The impact of prolonged second stage in twins on perinatal outcomes.

OBJECTIVE: To assess the duration of the second stage of labor in twin pregnancies in relation to maternal and neonatal outcomes. METHODS: A retrospective study between 2014 and 2020. Eligible cases were twin pregnancies that reached the second stage. The pre-defined groups were based on the total time spent in the second stage of labor; Group 1 (<1 h), group 2 (1-2 h), and group 3 (>2 h), which was considered the prolonged second stage group. RESULTS: Among the 439 planned vaginal births, successful vaginal delivery of both twins was achieved in 63.8%. Prolonged second stage was observed in

The risk of acute neonatal morbidities in the delivery room after primary cesarean at term: influence of labor and stage.

We evaluated acute neonatal morbidities in the delivery room associated with primary cesarean performed prior to labor and in the first or second stages of labor. A retrospective cohort study was conducted on subjects undergoing term, primary cesareans at the Women's Pavilion, Miller Children's Hospital in Long Beach, California from 2000 to 2007. Acute neonatal morbidities were tabulated as a function of time during labor when cesarean was performed. Composite neonatal morbidity was defined as the presence of at least one of the following: low 5-minute Apgar score, need for intubation, and/or

A Novel Partogram for Stages 1 and 2 of Labor Based on Fetal Head Station Measured by Ultrasound: A Prospective Multicenter Cohort Study.

OBJECTIVE: This study was aimed to describe continuous labor curves, including second stage, based on fetal head station. STUDY DESIGN: We performed a prospective multicenter cohort study. The inclusion criteria were women with singleton uncomplicated cephalic term pregnancies in labor, who delivered vaginally. We used a device that combines ultrasound imaging with position-tracking technology to monitor the head station noninvasively throughout labor. We collected data on demographics, labor parameters, and delivery and neonatal outcomes. RESULTS: A total of 613 women delivered vaginally, 3

Outcome in second- versus first-stage cesarean delivery in a teaching institution in eastern India.

We evaluated the maternal and perinatal complications of cesarean delivery performed in the second stage compared with the first stage of labor in nulliparous women. We performed a hospital-based cohort study in a teaching institution in Kolkata, West Bengal, India. The primary maternal outcomes measured included intraoperative surgical complications, duration of surgery, need for blood transfusion, wound infection, transfer to intensive care unit, and length of hospital stay. The neonatal outcomes included 5-minute Apgar score 3 or less, need for endotracheal intubation, admission to neonatal

[Uterine contractile activity in parturients of a group at risk for prolonged pregnancy prepared for labor by acupuncture reflexotherapy].

Prenatal preparation by using acupuncture was studied for effects on the clinical course and uterine contractility in 181 parturients who were at a high risk for prolonged pregnancy. With prenatal acupuncture, labor occurred in time in most parturients. Seven (3.8%) labors were delayed, 4 babies were born having signs of postmaturity. Antenatal amniotic fluid discharge was seen in 19% of the parturients, uterine inertia occurred in 19.8%, Cesarean deliveries were observed in 24 parturients. Internal tocographic assessment of uterine contractility revealed higher amplitudes, which contributed t

[How long can we wait at full dilatation. A study of maternal and neonatal morbidity related to the duration of the second stage of labour in nulliparous women].

OBJECTIVES: To determine the statistical association, in nulliparous patients, between maternal and fetal morbidity and the length of the second stage of labour. To precise whether a prolongation of this period of more than 2h may results in a dramatic increase of this morbidity. MATERIALS AND METHODS: Retrospective cohort study conducted in a level III referral centre between 1 April 2004 and 30 April 2005, including all nulliparous, term, cephalic, live singleton birth without fetal malformation in patients reaching the second stage of labour (n=1191). All deliveries were performed without

A cross-sectional study exploring the incidence of and indications for second-stage cesarean delivery over three decades.

OBJECTIVE: To observe the incidence of, indications for, and complications associated with second-stage cesarean delivery in 10-year intervals over 30 years. METHODS: The present analysis of prospectively collected data compared cesarean deliveries during 1976, 1986, 1996, and 2006 at John Radcliffe Hospital in Oxford, UK (n=3222). Pregnancy, delivery, and neonatal details were reviewed. RESULTS: The proportion of deliveries by cesarean in the second stage of labor increased from 0.5% (22/4464) in 1976 to 2.1% (124/5998) in 2006 (P<0.001). Selective episiotomy versus no episiotomy for severe perineal trauma: a systematic review with meta-analysis.

INTRODUCTION AND HYPOTHESIS: We hypothesized whether a non-episiotomy protocol or administration of selective episiotomy as an intrapartum intervention would modify the incidence of obstetric anal sphincter injuries (OASIS). METHODS: We registered this systematic review with the PROSPERO database (CRD42018111018). Prospective randomized controlled trials (RCTs) were included from databases until February 2019. The primary outcome was OASIS, and the secondary outcomes were any perineal trauma, duration of the second stage of labor, instrumental delivery, and post-partum hemorrhage. The risk of

Labor and Delivery of Twin Pregnancies.

Obstetricians who care for twin pregnancies should be aware of the challenges that may arise during the labor and delivery. With recognition of these issues and proper training, providers should be able to help women with twin pregnancies achieve a safe delivery for them and their babies. With the use of breech extraction of the second twin and active management of the second stage of labor, women with twin pregnancies can also achieve a high vaginal delivery rate of both twins.

A multicenter interventional program to reduce the incidence of anal sphincter tears.

OBJECTIVE: In Norway, we have experienced a gradual increase in the incidence of obstetric anal sphincter injuries from under 1% in the late 1960s to 4.3% in 2004. This study was aimed to assess whether an interventional program causes a decrease in the frequency of anal sphincter tears. METHODS: In all, 40,152 vaginal deliveries between 2003 and 2009 were enrolled in the interventional cohort study from four Norwegian obstetric departments. The focus of the intervention was on manual assistance during the final part of the second stage of labor. Data were analyzed in relation to occurrence o

Second stage disorders in patients following a previous cesarean section: vacuum versus repeated cesarean section.

OBJECTIVE: To investigate whether vacuum extraction due to failure of labor to progress (dystocia) during the second stage in a delivery following a previous cesarean section (CS) is related to increased adverse maternal and perinatal outcomes as compared with repeated CS. STUDY DESIGN: A retrospective cohort study of pregnancy and delivery outcomes of patients in their second deliveries attempting a vaginal birth after cesarean (VBAC) following one CS was conducted. Patients who delivered by vacuum extraction were compared with patients who underwent a repeated CS for failure of labor to pro

Medical and nonmedical factors influencing utilization of delayed pushing in the second stage.

OBJECTIVE: To evaluate factors impacting selection to delayed pushing in the second stage of labor. STUDY DESIGN: This case-control study was a secondary analysis of a large retrospective cohort study. Cases included women who delayed pushing for 60 minutes or more in the second stage of labor. Controls began pushing prior to 60 minutes from the time of diagnosis of complete dilation. Demographic, labor, and nonmedical factors were compared among cases and controls. Logistic regression modeling was used to identify factors independently associated with delayed pushing. RESULTS: We identified

Defining an abnormal first stage of labor based on maternal and neonatal outcomes.

OBJECTIVE: The objective of the study was to determine the threshold for defining abnormal labor that is associated with adverse maternal and neonatal outcomes. STUDY DESIGN: This study consisted of a retrospective cohort of all consecutive women admitted at a gestation of 37.0 weeks or longer from 2004 to 2008 who reached the second stage of labor. The 90th, 95th, and 97th percentiles for progress in the first stage of labor were determined specific for parity and labor onset. Women with a first stage above and below each centile were compared. Maternal outcomes were cesarean delivery in the

Intrapartum Fetal Electrocardiogram in Small- and Large-for-Gestational Age Fetuses.

OBJECTIVE: This study aimed to evaluate whether intrapartum fetal electrocardiogram (ECG) tracings with ST-elevation or depression occur more frequently in each stage of labor in small-for-gestational age (SGA) or large-for-gestational age (LGA), as compared with appropriate-for-gestational age (AGA) fetuses. STUDY DESIGN: We conducted a secondary analysis of a large, multicenter trial in which laboring patients underwent fetal ECG waveform-analysis. We excluded participants with diabetes mellitus and major fetal anomalies. Birth weight was categorized as SGA (<10th percentile), LGA (>90th pe

Duration of labor in consecutive deliveries: a retrospective data analysis.

PURPOSE: Labor is shorter in multiparous women. However, there are no individualized data on differences in duration of labor for consecutive deliveries in the same parturient. METHODS: We conducted a retrospective data analysis from 2004 to 2021 at the University Hospital of Zurich and included all women with 2 or more vaginal deliveries of a singleton child in cephalic position, between 22 and 42 weeks of gestation. Descriptive statistics were performed with SPSS version 25.0 (IBM, SPSS Inc., USA). The primary endpoint was the ratio between durations of labor stages in consecutive deliverie

Changes in intrapartum maternal placental growth factor levels in pregnancies complicated by fetal compromise at term.

BACKGROUND: Intrapartum fetal compromise (IFC) may result from the gradual decline in placental function during labour and can precipitate adverse neonatal outcomes. Placental growth factor (PlGF) is a biomarker of placental function. This study aims to investigate maternal PlGF levels and adverse perinatal outcomes in term labour. METHODS: Prospective observational study (Mater Mothers' Hospital, Brisbane). Eligibility: 37+0- 42+0 weeks gestation, singleton, cephalic, non-anomalous pregnancies. Cases of pre-eclampsia and fetal growth restriction were excluded. Maternal PlGF was sampled at th

Is Prolonged Labor Managed Adequately in Rural Rwandan Hospitals?

Unnecessary interventions to manage prolonged labor may cause considerable maternal and perinatal ill-health. We explored how prolonged labor was managed in three rural Rwandan hospitals using a partograph. A retrospective chart review was done to assess whether (A) the action line on the partograph was reached or crossed, (B) artificial rupture of membranes (ARM) performed, (C) oxytocin augmentation instituted, and (D) vacuum extraction (VE) considered when in second stage of labor. Adequate management of prolonged labor was considered if three clinical criteria were fulfilled in the first an

To push or not to push?

Women in the second stage of labour instinctively bear down for short periods of four to six seconds. Most of these spontaneous bearing down efforts are accompanied by the release of air, with several breaths being taken between each effort. Conventional pushing techniques with sustained breath holding compromise fetal well being by reducing maternal arterial pressure and the oxygenation of maternal blood. Adequate progress may be made in labour, with the delivery of a healthy baby, when the management encourages spontaneous maternal efforts.
