The view of the glottis at laryngoscopy after unexpectedly difficult placement of the laryngeal mask.

In 12 patients to whom a non-depolarising neuromuscular relaxant had been given and in whom placement of the laryngeal mask had failed unexpectedly, the view of the larynx at laryngoscopy and the ease of tracheal intubation were examined. The glottis was only partially seen at laryngoscopy in three patients and was not seen at all in another three patients. Tracheal intubation was difficult in three of them. It would appear that in some patients both placement of the laryngeal mask and tracheal intubation are difficult. It is thus inadvisable to paralyse patients electively and rely on the lar…

[May recurrent goiter be resected bilaterally? Value and results of intraoperative laryngoscopy].

Operations for recurrent goiter are considered to range among the most difficult procedures in thyroid surgery and are marked by unusually high subsequent damages of the recurrent nerve. Results from 89 patients with recurrent goiter operated over the last six years and our own experiences with intraoperative laryngoscopy are presented. This procedure is applicable in 60% of all cases with true bilateral thyroid recurrency and accounts for a realistic help in deciding whether to continue the operation with simultaneous resection of the contralateral side.

Prediction of difficult laryngoscopy: an assessment of the thyromental distance and Mallampati predictive tests.

Two hundred and fifty patients were assessed preoperatively using the Mallampati classification and by measuring their thyromental distances. The ease or difficulty of direct laryngoscopy was assessed at the time of induction of anaesthesia. Retrognathia was seen in 15.6% of patients and the incidence of difficult laryngoscopy without external laryngeal pressure was 8.2%. It was found that both assessments predicted less than two in three difficult laryngoscopies and had high false positive rates. It was found that external laryngeal pressure often improved the view of the glottis in difficult…

Video Versus Direct Laryngoscopy for Intubation: Updated Systematic Review and Meta-Analysis.

Direct laryngoscopy (DL) is a modality commonly used in endotracheal intubation (EI). Video laryngoscopy (VL) was introduced to further facilitate the procedure with enhancement in glottic views, which captures the video image of the vocal cords to be projected onto a screen, providing enhanced visualization. This real-time video projection aids in accurately placing the endotracheal tube (ETT) through the vocal cords. In emergency and critical care settings, both laryngoscopes are used for intubations. This study assesses the efficacy of both modalities by comparing success rates in first-att…

Glottic opening detection using deep learning for neonatal intubation with video laryngoscopy.

OBJECTIVE: This study aimed to develop an artificial intelligence (AI) method to augment video laryngoscopy (VL) by automating the detection of the glottic opening in neonates, as a step toward future studies on improving intubation outcomes. STUDY DESIGN: A deep learning model, YOLOv8, was trained on 1623 video frames from 84 neonatal intubations to detect the glottic opening and evaluated using 14-fold cross-validation on metrics like precision and recall. Additionally, it was compared with 25 medical providers of varied intubation experience to assess its relative performance. RESULTS: Th…

Paralysis Versus Non-Paralysis Anesthesia for Operative Laryngoscopy: A Randomized Controlled Trial.

OBJECTIVE: To compare outcomes between two standard-of-care anesthesia regimens for operative laryngoscopy: general anesthesia with a neuromuscular blocking agent (NMBA) versus remifentanil and propofol (non-NMBA). METHODS: This was a prospective, single-blinded, randomized controlled trial at a tertiary care center. Patients were randomized to either anesthesia using rocuronium (NMBA) or with remifentanil/propofol infusion alone (non-NMBA). Intraoperative impressions, anesthesia data, and post-operative patient surveys were collected. RESULTS: Sixty-one patients who underwent suspension lar…

Exercise-induced laryngeal obstruction: From clinical examination to continuous laryngoscopy during exercise.

The diagnosis of exercise-induced laryngeal obstruction (EILO) should be suspected when exercise triggers inspiratory stridor. EILO is common in adolescent populations and has a significant impact on sports. Identification of laryngeal obstruction during continuous laryngoscopy during exercise (CLE) is the gold standard diagnostic tool for this disorder, which is not widely known in France. The challenge faced by otolaryngologists is to identify, among patients referred by pulmonologists or sports physicians, those with exercise-induced inspiratory symptoms or poorly controlled exercise-induce…

Value of oropharyngeal Mallampati classification in predicting difficult laryngoscopy among obese patients.

The prediction of difficult intubation in obese patients was investigated by co-estimating the degree of visibility of oropharyngeal structu … d by the examiner. Difficult intubation was defined as inadequate exposure of the glottis by direct laryngoscopy. Both oropharyngeal class methodologies were of equal sensitivity, whereas the modified technique presented a significantly higher positive predictive value (50.0% vs. 37.2%, P < 0.01). Statistical analysis revealed an increased risk of difficult la …

Malignant laryngeal lesions mimicking vocal process granuloma on flexible laryngoscopy: Two case reports.

Vocal process granuloma (VPG) is a common benign inflammatory lesion of the posterior glottis, typically induced by intubation injury, laryngopharyngeal reflux, or vocal abuse. Clinicians usually make an initial clinical impression of VPG merely through laryngoscopy, and routine biopsy is generally not performed. However, malignant lesions arising at the vocal process may appear highly similar to laryngeal granulomas under flexible laryngoscopy. We present two patients with a history of tracheal intubation. Their granuloma-like lesions were initially regarded as benign VPG, while postoperative…
