Risk Factors, Clinical Outcomes, and Prognostic Factors of Bacterial Keratitis: The Nottingham Infectious Keratitis Study.

Background/Aim: To examine the risk factors, clinical characteristics, outcomes, and prognostic factors of bacterial keratitis (BK) in Nottingham, UK. Methods: This was a retrospective study of patients who presented to the Queen's Medical Centre, Nottingham, with suspected BK during 2015-2019. Relevant data, including the demographic factors, risk factors, clinical outcomes, and potential prognostic factors, were analysed. Results: A total of 283 patients (n = 283 eyes) were included; mean age was 54.4 ± 21.0 years and 50.9% were male. Of 283 cases, 128 (45.2%) cases were culture-positive. Re…

Epithelial keratitis mimicking herpes simplex keratitis in a patient after cataract surgery: A case report.

RATIONALE: Epithelial keratitis is a common complication after cataract surgery. Many factors have been attributed to this clinical phenomenon. PATIENT CONCERNS: An 82-year-old woman without previous herpes simplex keratitis (HSK) underwent an uncomplicated clear corneal phacoemulsification procedure in the right eye. In the late postoperative period, epithelial keratitis developed in this same eye. DIAGNOSIS: An initial diagnosis of HSK was made clinically. The lesion was refractory to antiviral treatment and had progressed. After topical acyclovir cessation and vigorous lubrication, a diag…

Microbial keratitis trends following refractive surgery: results of the ASCRS infectious keratitis survey and comparisons with prior ASCRS surveys of infectious keratitis following keratorefractive procedures.

In 2008, the American Society of Cataract and Refractive Surgery (ASCRS) surveyed its 9121 United States and international members to evaluate the changing trends and incidence, culture results, treatment, and visual outcomes of infectious keratitis following keratorefractive procedures worldwide. This paper presents and analyzes the results with comparisons to the data in surveys conducted in 2001 and 2004. Nineteen infections were reported by 14 surgeons who had performed an estimated 20,941 keratorefractive procedures, an incidence of 1 infection in every 1102 procedures. Sixteen cases pres…

Incidence of Acanthamoeba Keratitis in Switzerland.

Despite rising global reports of Acanthamoeba keratitis (AK), the incidence of AK in Switzerland remains unknown. This investigator-initiated, retrospective, multicenter study assessed the nationwide incidence of PCR- and/or culture-positive Acanthamoeba results from January 2010 to December 2023. Data were collected from all tertiary care and large ophthalmological facilities in Switzerland, fully anonymized, and aggregated by month and year. We considered all corneal scraping results, whereby the detection method was specific to local standards. We identified 271 PCR- or culture-positive Aca…

Applications of photodynamic therapy in keratitis.

Keratitis is corneal inflammatory disease which may be caused by several reason such as an injury, allergy, as well as a microbial infection. Besides these, overexposure to ultraviolet light and unhygienic practice of contact lenses are also associated with keratitis. Based on the cause of keratitis, different lines of treatments are recommended. Photodynamic therapy is a promising approach that utilizes light activated compounds to instigate either killing or healing mechanism to treat various diseases including both communicable and non-communicable diseases. This review focuses on clinicall…

Steroids in the Management of Infectious Keratitis.

PURPOSE: To summarize the evidence base on the use of topical corticosteroids for infectious keratitis. METHODS: Narrative review. RESULTS: Infectious keratitis is a painful condition that often results in visually significant corneal stromal scarring, even when antimicrobial therapy is successful. Corticosteroids may reduce inflammation and subsequent scar formation and while relieving the acute ocular pain associated with a corneal ulcer. However, corticosteroids also reduce the host immune response, which could hinder the ability to clear infection. The safety and effectiveness of cortico…

Intelligent Diagnosis and Classification of Keratitis.

A corneal ulcer is an open sore that forms on the cornea; it is usually caused by an infection or injury and can result in ocular morbidity. Early detection and discrimination between different ulcer diseases reduces the chances of visual disability. Traditional clinical methods that use slit-lamp images can be tiresome, expensive, and time-consuming. Instead, this paper proposes a deep learning approach to diagnose corneal ulcers, enabling better, improved treatment. This paper suggests two modes to classify corneal images using manual and automatic deep learning feature extraction. Different

A Clinical Case of Viral Keratitis.

Keratitis is a pathological condition involving inflammation of the cornea. It can be an infectious or non-infectious disease. The causative organisms of keratitis are categorized as bacteria, viruses, fungi, or parasites. The viruses responsible for causing keratitis are herpes simplex virus (HSV), varicella-zoster virus, and adenoviruses. The clinical features of this infection may range from pain and redness of the eye to scarring of the cornea or blindness. We present the case of a 71-year-old elderly female patient suffering from viral (HSV) keratitis. She was referred to the department o…

Update on the management of fungal keratitis.

PURPOSE: The aim of this article is to introduce the recent advance on the studies of fungal keratitis published over past 5 years. METHODS: We performed literature review of articles published on PubMed, Google Scholar, CNKI and Web of Science relevant to the diagnosis, pathogenesis and novel treatment of fungal keratitis. RESULTS: Excessive inflammation can lead to stromal damage and corneal opacification, hence the research on immune mechanism provides many potential therapeutic targets for fungal keratitis. Many researchers discussed the importance of earlier definitive diagnosis and wer…

Pathobiology and treatment of viral keratitis.

Keratitis is one of the most prevalent ocular diseases manifested by partial or total loss of vision. Amongst infectious (viz., microbes including bacteria, fungi, amebae, and viruses) and non-infectious (viz., eye trauma, chemical exposure, and ultraviolet exposure, contact lens) risk factors, viral keratitis has been demonstrated as one of the leading causes of corneal opacity. While many viruses have been shown to cause keratitis (such as rhabdoviruses, coxsackieviruses, etc.), herpesviruses are the predominant etiologic agent of viral keratitis. This chapter will summarize current knowledg…
