Avoiding vascular complications in insular glioma surgery - A microsurgical anatomy study and critical reflections regarding intraoperative findings.

INTRODUCTION: Vascular lesions in insular glioma surgery can severely impact patients' quality of life. This study aims to present the results of our dissections and authors' reflections on the insular vascular anatomy. MATHERIALS AND METHODS: The insular vascularization was examined using ×3 to ×40 magnification in 20 cadaveric cerebral hemispheres in which the arteries and veins had been perfused with colored silicone. RESULTS: In insular gliomas, this individualization of the anatomical structures is rarely possible, as the gyri are swollen by the tumor and lose their individuality.

Intraoperative pressure monitoring of the lower leg for preventing compression-related complications associated with the lithotomy position.

BACKGROUND: Several serious complications are associated with the lithotomy position, including well-leg compartment syndrome and peroneal nerve paralysis. The aims of this study were to identify risk factors for the intraoperative elevation of lower leg pressure and to evaluate the effectiveness of monitoring external pressure during surgery for preventing these complications. METHODS: The study included 106 patients with a diagnosis of sigmoid colon or rectal cancer who underwent elective laparoscopic surgery between June 2019 and December 2020. We divided the posterior side of the lower le

Wound healing complications with intraoperative brachytherapy for head and neck cancer: a unique form of radiation injury.

BACKGROUND: Intraoperative brachytherapy (IOBT) to the tumor bed coupled with surgery has been shown to increase survival and to improve locoregional disease control after head and neck tumor extirpation. Flap reconstruction attempts to restore patient anatomy, while also covering the radioactive implants. The purpose of this study was to better characterize the wound healing complications experienced by patients undergoing reconstruction in the setting of IOBT after tumor ablation, as well as to identify risk factors predicting complications and the need for reoperation. METHODS: A retrospec

[Tube obstruction in operation of esophageal atresia. Brief review of intraoperative complications based on a case report].

In this case report we discuss the anaesthetic management of newborns with esophageal atresia classified as Vogt III b. This type is characterised by an upper esophageal pouch which ends blindly and a distal tracheoesophageal fistula. Commonly associated diseases are cardiac, renal, vertebral and anal anomalies. The most important intraoperative anaesthesiological complications are acidosis, hypoxaemia, gastric distension, endotracheal tube obstruction, tracheal compression, cardiac arrhythmias and atelectasis. In the presented case an endotracheal tube obstruction with hypercapnia occurred wh

Effects of intraoperative topical hemostatic agents on chest tube output and thoracic complications following vertebral body tethering.

PURPOSE: Vertebral body tethering (VBT) is an emerging treatment option for idiopathic scoliosis, but routinely requires intraoperative placement of a chest tube (CT). Topical thrombin and injectable collagen have routinely been used as a hemostatic agent, but data are lacking on its use in VBT. Thus, we aim to investigate the impact of the topical hemostatic use on CT output and duration, and the subsequent rate of clinically-significant hemothorax and effusion after VBT. METHODS: Retrospective review was conducted on patients who underwent VBT at a single institution between 2015 and 2024.

Intraoperative open lung condition and postoperative pulmonary complications. A secondary analysis of iPROVE and iPROVE-O2 trials.

BACKGROUND: The preventive role of an intraoperative recruitment maneuver plus open lung approach (RM + OLA) ventilation on postoperative pulmonary complications (PPC) remains unclear. We aimed at investigating whether an intraoperative open lung condition reduces the risk of developing a composite of PPCs. METHODS: Post hoc analysis of two randomized controlled trials including patients undergoing abdominal surgery. Patients were classified according to the intraoperative lung condition as "open" (OL) or "non-open" (NOL) if PaO2 /FIO2 ratio was ≥ or <400 mmHg, respectively. We used a multiva

Intraoperative stent placement for the treatment of acute portal vein complications in pediatric living donor liver transplantation.

PURPOSE: Pediatric living donor liver transplantation (LDLT) in low weight recipients remains one of the most complex surgical procedures, with portal vein (PV) complications occurring in up to 19% of cases. When decreased PV flow is diagnosed intra- or perioperatively, intraoperative stent placement is a good substitute for surgical adjustment. Still, at the present moment, little is known about the technical feasibility, safety, efficacy, and long-term outcome of intraoperative stenting in LDLT. METHODS: Between 2006 and 2017, seven pediatric recipients underwent PV stent placement during t

Complications of combined intraoperative radiation (IORT) and external radiation (ERT) of the upper abdomen: an experimental model.

An experimental model in the rabbit is presented which is suitable for analysis of clinically relevant, early side-effects of combined upper abdominal IORT and ERT. Fractionated ERT alone given through an upper abdominal a.-p. field including the entire stomach caused gastric ulcerations within < or = 58 days. Latent times decreased with increasing dose and the ED50 for occurrence of ulcers was 39 +/- 3.3 Gy. Single doses of IORT of 20-40 Gy alone administered through a 2-cm diameter field localized on the coeliac axis and carefully excluding any intestinal mucosa caused neither gastric ulcera

Intraoperative dorsal lung ventilation reduction and postoperative pulmonary complications after laparoscopic colorectal cancer surgery: A prospective cohort study.

BACKGROUND: Postoperative pulmonary complications (PPCs) remain common following surgery and contribute substantially to postoperative morbidity. Reduced ventilation in the dorsal lung regions, reflecting dorsal atelectasis during mechanical ventilation after anesthesia induction, may increase the risk of PPCs. This study aimed to investigate the association between intraoperative reduction in dorsal lung ventilation and the development of PPCs. METHODS: Patients undergoing laparoscopic colorectal cancer surgery were enrolled, and intraoperative regional lung ventilation was assessed using el

Impact of different intraoperative MAP management goals on the development of severe postoperative complications in elderly hypertensive patients.

BACKGROUND: Hypertension is highly prevalent among elderly surgical patients and is associated with impaired intraoperative hemodynamic stability and an elevated risk of postoperative complications. Although mean arterial pressure (MAP) is a key determinant of organ perfusion, the optimal intraoperative MAP target for elderly hypertensive patients has not been clearly defined. METHODS: This retrospective study analyzed clinical data from elderly hypertensive patients (n = 354). Patients were categorized into three groups based on intraoperative MAP: 65–75 mmHg, 75–85 mmHg, and 85–95 mmHg. The
