Ambulatory surgery has become an important component in the provision of surgical care. In 1987 a day-case surgical unit was established at Red Cross War Memorial Children's Hospital, Cape Town. Experience over a 6-year period (1987-1992) is reviewed in relation to the nature and total number of surgical procedures, pre-operative preparation, daily utilisation of the facility, postoperative care and complications. During this period 16,538 patients (mean age 3 years) were operated on in the unit. Nine surgical disciplines participated. Pre-operative assessment and preparation reduced the rate

Renoprotective measures are of utmost importance for the preservation of renal function and prevention of acute renal failure of kidneys with impaired function subjected to operative procedures. Preoperatively a thorough nephrological work-up and preparation of the patient has to be performed. Potentially nephrotoxic drugs should be avoided. Examinations with radio contrast agents may lead to a deterioration of renal function, especially if risk factors are present. Hydration is an important part of preoperative renoprotection. Intraoperative renoprotective measures include atraumatic surgical

The authors relate the anesthetic and pre-, per- and post-operative care problems they had to meet with six pairs of siamese twins separated at the Hôpital des Enfants Malades (Paris), between 1960 and 1978. 77 cases of such anomalies have been reported since the beginning of the XXth century. The authors point out the importance of: 1) a precise pre-operative assessment of the anatomo- and physiopathological status of the conjoined twins, 2) an outstanding technical organization, 3) a medical and nursing team specialized in pediatric anesthesia and intensive care to face with the predictable

BACKGROUND: Perioperative hyperglycemia in people with diabetes is associated with increased morbidity, mortality, and health care costs. Despite guideline recommendations to institute interventions to reduce hyperglycemia, standardized protocols that integrate into clinical workflows are lacking. In this article, we evaluate the efficacy of a digitally embedded, glycemic management protocol in people with diabetes undergoing surgery.
METHODS: We conducted a retrospective analysis of a quality improvement study conducted at a tertiary-care academic hospital. Adults with diabetes undergoing no

OBJECTIVES: To evaluate the independent association of a standardized, multi-modal intraoperative nursing protocol on postoperative complications in patients undergoing open radical gastrectomy for gastric cancer.
METHODS: This retrospective cohort study included 89 patients who underwent open radical gastrectomy. Patients were categorized into a standardized care group (n=44) receiving a multi-modal nursing protocol and a non-standardized care group (n=45). The primary outcome was 30-day complications (Clavien-Dindo ≥II). Multivariable logistic regression adjusted for confounders.
RESULTS:

PURPOSE: To evaluate whether intraoperative patient positioning strategies improve stone-free outcomes and safety compared with standard lithotomy during retrograde intrarenal surgery (RIRS) for renal calculi.
METHODS: PubMed, Embase, Cochrane Library, Web of Science, CNKI, Wanfang, and SinoMed were searched from inception to December 2025. Comparative studies of intraoperative positioning strategies versus standard lithotomy during RIRS were included. Risk ratios (RRs) and mean differences (MDs) with 95% confidence intervals (CIs) were calculated using random-effects models where quantitativ

Advances in knowledge have significantly enhanced our understanding of the structure and function of microcirculation. The tools used to study microcirculation have been refined and diversified. This review presents existing tools for anaesthesiologists and highlights two key areas where microcirculatory knowledge can be applied in anaesthesia: perioperative hemodynamic management and preoperative patient assessment. Preoperative microcirculatory assessment can identify dysfunction associated with chronic conditions and predict perioperative outcomes. This information allows for personalised p

Surgeons and anesthesia clinicians commonly face a hemodynamic disturbance known as intraoperative hypotension (IOH), which has been linked to more severe postoperative outcomes and increases mortality rates. Increased occurrence of IOH has been positively associated with mortality and incidence of myocardial infarction, stroke, and organ dysfunction hypertension. Hence, early detection and recognition of IOH is meaningful for perioperative management. Currently, when hypotension occurs, clinicians use vasopressor or fluid therapy to intervene as IOH develops but interventions should be taken

Dermoscopy of the nail unit (onychoscopy) is a method which allows for non-invasive observation of the nail structures, increasing the accuracy of clinical diagnosis. Currently, it is used in evaluation of both inflammatory and neoplastic conditions of the nail unit. However, in contrast to the skin, the anatomy of the nail unit prevents direct observation of nail bed or nail matrix structure during classic onychoscopy. Intra-operative onychoscopy is a variant of the technique which uses direct visualization of the nail unit structures after nail plate avulsion. The aim of this systematic revi

OBJECTIVES: A wide variety of intraoperative tests are available in cochlear implantation. However, no consensus exists on which tests constitute the minimum necessary battery. We assembled an international panel of clinical experts to develop, refine, and vote upon a set of core consensus statements.
DESIGN: A literature review was used to identify intraoperative tests currently used in the field and draft a set of provisional statements. For statement evaluation and refinement, we used a modified Delphi consensus panel structure. Multiple interactive rounds of voting, evaluation, and feedba

Ulnar nerve dysfunction following distal humerus fractures is a well-recognized phenomenon. There is no consensus regarding optimal handling of the ulnar nerve during surgical management of these fractures between in situ management and transposition. Using an electronic database to identify retrospective studies involving surgical fixation of distal humerus fractures yielded 46 studies, 5 trials meeting the authors' inclusion criteria, totaling 362 patients. An overall incidence of 19.3% for ulnar neuropathy was identified. Of those patients undergoing in situ release, the incidence was 15.3%

PURPOSE: Takotsubo cardiomyopathy (TTCM) is a form of stress cardiomyopathy that may occur in the perioperative period and among the critically ill. Therefore, anesthesiologists should be aware of its diagnosis and treatment. The aim of this narrative review is describe the features of TTCM and its relevance to the practice of anesthesiology.
PRINCIPAL FINDINGS: Takotsubo cardiomyopathy occurs in about 2-9/100,000 persons in the general population annually and may occur in up to one in 6,700 cases in the perioperative period. Takotsubo cardiomyopathy often presents like an acute coronary synd

Radiofrequency ablation is successful in most supraventricular tachyarrhythmias and the first results in percutaneous applications in atrial fibrillation have been published. At present, all the surgical techniques for the treatment of atrial fibrillation use extensive atrial incisions. A case report is presented of a 67 year old woman with chronic atrial fibrillation and rheumatic mitral and tricuspid valve disease. At surgery, pulmonary vein isolation was performed using a continuous linear ablation around the pulmonary veins, instead of using extensive atrial incisions. Concommitantly, a mi

Indications for mechanical ventilation have evolved substantially since widespread use of ventilatory support began in the early 1960s. While the metabolic and blood-gas alterations that mandate institution of ventilatory support have remained unaltered, new noninvasive modes of ventilation have widened the therapeutic options available to patients in acute respiratory failure. An understanding of the effect of mechanical ventilation on other organ systems has clarified the role of mechanical ventilation in the treatment of conditions other than respiratory failure such as stroke or head injur

Malignant hyperthermia (MH) is an acute syndrome that occurs in genetically susceptible persons who have been exposed to certain anesthetic agents. It can also be induced by catecholamines activated by stress. One in 200 people is at risk for developing malignant hyperthermia. The mortality rate may be as high as 15%. A thorough history obtained before surgery is of extreme importance to prevent an MH crisis and in the management of those susceptible to the disorder. If MH is suspected a diagnosis, which can be confirmed by muscle biopsy, should be ruled out before surgery.
