Ancillary medical therapy: help or hindrance.

Aside from the more traditional methods of treating bronchial asthma, it has been the purpose of this review to consider some of the ancillary modes of therapy and to decide if they have a place in clinical practice. Regarding antihistamines, there is little evidence to support their therapeutic use in the management of asthma, but there is strong evidence for their safety in the treatment of allergic rhinitis or urticaria in asthmatic patients. Iodides have been used for years, but there is no good body of evidence to demonstrate their effectiveness. While they may be helpful in rare cases, o

Magnesium sulfate for acute exacerbations of chronic obstructive pulmonary disease.

BACKGROUND: Chronic obstructive pulmonary disease (COPD) is a chronic and progressive disease, often punctuated by recurrent flare-ups or exacerbations. Magnesium sulfate, having a bronchodilatory effect, may have a potential role as an adjunct treatment in COPD exacerbations. However, comprehensive evidence of its effects is required to facilitate clinical decision-making. OBJECTIVES: To assess the effects of magnesium sulfate for acute exacerbations of chronic obstructive pulmonary disease in adults. SEARCH METHODS: We searched the Cochrane Airways Trials Register, CENTRAL, MEDLINE, Embase

[Effects of a beta 2-agonist, sodium cromoglycate, and an anticholinergic agent on hyperventilation-induced bronchoconstriction in sensitized rabbits].

Hyperventilation can induce bronchoconstriction in ovalbumin-sensitized rabbits. To investigate the roles of the beta-receptor parasympathetic nervous system and of chemical mediators in hyperventilation-induced bronchoconstriction (HIB), the effects of a beta 2-agonist, of sodium cromoglycate, and of an anticholinergic agent on HIB were studied. Rabbits were divided to four groups and treated as follows. Group 1: Control (n = 7, 0.9% saline); Group 2: Procaterol (n = 4, 50 micrograms/l); Group 3: Sodium cromoglycate (n = 4, 10 mg/ml); and Group 4: Ipratropium bromide (n = 6, 1 mg/ml). Each dr

Drug therapy of childhood asthma.

Drug therapy is used to prevent and control asthma, and also to reduce the frequency and severity of its exacerbations, and reverse airflow obstruction. Asthma medications are thus categorized into two general classes--bronchodilators (relievers) and anti-inflammatory drugs (preventers). Short acting beta2-agonists is the therapy of choice for relief of acute symptoms and prevention of exercise induced bronchospasm (EIB). Corticosteroids are the most potent and effective anti-inflammatory medication currently available. Inhaled form is used in the long-term control of asthma. Systemic corticos

Acute exacerbations of chronic bronchitis: focusing management for optimum results.

Acute exacerbations of chronic bronchitis can be recognized clinically by (1) increased cough and dyspnea, (2) a change in character of sputum, and (3) an increase in quantity of sputum. Routine chest radiographs are probably not warranted in initial evaluation. Therapy is aimed at control of inflammation, infection, bronchoconstriction, and mucin production. Corticosteroids improve flow rates in patient with respiratory insufficiency. Antibiotic therapy appears to decrease hospital stay and improve flow rates in patients with bacterial infection, as determined by sputum examination or the pre

Allergic and Nonallergic Rhinitis: Update on Pathophysiology and Clinical Management.

An understanding of the pathophysiology will dictate appropriate therapy for allergic and nonallergic rhinitis. Cholinergic pathways when stimulated produce typical secretions that can be identified by their grandular constituents so as to implicate neurologic stimulation. By contrast secretions typical of increased vascular permeability are found in allergic reactions and upper respiratory infections. In general it is worthwhile to treat early since the nose can be primed so as to produce symptoms at lower doses as the season continues. The mainstay of therapy continues to be antihistamines,

Effect of sodium metabisulphite on bronchial blood flow in conscious sheep: pharmacological modulation.

1. Sodium metabisulphite (MBS) can induce bronchoconstriction in patients with asthma. We investigated the effects of MBS aerosol on bronchial blood velocity (Vbr) and pulmonary resistance in intubated conscious sheep. 2. Bronchial blood velocity was measured by implanting a 20 MHz ultrasonic Doppler flow probe on the common bronchial branch of the bronchoesophageal artery. 3. Inhaled MBS induced a dose-dependent, transient increase in Vbr lasting for a few minutes without any changes in aortic and pulmonary artery pressures. There was some tachyphylaxis of the Vbr response to successive inhal

Acute bronchial asthma.

Acute asthma is the third commonest cause of pediatric emergency visits at PGIMER. Typically, it presents with acute onset respiratory distress and wheeze in a patient with past or family history of similar episodes. The severity of the acute episode of asthma is judged clinically and categorized as mild, moderate and severe. The initial therapy consists of oxygen, inhaled beta-2 agonists (salbutamol or terbutaline), inhaled budesonide (three doses over 1 h, at 20 min interval) in all and ipratropium bromide and systemic steroids (hydrocortisone or methylprednisolone) in acute severe asthma. [Ventilation in special situations. Mechanical ventilation in status asthmaticus].

The indications for mechanical ventilation in status asthmaticus are cardiopulmonary arrest, significant alteration of consciousness, respiratory exhaustion, and progressive respiratory insufficiency despite aggressive bronchodilator treatment. In mechanical ventilation for status asthmaticus, a specific strategy directed at reducing dynamic hyperinflation must be used, with low tidal volumes and long expiratory times, achieved by diminishing respiratory frequency. This ventilatory pattern produces permissive hypercapnia, which is generally well tolerated with suitable sedation. The best metho

Anticholinergic agents in chronic bronchitis and emphysema.

Inhaled anticholinergic agents produce as much absolute increase in airflow in chronic bronchitis as in asthma, namely about 0.2-0.3 litres increase in the FEV1 and a doubling of specific airways conductance. Their major advantage in bronchitis is due to the fact that adrenergic agents are relatively poor bronchodilators in this disorder. In bronchitis and emphysema a large number of studies suggest that anticholinergic agents not only are more potent than adrenergic agents but, when given in optimal dosage, achieve all of the available bronchodilatation in these patients. Possible reasons for

Hyperventilation-induced asthma: evidence for two mechanisms.

The mechanism by which airway cooling induces airflow obstruction in asthmatic subjects has not yet been established. Using a pair of isocapnic hyperventilation challenges, with a 40-minute interval, we looked for the presence of a refractory period in 19 asthmatic patients (aged 9-18 years). The subjects fell into two groups. The eight in the "non-refractory" group showed less than a 25% reduction in response to the second challenge, but the 11 in the "refractory" group showed at least a 35% reduction. Twelve subjects also performed a hyperventilation challenge after cholinergic blockade with

MAGNEsium Trial In Children (MAGNETIC): a randomised, placebo-controlled trial and economic evaluation of nebulised magnesium sulphate in acute severe asthma in children.

BACKGROUND: There are few data on the role of nebulised magnesium sulphate (MgSO4) in the management of acute asthma in children. Those studies that have been published are underpowered, and use different methods, interventions and comparisons. Thus, no firm conclusions can be drawn. OBJECTIVES: Does the use of nebulised MgSO4, when given as an adjunct to standard therapy in acute severe asthma in children, result in a clinical improvement when compared with standard treatment alone? DESIGN: Patients were randomised to receive three doses of MgSO4 or placebo, each combined with salbutamol an

Irritant contact facial dermatitis due to nebulizer therapy.

A case is reported of a patient with long-standing, severe airflow obstruction requiring long-term nebulizer therapy developing a facial dermatitis in the area bounded by the nebulizer mask. The facial dermatitis seems to be the result of the combined irritancy of the nebulizer solutions and moisture, and prophylactic measures are suggested for patients requiring long-term nebulizer therapy.

SCH 1000 in psychogenic asthma.

The effects of a beta 2-sympathomimetic (salbutamol or fenoterol) and the anticholinergic SCH 1000 administered both in combination and with placebo were compared in a double-blind random crossover design. Thirty-three patients with atopic, non-atopic and psychogenic asthma were studied. After inhalation of the sympathomimetic in combination with SCH 1000 or placebo the increase in FEV1 was significantly better for three hours (p less than 0.05) than after inhalation of SCH 1000 alone. There was no difference in the response to SCH 1000 in the atopic or non-atopic groups. However, in patients

Chronic obstructive pulmonary disease.

Outpatient management of chronic obstructive pulmonary disease (COPD) is reviewed in this paper. Smoking cessation is probably important, although its benefit in established COPD is unproven. Bronchodilator therapy may be of more than symptomatic benefit and is indicated in virtually all patients. Specific beta 2-agonists are the most widely used agents and can be given in substantially larger doses than are usually recommended. Ipratropium bromide, an anticholinergic drug, is about as effective as a beta 2-agonist, but in large doses the two drugs do not seem to have additive effects, unlike

Severe intraoperative bronchospasm treated with a vibrating-mesh nebulizer.

Bronchospasm and status asthmaticus are two of the most dreaded complications that a pediatric anesthesiologist may face. With the occurrence of severe bronchospasm and the inability to ventilate, children are particularly vulnerable to apnea and ensuing hypoxia because of their smaller airway size, smaller lung functional residual capacity, and higher oxygen consumption rates than adults. Nebulized medication delivery in intubated children is also more difficult because of smaller endotracheal tube internal diameters. This case demonstrates the potentially lifesaving use of a vibrating-mesh m

Chronic obstructive pulmonary disease treatment options.

OBJECTIVE: To discuss current therapies for the treatment of chronic obstructive pulmonary disease (COPD) and their efficacy and use according to current treatment guidelines. SUMMARY: The overall approach to the management of stable COPD is a stepwise increase in treatment, depending on disease severity. Inhaled bronchodilators are the cornerstone of symptomatic management of COPD and include beta 2-agonists, anticholinergics, and methylxanthines. Short-acting beta 2-agonist bronchodilators (e.g., albuterol) have a quick onset of action and are useful for rescue. Long-acting bronchodilators

Vasomotor rhinitis update.

PURPOSE OF REVIEW: This review was conducted to examine new data on vasomotor rhinitis, a common clinical problem. RECENT FINDINGS: Recent publications highlight advances in the study of the pathophysiology of vasomotor rhinitis. Electron microscopic and ultracytochemical evaluation of the nasal mucosa in vasomotor rhinitis demonstrates an emerging role of neuropeptides and nitric oxide in the pathogenesis of vasomotor rhinitis. Ozone, cigarette smoke, and other environmental factors may trigger neurogenic mechanisms that lead to vasomotor rhinitis. Objective tests have documented the presenc
