Unique case of abdominal aortic aneurysm causing obstructive jaundice and duodenal obstruction.

Obstruction of the duodenum by an abdominal aortic aneurysm (AAA) (aortoduodenal syndrome) has rarely been reported. It presents with symptoms of upper gastrointestinal (GI) tract obstruction. Obstructive jaundice caused by an AAA is also extremely rare and requires high clinical suspicion. We present a unique case with informed consent of an elderly gentleman who presented with both rare presentations over a course of few months. After extensive literature search, we have found case reports of patients showing either of the two unusual presentations, but none were found to show them together.

[Selection of drainage method for resectable cholangiocarcinoma complicated by obstructive jaundice].

OBJECTIVE: To compare the results of percutaneous and endoscopic biliary decompression for resectable perihilar tumor (PT). MATERIAL AND METHODS: Treatment outcomes were analyzed in patients with PT who underwent surgery between 1999 and 2023. The 1st group included patients who underwent percutaneous transhepatic cholangiostomy as the first stage for biliary decompression, the 2nd group - patients who underwent endoscopic biliary drainage. RESULTS: There were 58 patients with percutaneous transhepatic cholangiostomy and 18 ones with endoscopic biliary drainage. Both groups were comparable i

Ampulla of Vater Metastasis From Colon Cancer Presenting As Obstructive Jaundice.

A woman in her late 60s presented with obstructive jaundice. She had mucinous adenocarcinoma of the colon six months ago and underwent surgery followed by adjuvant chemotherapy. A computed tomography scan revealed a prominent ampulla with duodenal wall thickening, dilated intrahepatic biliary radicles, bile ducts, and the main pancreatic duct. Duodenoscopy revealed a bulky, friable, and ulcerated ampulla of Vater (AOV) with partial luminal narrowing. The biopsy and immunohistochemistry (IHC) confirmed adenocarcinoma of colonic origin metastasizing to the AOV. She underwent an endoscopic retrog

Mirizzi Syndrome: The Uncommon and Overlooked Surgical Cause of Obstructive Jaundice.

We present a case of Mirizzi syndrome in a patient who exhibited abdominal pain along with signs of obstructive jaundice. The minimally invasive approach was complicated, necessitating an open laparotomy, which ultimately prolonged the hospital stay. A female in her early 60s with a history of rheumatoid arthritis (on methotrexate) and hypothyroidism presented to the emergency department with jaundice as her chief complaint. Upon arrival, the patient was vitally stable. The only pertinent finding was scleral icterus. Laboratory results revealed AST of 263 U/L, ALT of 233 U/L, ALP of 1246 U/L,

Obstructive jaundice, a rare presentation of lung cancer: A case report.

Lung cancer metastases to the pancreas are rare but potentially life-threatening. Oftentimes, the presence of symptoms is indicative of extensive disease burden. This report describes a case of primary lung adenocarcinoma metastasizing to the head of the pancreas presenting as obstructive jaundice. The patient was a 61-year-old female veteran who presented with a chronic dyspnea, weight loss, and 3-weeks of nausea and vomiting found to have jaundice, elevated alkaline phosphatase levels, hyperbilirubinemia, and transaminitis. Imaging of her chest revealed large pulmonary parenchymal nodules th

An Unusual Case of Obstructive Jaundice Secondary to Pyogenic Liver Abscesses.

Pyogenic liver abscesses (PLA) are an uncommon, but potentially life-threatening infection. Although the link between amebic liver abscesses and obstructive jaundice is well documented, there are few cases of PLA leading to this complication. We present a case of multiple massive PLA and obstructive jaundice on initial presentation. The patient was treated for six weeks with antibiotics and percutaneous drains placed in the largest abscesses and was discharged after clinical improvement and resolution of the hyperbilirubinemia. This case highlights the importance of clinician awareness of othe

Splenic Artery Pseudoaneurysm in Chronic Pancreatitis Causing Obstructive Jaundice: Endovascular Management.

BACKGROUND: Splenic artery pseudoaneurysm (SAP) around the pancreatic head causing obstructive jaundice is an extremely rare complication but can be life threatening once occurs. This case report is to raise awareness of this catastrophic complication and share our experience of successful endovascular management. METHODS: A 47-year-old male with a history of chronic pancreatitis clinically presented with epigastric pain and jaundice. Proximal SAP complicated with obstructive jaundice was confirmed by laboratory and imaging investigations. The SAP was successfully treated by transarterial coi

The application of Compont gel in chronic obstructive jaundice rats model.

PURPOSE: To establish a new rat model, the pathogenesis of which is closer to the clinical occurrence of chronic obstructive jaundice with liver fibrosis. METHODS: 90 SD rats were randomly divided into 3 groups. Group A common bile duct ligation, group B common bile duct injection compont and group C injection saline. The serum of three groups was extracted, and the liver function was detected by ELISA. HE staining, Masson staining and immunohistochemistry were used to detect liver pathology. RESULTS: Group B showed a fluctuant development of jaundice, obstructive degree reached a peak at 2 …

Bile and circulating HMGB1 contributes to systemic inflammation in obstructive jaundice.

BACKGROUND: Obstructive jaundice (OJ) patients with cholangitis are prone to sepsis; however, the underlying mechanisms are still not clear and need to be clarified. METHODS: Analyzing all available published data related to the title of this article. RESULTS: OJ leads to absence of gut luminal bile and accumulation of hepatic and circulating bile acids. Absence of gut luminal bile deprives the gut from its antiinflammatory, endotoxin-binding, bacteriostatic, mucosal-trophic, epithelial tight-junction maintaining, and gut motility-regulating effects, leading to gut bacterial overgrowth, muco

Distal Common Bile Duct Tuberculosis with Obstructive Jaundice: A Case Report.

Tuberculosis (TB) isolated in Common Bile Duct (CBD) is very rare and its treatment is somewhat controversial. We report a case of distal CBD tuberculosis diagnosed as cholangiocarcinoma. A 40 year old man presented with obstructive jaundice without abdominal pain, fever or weight loss. Ultrasonography and Magnetic Resonance Cholangio-Pancreatography (MRCP) indicated a mass lesion in the Distal CBD, which caused structure of the distal common bile duct. As Cholangiocarcinoma was suspected, he underwent Whipple procedure (pancreaticoduodenectomy). Histological examination of resected specimen r

Ampullary Metastasis From Breast Cancer: A Rare Cause of Obstructive Jaundice.

Breast cancer is the most common tumor in women and the first cause of death for malignancy in the female. Bile ducts are not among the common sites of metastasis from breast cancer. Few cases of obstructive jaundice due to metastatic breast cancer have been described in the literature and they mostly resulted from widespread liver metastases that eventually involved the bile ducts. We report an exceptional case of ampullary metastasis in the absence of liver metastases. Sporadic reports have been published about the involvement of the ampulla by breast cancer metastasis. This case emphasizes

A presenting with obstructive jaundice in pulmonary adenocarcinoma: a case report.

INTRODUCTION: Obstructive jaundice caused by metastases to the distal common bile duct or the ampulla of Vater is often observed in patients with various advanced cancers; however, metastasis of lung cancer to the ampulla of Vater with subsequent development of jaundice is rare. CASE PRESENTATION: The patient was a 41-year-old Chinese female who presented with apparent jaundice and itching. An enlarged right supraclavicular lymph node was found during physical examination. Laboratory tests revealed significantly elevated bilirubin and aminotransferase. Imaging examinations, including ultrason

Neurofibroma of the bile duct: a rare cause of obstructive jaundice.

Neurofibromas of the common bile duct are extremely rare. The lack of specific clinical or radiological features makes preoperative diagnosis in the absence of histology difficult. We report the case of a female patient who presented with obstructive jaundice and evidence of a common bile duct stricture on imaging. She underwent an exploratory laparotomy, and intraoperative frozen section confirmed clear margins and a benign lesion. Excision of the extrahepatic bile duct and A Rouxen-Y hepaticojejunostomy was performed. We discuss the clinical features and management of neurofibromas of the bi

Isolated biliary cryptococcosis manifesting as obstructive jaundice in an immunocompetent adult.

Cryptococcosis is a symptomatic fungal infection caused by Cryptococcus, which frequently occurs in patients who are immunologically compromised or chronically ill. Localized involvement of the hepatobiliary system in an immunocompetent adult is extremely rare. We report a unique case of isolated biliary cryptococcosis manifesting as obstructive jaundice and mimicking hilar cholangiocarcinoma in an immunocompetent woman. By integrating surgical and antifungal drug therapy, the disease was effectively controlled. Despite an increasing incidence of biliary malignancies, hepatobiliary surgeons an
