An Acquired Anterior Glottic Web Model by Heat Injury with a Laryngoscopic Approach in a Rabbit.

Acquired anterior glottic webs (AGW) can lead to abnormally elevated phonatory pitch, dysphonia, and airway obstruction requiring urgent intervention. In this study, we construct a novel AGW rabbit model using heat injury by a laryngoscopic way. A primary study was conducted to identify the injury depth in rabbits' vocal folds (VFs) by graded heat energy, and the heat energy for the incurrence of epithelial layer, lamina propria, and muscular layer (ML) injury was 25, 30 and 35 W, respectively. Then, four different models were designed based on the depth and degree of the injury to determine t

A study to evaluate nasotracheal intubation using Airtraq laryngoscope with a bougie and without a bougie.

BACKGROUND AND AIMS: Airtraq has been found to be useful in improving the view of the glottis. However, directing the tube tip into the glottis can be challenging during nasotracheal intubation. This problem gets resolved if the bougie is placed first. The present study was conducted for the evaluation of nasotracheal intubation using a nasal Airtraq laryngoscope with and without a bougie. METHODS: Fifty patients of either gender, aged between 18 and 60 years, who belonged to the American Society of Anesthesiologists physical status (PS) I or II, requiring nasotracheal intubation were include

Comparison of McGrath Videolaryngoscope and Macintosh Laryngoscope in Children with Torticollis: Randomized Controlled Trial.

We investigated the efficacy of the McGrath videolaryngoscope compared with the Macintosh laryngoscope in children with torticollis. Thirty children aged 1-10 years who underwent surgical release of torticollis were randomly assigned into the McGrath and Macintosh groups. Orotracheal intubation was performed by a skilled anesthesiologist. The primary outcome was the intubation time. The Cormack-Lehane grade, lifting force, intubation difficulty scale (IDS), difficulty level, and intubation failure rate were also assessed. The intubation time was significantly longer in the McGrath group than i

First Case Report of Intubation With a Total Control Introducer and a Hyperangulated Video Laryngoscope.

We describe the first use of an articulating Total Control Introducer (TCI) and video laryngoscope (VL) to guide intubation of a patient with known difficult airway. The patient was an 84-year-old woman with a Mallampati IV airway assessment, small mouth opening, limited neck extension, and micrognathia. We utilized a VL to visualize the glottis and an articulating TCI for endotracheal tube placement. Anteflexion and retroflexion of the introducer tip allowed dynamic navigation through the upper airway and into the trachea. Intubation was performed on first attempt without complications.

A Comparative Evaluation of Hemodynamic Response and Ease of Intubation using Airtraq and McCoy Laryngoscope.

BACKGROUND: Various airway gadgets have been devised to overcome difficult airway scenario in anesthesia practice. It includes simple rigid laryngoscope to complex fiber-optic intubating devices; however, there is weak evidence to support the superiority of one device over the other. AIMS AND OBJECTIVE: The ease of intubation, time and the hemodynamic variability between two groups during intubation are considered as the primary outcome measures. MATERIALS AND METHODS: In this prospective study, 60 patients of either sex undergoing elective surgery who required tracheal intubation were inclu

Muscle Tension Dysphonia: Which Laryngoscopic Features Can We Rely on for Diagnosis?

OBJECTIVES: Muscle tension dysphonia (MTD) is generally diagnosed through clinical history and physical examination. Several diagnostic or classification systems exist, such as those of Van Lawrence, Morrison-Rammage, and Koufman, that delineate MTD and distinguish subtypes on the basis of laryngoscopic features. The aim of this study is to determine which of the clinical features included in these classifications are most related to the aerodynamic profile of MTD. STUDY DESIGN: This is an analytic retrospective study. MATERIAL AND METHODS: This study evaluates a series of 30 consecutive pat

Simple method for obtaining the optimal laryngoscopic view in children: A prospective observational study.

BACKGROUND: Head and neck positioning has an important effect on laryngeal visualization during laryngoscopy. For small children and infants, a head-flat position is traditionally assumed; however, during laryngoscopy, the optimal head position may result in a superior laryngeal view in certain patients. METHODS: We investigated whether the alignment of the external auditory meatus and the sternal notch with a pillow with an individualized height is associated with improved laryngeal visualization during direct laryngoscopy in pediatric patients. We enrolled 49 pediatric patients (3 to 6years

Palatal and retropharyngeal injury secondary to intubation using the GlideScope® video laryngoscope.

INTRODUCTION: There are few reports of injury to the soft palate and retropharynx sustained during intubation with the GlideScope® video laryngoscope. Most reports are of isolated injury to the soft palate. CASE HISTORY: We describe a patient in whom the retropharynx was injured but the extent of the injury was not observed initially. The patient did not suffer severe sequelae from this injury. However, this injury can cause serious sequelae if it is not recognised (eg development of a retropharyngeal abscess). CONCLUSIONS: We recommend that any patient who sustains injury to the soft palate

The intubation scoop (i-scoop) - a new type of laryngoscope for difficult and normal airways.

The i-scoop is an intubation device with a curved guiding bar with laterally located lenses at its tip, rather than a blade. Twenty-five anaesthesiologists intubated a manikin that simulated first a normal and then a difficult airway. All participants were able to intubate the difficult airway with a good view of the glottis using the i-scoop. None was able to intubate using seven other laryngoscopes (Macintosh laryngoscope, GlideScope(®) GVL and AVL, McGrath(®) (Series 5/MAC), C-MAC(®) , A.P. Advance(™) ). Intubation was successful only with the Airtraq(®) (n = 10), the Airway Scope (n = 5), …

Glide video laryngoscope for the management of foreign bodies impacted at the hypopharyngeal level in adults.

BACKGROUND: Cricopharyngeal foreign bodies (FBs) impaction in adults is a common clinical problem; without treatment, the sequelae may be lethal due to local and/or mediastinal infection. When direct laryngoscopy and flexible fiberoptic endoscopy are ineffective, rigid endoscopy is the method of choice requiring general anesthesia. The new video laryngoscopes represent a great advancement in the assessment of the laryngeal inlet. Aim of the study was to assess the feasibility of identifying and removing FBs impacted at crycofaringeal and upper oesophageal sphincter by the video laryngoscope. …

Novel laryngoscopic strategies to improve evaluation of the site and extent of primary hypopharyngeal tumours.

OBJECTIVE: To investigate different strategies for displaying the hypopharynx and oesophageal entrance during laryngoscopy for hypopharyngeal cancer. PATIENTS AND METHODS: A total of 113 patients with hypopharyngeal cancer underwent laryngoscopy prior to surgery. The hypopharynx was displayed by: (1) pulling the anterior cervical skin; (2) having the patient perform the Valsalva balloon-blowing manoeuvre; and (3) injecting oxygen through the biopsy channel to expose the oesophageal entrance. The effect of these methods on visualisation of primary tumour size and extent was assessed. RESULTS:

Novel use of a Weerda laryngoscope for transoral excision of a cervical ganglioneuroma: a case report.

INTRODUCTION: A ganglioneuroma is a benign neoplasm arising from neural crest cells of the sympathetic nerve fibers and is most commonly seen in the posterior mediastinum or retroperitoneum. Although very uncommon, ganglioneuromas must be included in the differential diagnosis of neck masses. In young adult women, neck incisions made for excision of these benign tumors should be avoided whenever possible. CASE PRESENTATION: We herein describe the case of a 19-year-old Japanese woman with a ganglioneuroma. The tumor was found in the parapharyngeal space, an unusual location. A fine-needle aspi

Indirect laryngoscopy with rigid 70-degree laryngoscope as a predictor of difficult direct laryngoscopy.

INTRODUCTION AND OBJECTIVES: The commonly-used predictors for difficult airway management are not very accurate. We investigate the power of indirect laryngoscopy with the rigid 70-degree laryngoscope as a predictor of difficult visualisation of the larynx with direct laryngoscopy. METHODS: We performed preoperative indirect laryngoscopy with the rigid laryngoscope on 300 patients. The vision obtained was classified into four grades: 1 (vocal cords visible), 2 (posterior commissure visible), 3 (epiglottis visible) and 4 (no glottic structure visible). Grades 3 and 4 were considered predictors

Evaluation of the Pentax-AWS(®) and the Macintosh laryngoscope in difficult intubation: a manikin study.

BACKGROUND: The Pentax-AWS (AWS(®)), a new video laryngoscope, has been shown to be useful in cases of difficult intubation. We hypothesized that the AWS(®) would be more useful in the settings of a narrow upper airway than the Macintosh laryngoscope. We compared each device in simulated scenarios of representative difficulty of tracheal intubation using a manikin. The primary endpoint was the rate of successful intubation. METHODS: With each device, 23 anesthesiologists performed tracheal intubation in a SimMan(®) manikin in the following scenarios: (1) normal airway, (2) tongue edema, (3) c

Prospective study of early detection of pharyngeal superficial carcinoma with the narrowband imaging laryngoscope.

BACKGROUND: The newly developed narrowband imaging (NBI) gastrointestinal endoscope makes possible the detection of superficial carcinoma in the oropharynx and hypopharynx, which is difficult with the conventional laryngoscope. Here, we investigated whether the combined use of laryngoscope with NBI allows the detection of superficial carcinoma in this region. METHODS: A total of 51 superficial, histologically confirmed lesions in 29 patients were studied. The quality of visualization of superficial carcinoma in the oropharynx and hypopharynx using the NBI-equipped laryngoscope was evaluated i
