Identification of the potential targets for keloid and hypertrophic scar prevention.

PURPOSE: We aimed to explore the molecular mechanism of pathologic skin scar and novel target for scar prevention. MATERIALS AND METHODS: Microarray data derived from keloid and hypertrophic scar were downloaded from ArrayExpress database. The common differentially expressed genes (DEGs) in keloid and hypertrophic scar samples were investigated by function and pathway analysis. The protein-protein interaction (PPI) network was constructed and the modules were screened. RESULTS: There were a total of 485 DEGs related with skin scar, including 247 up-regulated genes and 238 down-regulated gene…

Autologous fat grafting in keloids and hypertrophic scars: a review.

Keloid and hypertrophic scars are unique human dermal fibroproliferative disorders of the injured skin and are associated with pain, itch and can cause functional limitations. A number of genetic, systemic and local factors have been identified in the formation of keloids and hypertrophic scars. Studies have shown that adipose-derived stem cells have angiogenic and antiapoptotic properties which has effects on wound healing, soft-tissue restoration and scar remodelling, and thus may have a role in managing keloid scaring. However, this role is not well described in the literature. A systemic r…

Earlobe keloids: emerging cosmetic complication of ear-piercing.

BACKGROUND: Keloid is not a homogeneous biological entity. The exact pathologic mechanism of the lesion continues to be an enigma for physicians and researchers alike and no specific gene or sets of genes have been identified. The art of body piercing is ancient to people in the developed countries but over years it has rejuvenated to part of their fashion process. Ear-piercing or extra ear-piercing is becoming more rampant among youths in this part of the world, this however have been shown to cause hypertrophic scaring, keloids and its attendant cosmetic problems. OBJECTIVES: The objective …

Keloid formation occurring in the distribution of a congenital vascular malformation.

A 35-year-old African American man presented with complaints of malodorous drainage from hypertrophic lesions on his occipital scalp (Figure 1, inset). The patient had no family history of keloid formation and no other keloids on his body. The hypertrophic mass on his scalp had been present for 10 years and had not been a result of any type of mechanical, surgical, or laser treatment. It corresponded to the distribution of a large vascular malformation over the occiput (Figure 1). The vascular malformation extended from the occipital scalp to the right parietal scalp, the right side of the fac…

[Hyperplastic scars and keloids. Part I: basics and prevention].

Aberrant wound healing results in unsightly scarring, hypertrophic scarring, and keloid formation, causing functional and cosmetic deformities, discomfort, psychological stress, and patient dissatisfaction. Scar prevention and management continue to be important issues for the facial plastic surgeon. This article presents an overview on the pathogenesis of a scar and of the different types of scars. Differences between keloids, hypertrophic scars and normal scars include distinct scar appearance, histologic morphology and cellular function in response to growth factors. Recent advances in our …

Regulation of collagen gene expression in keloids and hypertrophic scars.

The synthesis of type I and III collagens in cultured skin fibroblasts from normal skin, normal scar, hypertrophic scar, and keloids was examined. The ratio of type I/III collagen was significantly elevated in keloids compared to that in the other groups. When mRNA steady-state levels coding for alpha 1(I) procollagen were determined, it was apparent that this increase in the type I/III collagen ratio in keloids was paralleled by a specific increase in alpha 1(I) procollagen mRNA. This specific increase in alpha 1(I) procollagen mRNA in keloids was the result of increased gene expression becau…

Perivascular myofibroblasts and microvascular occlusion in hypertrophic scars and keloids.

Microvessels in normal skin, granulation tissue, hypertrophic scar, keloid, and mature scar from human subjects were studied by transmission electron microscopy. Comparative observations suggested that most microvessels in hypertrophic scar and keloid are occluded or partially occluded, apparently owing to an excess of endothelial cells. Endothelial cell contraction was also supported by the observations, and perivascular satellite cells (pericytes), some of which were identified as myofibroblasts, were observed in hypertrophic scars and keloids. Among findings from statistical analyses were t…

Structure of the collagen nodule from hypertrophic scars and keloids.

The collagen nodule is the structural unit of all hypertrophic scars in keloids. It is never found in the fully mature scars. A study of the fine structure of the nodule might reflect its origin, and, hence, origin of the hypertrophic scar and keloid. Nodules are composed of a marked increase of unidirectional collagen fibrils aligned in a highly stressed orientation. There is also a marked increase in the number of fibroblasts. Few microvessels appear within the nodule; rather, they remain peripheral and encompass the main body of the nodule as a net. There is a heavier concentration of micro…

Mitochondrial dysfunction promotes keloid progression through metabolic reprogramming: A narrative review.

Keloids are a refractory fibroproliferative disorder characterized by excessive extracellular matrix (ECM) deposition and invasive growth beyond the original wound boundaries, severely impairing patients' quality of life. Accumulating evidence highlights that metabolic reprogramming and mitochondrial dysfunction are important in keloid pathogenesis, particularly in regulating the biological behavior of keloid fibroblasts (KFBs). This narrative review focuses on the sustained development and progression of keloids rather than the triggering factors of disease initiation, and systematically revi…

Surgery combined with postoperative radiotherapy gives excellent results in keloid disease.

OBJECTIVE: The optimal treatment of keloid scars remains unclear. Current protocols advise treatment with triamcinolone injection, when applicable, and surgery followed by adjuvant radiotherapy. However, the dosage and treatment modality of the radiotherapy vary among centres. This retrospective case series evaluated the long-term results of a single institution regarding keloid treatment using surgical excision, adjuvant external radiotherapy with site-dependent dosage, and wound site immobilisation with surgical tape and pressure treatment. METHOD: A review of the patient registry from Tamp…

Molecular and bioinformatic mechanism of asiaticoside on macrophage-mediated keloid pathogenesis.

BACKGROUND: Keloid is a fibroproliferative skin disorder driven by dysregulated inflammation and fibrosis, involving complex interactions between keloid fibroblasts (KFs) and immune cells. Although macrophages are implicated in keloid pathogenesis, the exact mechanisms remain poorly defined. Asiaticoside, a natural triterpene molecule, has shown therapeutic potential for pathological scarring, yet its molecular mechanisms warrant further investigation. PURPOSE: This study aims to elucidate the macrophage subtype-specific contributions to keloid progression and to investigate the anti-inflamma…

Multi-omics analysis to explore the molecular mechanisms related to keloid.

BACKGROUND: Keloid is a benign skin tumor that result from abnormal wound healing and excessive collagen deposition. The pathogenesis is believed to be linked to genetic predisposition and immune imbalance, although the precise mechanisms remain poorly understood. Current therapeutic approaches may not consistently yield satisfactory outcomes and are often accompanied by potential side effects and risks. The high recurrence rate and refractory nature of keloid nodules present significant challenges and uncertainties in their management. Given the lack of effective treatment strategies, it is e…
