BACKGROUND: This study investigated the epidemiology and causes of bearing dislocations following mobile-bearing unicompartmental knee arthroplasty (MUKA) and determined whether the incidence of primary bearing dislocations decreases as surgeon experience increases.
METHODS: We retrospectively reviewed the bearing dislocations following MUKAs performed by 14 surgeons with variable experience levels. Causes of bearing dislocations were determined based on the surgical records, radiographs, and operator's suggestion. Using a chi-squared test, the incidence of bearing dislocation was compared be

PURPOSE OF THE STUDY: Appropriate treatment of patellar dislocation in children and adolescents remains a subject of debate. Ligamentous transfer or "soft rod" is a technique for realigning the extensor mechanism that can be adapted to children. We analyzed results obtained in our patients.
MATERIAL AND METHODS: Between 1979 and 2000, 35 children (50 knees) underwent realignment which was combined in all cases with lateral retinacular release and medial reefing using the Insall procedure. Complementary procedures were needed in certain cases (quadriceps lengthening, femoral sulcus plasty, qua

Postoperative instability following total knee arthroplasty (TKA) is a serious complication that challenges both the surgeon and the patient. Management becomes particularly complex when instability is recurrent, as restoration of mechanical alignment and soft-tissue balance is critical for a successful outcome. We report a case of a man in his sixties who presented with knee dislocation after a primary TKA performed elsewhere. The patient subsequently underwent tibial insert exchange, following which he again experienced recurrent dislocation. Definitive management was achieved with revision …

BACKGROUND: Anterior tibiofemoral dislocation is a severe complication of a total knee arthroplasty. It is rare, and it is distinctly different from bearing spinout. Most tibiofemoral dislocations are posterior. Anterior dislocation has previously been described in various prosthesis designs, but not in mobile-bearing prostheses. We present two cases and provide recommendations for the management of this rare and severe injury.
CASE DESCRIPTION: Two cases of complete anterior tibiofemoral dislocation were brought on by trauma, fifteen and eight years after initial implantation of mobile-beari

We describe the diagnosis, treatment, rehabilitation, and complications of a 27-year-old male paraskiier who sustained a closed high-velocity knee dislocation after colliding with a tree at a speed of 45 mph and falling 40 feet to the ground. Extensive surgery to repair and reconstruct the knee included open lateral capsular repair, open anatomical posterolateral corner reconstruction with split Achilles tendon allograft, open iliotibial band reconstruction with Achilles tendon allograft, arthroscopic anterior cruciate ligament reconstruction with bone-patellar tendon-bone autograft, and doubl…

BACKGROUND: Knee dislocations are rare injuries with potentially devastating vascular complications. An expeditious and accurate diagnosis is necessary, as failing to diagnose vascular injury can result in amputation; however, the best diagnostic approach remains controversial.
QUESTIONS/PURPOSES: We asked: (1) What patient factors are predictors of vascular injury after knee dislocation? (2) What are the diagnostic utilities of palpable dorsalis pedis or posterior tibial pulses, and the presence of an ankle-brachial index (ABI) of 0.9 or greater?

Old neglected dislocation of knee joint is a rare injury. Any orthopaedic surgeon would have faced only a few cases of unreduced neglected dislocation in his life time practice. We report the case of a 30-year old male patient with one month old unreduced knee dislocation which was managed with open reduction and stabilization with two intra-articular crossed Steinman pins for six weeks, followed by removal of the pins and gradual weight bearing in hinged knee brace. At the end of one year, range of movement of knee joint was 0 to 50 degree with minimal knee pain on walking.

PURPOSE OF THE STUDY: Complex femorotibial dislocation of the knee joint generally results from high-energy trauma caused by a traffic or a contact sport accident. Besides disruption of the cruciate ligaments, in 10-25% of patients present concomitant palsy of the common peroneal nerve and more rarely disruption of the popliteal artery. The purpose of this work was to assess outcome in a monocentric consecutive series of knee dislocations with ischemia due to disruption of the popliteal artery and to focus on specific aspects of management.
MATERIAL AND METHODS: This retrospective series incl…

From a one-surgeon series of 2485 patients, we report on 10 patients with rotating platform dislocation after primary Low Contact Stress total knee arthroplasty. All dislocations occurred within 2 years of the index procedure. Of the 10 patients, nine required open reduction. Five of these patients also had exchange of the original insert. One patient was treated by closed reduction. All knees were immobilized in a cast for 8 weeks. Eight of the 10 patients had no additional dislocation and at followup (average, 35 months; range, 12 months-5 years), had a stable functional joint. Two patients

Permanent post-traumatic patellar dislocation is a rare entity resulting from recurrent trauma. This uncommon condition is often confused clinically with congenital dislocation of the patella. Treatment options, although not well defined, include observation, various soft tissue and bony procedures for realigning the patella and patellectomy. To our knowledge, this is the first case reporting the use of conventional non-constrained total knee arthroplasty with patellar resurfacing and soft tissue realignment procedure for long-standing patellar dislocation in a single stage. An interesting fea

We report a rare case of complete knee dislocation following anterior cruciate ligament (ACL) disruption without any other ligament tears. The pathology of the knee joint was torn ACL, intact other ligaments, osteochondral fractures and bone bruise of the lateral femoral condyle, and torn lateral meniscus. In this case, osteochondral fracture resulting from the anteriorly sublaxiation of the tibia following ACL disruption was considered to prevent from spontaneous reduction. This case suggests that anterolateral knee dislocation and spontaneous reduction may occur in ACL injuries.

The right lower extremities of 64 young rabbits were immobilized by a plaster spica. The animals developed a gait pattern, which included internal tibial rotation and adduction of the left (unimmobilized) tibia. Twenty-one of the animals developed medial patellar dislocation in the unimmobilized lower extremity. The mechanism of the patellar dislocation in this experimental model was possibly overstretching of the lateral colateral ligament and the lateral side of the joint capsule, associated with medial rotation of the tibia and the tibial tubercle. The direction of patellar pull when glidin

Knee dislocations and multiligament knee injuries represent a significant clinical and surgical challenge, particularly when associated with patellar tendon rupture, a rare combination with a high functional impact that may initially go unrecognized. Management of these injuries requires meticulous planning to restore anatomy, joint stability, and extensor mechanism function in order to optimize long-term functional outcomes. We report the case of a patient who sustained a knee dislocation associated with patellar tendon rupture, which was surgically managed using a two-stage approach. Current…

BACKGROUND: Acetabular fracture with concomitant hip dislocation (dislocation-fracture) is a high-energy injury requiring urgent management. Although less life-threatening than pelvic ring fractures, these injuries are often associated with ipsilateral knee trauma, which may be overlooked in the acute setting. Delayed recognition can lead to secondary meniscal and chondral damage.
PURPOSE: To investigate whether hip dislocation in the setting of acetabular fracture is an independent risk factor for ipsilateral knee injury.
METHODS: We retrospectively reviewed 180 patients (146 men, 34 women;

INTRODUCTION AND IMPORTANCE: Posterolateral knee dislocations are rare, complex injuries predominantly resulting from high-energy trauma. They present significant diagnostic and therapeutic challenges, crucial for maintaining long-term knee function and stability.
CASE PRESENTATION: We report the case of Mr. Y.G., a 34-year-old male who suffered a left knee posterolateral dislocation due to a motorcycle accident. Clinical examination and imaging revealed a valgus deformity, swelling, ecchymosis, and a persistent medial joint line groove. The injury was classified based on NEYRET et al. criter…

The case of a 56-year-old male patient with locking of the knee in extension without trauma is presented. Patient history, clinical examination and radiographs confirmed a superior dislocation of the patella. Closed reduction led to full weight bearing with free range of motion without further complaints. Knowledge of this uncommon cause of a locked knee joint leads to immediate treatment avoiding complex imaging modalities, such as computed tomography (CT) or magnetic resonance imaging (MRI). No further treatment was necessary in the majority of the cases published in the literature.

CASE: A 43-year-old man suffered an irreducible posterolateral knee dislocation while snowboarding with associated tears of the anterior cruciate, posterior cruciate, medial collateral, and posterolateral corner ligaments. Two closed reduction attempts failed, and magnetic resonance imaging revealed incarcerated soft tissue from a tertiary gastrocnemius muscle head. The patient underwent open reduction and repair/reconstruction of his multiligamentous knee injury. At the 6-year follow-up, the patient did not have pain or instability and returned to recreational activities.
CONCLUSIONS: This c…

BACKGROUND: Knee dislocation may be associated with an injury to the common peroneal nerve with a subsequent foot drop. Previous studies have demonstrated good functional results after posterior tibial tendon transfer in patients with foot drop. No studies, to our knowledge, have focused exclusively on knee dislocation as the cause of common peroneal nerve injury leading to foot drop.
QUESTIONS/PURPOSES: We determined the percentage of patients developing common peroneal nerve paresis after knee dislocation, the symptom improvement rate in these patients, and patient-reported outcomes (Americ…
