduction of the CLS stem, 800 consecutive cementless total hip arthroplasties (THA) were analyzed. The incidence of calcar fracture in the first 400 THA was compared with the second 400 THA, in order to study a potential learning curve effect. According to the instruction for users, varus positioning of the stem was avoided and a femoral neck osteotomy was aimed relat

Atraumatic vertebral artery dissection after cervical corpectomy: a traction injury?

STUDY DESIGN: Case report with review of the literature. OBJECTIVES: Presented is the first case of vertebral artery dissection secondary to intraoperative traction in cervical spine surgery. The pathogenesis and management of vertebral dissection in the immediate postoperative period are reviewed in detail. SUMMARY OF BACKGROUND DATA: Vertebral artery dissection is commonly associated with direct trauma, atraumatic or spontaneous. There are numerous reports of direct injury to the vertebral artery with cervical spine surgery, and it is a well-recognized risk. Cervical traction is also used

Intraoperative identification of a left non-recurrent laryngeal nerve by neuromonitoring: a critical anatomical finding-case report.

Injury to the recurrent laryngeal nerve (RLN) and its non-recurrent variant (NRLN) remains one of the most significant complications of thyroid and parathyroid surgery. The NRLN is a rare anatomical variation that arises directly from the cervical vagus nerve without looping into the thoracic cavity, most often associated with an aberrant subclavian artery. Identification of this structure is essential to avoid iatrogenic injury, particularly during thyroidectomy. We report the intraoperative neuromonitoring (IONM) identification of an unexpected non-recurrent left inferior laryngeal nerve; em

Surgical Challenges in Papillary Thyroid Cancer Patient With Kartagener's Syndrome: A Case Report.

Kartagener's syndrome is a rare disorder defined by situs inversus, chronic sinusitis, and bronchiectasis, with mirror-image anatomy that may alter recurrent laryngeal nerve course and vascular structures. No previous reports have described thyroid cancer surgery details in this population. We present a female patient with Kartagener's syndrome who underwent right thyroidectomy and lymph node dissection for papillary thyroid carcinoma. Histopathology confirmed nodal metastasis, and intraoperative findings revealed a normal recurrent laryngeal nerve but a variant transverse cervical artery. Thi

Mapping visuospatial and self-motion perception functions in the left parietal lobe.

Parietal lobe functions include somesthesia, language, calculation, self-motion perception, and visuospatial awareness. In this video, the authors show the intraoperative mapping of a left parietal lobe for a low-grade glioma resection. Standard sensory and language mapping were performed. Interestingly, by using the "Line Bisection" task, subcortical stimulation of the gyrus angularis was repeatedly associated with ipsilateral spatial neglect, often described in the right parietal lobe. In a similar way, subcortical stimulation in a more posterior point elicited episodes of vertigo, probably

External iliac artery thrombosis after open reduction of an acetabular fracture: a case report.

Major arterial injury from open reduction and internal fixation of an acetabular fracture through an ilioinguinal approach is rare. Detection of a major intraoperative arterial injury is simple. Detection of a more occult injury leading to thrombosis is not. We report the third known case of thrombosis of the external iliac artery following open reduction and internal fixation of an anterior column fracture. This case is of particular interest because it highlights the ability of young patients to compensate for major vessel occlusion and still maintain a well-perfused foot. Heightened awarene

Arthrogram-assisted fixation of slipped capital femoral epiphysis: a CT and radiographic study.

BACKGROUND: Intraoperative fluoroscopy does not always provide the operating surgeon with optimal visualization of a slipped capital femoral epiphysis (SCFE). Arthrography can be used to enhance fluoroscopic images of these patients. This study retrospectively compared the screw placement between patients who received conventional versus arthrographic-assisted in situ screw fixation for SCFE. METHODS: We reviewed the charts and radiographs of all patients diagnosed with a SCFE at our institution from 2005 to 2010. We isolated those who received postoperative computed tomography (CT) scans to

Occurrence of severe hypotension after indocyanine green injection during the intraoperative period.

Indocyanine green (ICG) is generally considered to be safe for IV administration and has a very low incidence of complications. We report 2 cases of severe hypotension immediately after low dose administration of IV ICG. The first case is a 69-year-old woman who developed severe hypotension after a second 5 mg IV bolus of ICG given shortly after the placement of an aneurysm clip during an otherwise uneventful surgery performed under general anesthesia. The second case is a 56-year-old woman with moyamoya disease who became hypotensive subsequent to a 2.5 mg ICG bolus at the conclusion of an ex

Intraoperative Fluoroscopy Exposure: A Systematic, Multi-specialty Review of Occupational Radiation Exposure to Surgeons.

Fluoroscopy is commonly used intraoperatively to obtain real-time procedural imaging. While this technique is integral for patient care, it increases the exposure of surgeons and staff to ionizing radiation. To date, no comprehensive systematic review has examined the radiation risks associated with fluoroscopy or the protective strategies implemented across surgical subspecialties. Following PRISMA (Preferred Reporting Items for Systematic reviews and Meta-Analyses) guidelines, we searched PubMed and EMBASE to identify studies reporting surgeons' intraoperative occupational radiation exposure
