Solitary fibrous tumor of the larynx.

BACKGROUND: True mesenchymal, non-cartilaginous neoplasms of the larynx are rare. Extrapleural solitary fibrous tumor (SFT) is a localized neoplasm characterized by proliferation of thin-walled vessels and collagen-producing cells and is considered within the "hemangiopericytoma-solitary fibrous tumor" spectrum. SFT primary in the larynx is exceptional. DESIGN: Case report set in a comparison with other cases reported in the English literature (MEDLINE 1966 to 2007). RESULTS: A 49-year old white male presented with difficulty breathing, progressive over the past 2 years. He denied dysphagia …

[Imaging of tumors of the larynx and hypopharynx].

Evaluation of patients with laryngeal and hypopharyngeal tumors is based on multidetector CT imaging. The cervicomediastinal CT protocol and general guidelines with regards to planning and acquisition will be detailed. The primary role of imaging is accurate tumor staging but also detection of possible tumor extension to the superior aerodigestive tract and nodal areas. Therefore, images are acquired from the skull base to the cervicomediastinal junction. A chest CT must also be performed to look for distant metastases or other primary tumor. Deep tumor extension, detected only by imaging, is

Chondrosarcoma of the larynx: a case report.

Chondrosarcomas of the larynx are rare cancers and are more frequently located at cricoid cartilage level. They are characterised by a low tendency to metastatic diffusion (low grade). The treatment of choice is surgery, which may be endoscopic or "open partial surgery", if extension of the cancer is limited. Prognosis is generally good. In this report, a case of low malignancy chondrosarcoma of the larynx is presented, which was treated surgically with a glottic-hypoglottic laryngectomy according to Serafini-Bartual. Chondrosarcoma of the larynx shows a slow and painless growth, the first sym…

Amyloidosis of the larynx: a case report.

A 75-year-old female patient presented with hoarseness and foreign body sensation in the back of her throat of one-month history. Direct laryngoscopy showed bilateral, yellow, hyperemic masses on the left false vocal fold and laryngeal ventricle. Both true vocal folds were mobile. Excisional biopsies of the right false vocal fold and ventricle showed extracellular, insoluble, fibrillar protein accumulation, consistent with amyloidosis. There was no evidence for neoplasm. Serum and urine electrophoreses were negative. The work-up for systemic amyloidosis and multiple myeloma were negative. Trea

Paraganglioma of the larynx: a case report.

BACKGROUND: Paragangliomas arise from paraganglion cells as part of the diffuse neuroendocrine system. These tumors are generally considered benign. They appear rarely in the larynx and most of them are supraglottic. CASE REPORT: We present the case of a 45-year-old female patient who presented with a history of hoarseness for two months and difficulties in swallowing for twenty days. Fiberoptic endoscopic examination showed a submucosal mass involving the left side of the larynx. The subglottic area of the larynx was normal as were the base of the tongue and pharynx. The mobility of the true…

Management of larynx and trachea donors.

INTRODUCTION: Laryngeal transplantation is a possibility for patients with irreversible laryngeal disease, such as complex trauma and larynx cancer. The objective of performing this procedure was to solve problems that these patients face with a laryngectomy. The medical literature has reviews about larynx transplantations, but almost nothing about the larynx donor. The following is our experience on management of these donors. MATERIALS AND METHODS: Selection criteria was as follows: (1) 18-50 years old; (2) gender and ABO blood type matched between donor and recipient; (3) No abuse of tobac…

[Larynx preservation, state of the art].

Larynx preservation in advanced pharyngolaryngeal cancers has been a major challenge in clinical research over the past two decades. Subtotal surgery (endoscopic laser surgery, supracricoid partial laryngectomies and hemilaryngopharyngectomies) has allowed reducing the indications of the so-called "mutilating" surgery. On the other hand, the modification of fractionation has notably improved the results of definitive irradiation (but most probably for supraglottic tumors). The main clinical research has been carried out with either sequential or concurrent chemo-irradiation. Induction chemothe…

Rosai-Dorfman disease of the larynx.

We present a case of an 81-year-old man with a history of worsening dysphonia of six months duration. A year before a sinus histiocytosis with massive lymphadenopathy (Rosai-Dorfman disease) was diagnosed on a submandibular lymph node biopsy. On presentation the patient showed a mass in the left subglottic area with a modest reduction in the airway space. The patient underwent an endoscopic CO2 laser excision of the mass, without post-operative complications. Histopathological examination and electron microscope images confirmed the previous diagnosis of Rosai-Dorfman disease. The clinical and

Sensory discrimination in the larynx and hypopharynx.

The traditional approach to evaluating swallowing disorders has focused on upper aerodigestive tract motor function. The evaluation of the sensory apparatus in the larynx and pharynx as part of the workup of dysphagia has heretofore not been addressed. The purpose of this work is to discuss a new, noninvasive method to measure sensory discrimination in the supraglottic larynx and hypopharynx. An air pulse, of precise duration and pressure, was delivered through a flexible fiberoptic telescope to the submucosally situated superior laryngeal nerve along the anterior wall of the pyriform sinus. T…

Langerhans cells in the larynx and the hypopharynx.

Langerhans cells are important in the human immune system as a part of the dendritic cell system. This study was carried out to demonstrate the presence of Langerhans cells and their distribution in normal human larynx and hypopharynx, by light (S-100 polyclonal antibody) and transmission electron microscopy. The results are summarized as follows: 1) Langerhans cells with cytoplasmic granules (Langerhans granules or Birbeck's granules) were situated in the suprabasal region of the squamous epithelium in the larynx and the hypopharynx. 2) They distributed in the membranous portion of the vocal …

Carcinoma of the larynx and hypopharynx in the elderly.

The outcome of a series of 68 patients over 75 years old with carcinoma of the larynx, and 33 patients over 75 years old with hypopharyngeal carcinoma managed by a single oncological team during a 10-year period was studied. Sixty-one of the patients with laryngeal carcinoma were treated with curative intent. The actuarial 3-year survival rate of the whole group was 45%. This contrasts with hypopharyngeal carcinoma in which we found a 3-year survival of 11% with only 17 of the 33 patients suitable for treatment with curative intent. In view of the anticipated poor prognosis of hypopharyngeal c…

Benign pathologic responses of the larynx.

Benign laryngeal lesions were examined for patterns of injury indicated by deposition of fibronectin and collagen type IV. An immunoperoxidase technique was used to compare 33 fresh or paraffin-embedded tissues with regard to their staining of monoclonal antibodies directed against fibronectin and collagen type IV. Two types of patterns were recognized. One pattern showed intense fibronectin deposition in the superficial layer of the lamina propria, often coupled with basement membrane zone injury, indicated by thick collagen type IV bands. The other pattern showed rare basement membrane zone

Malignant fibrous histiocytoma of the larynx.

Malignant fibrous histiocytoma of the upper respiratory tract is rare. We report a case of a glottic malignant fibrous histiocytoma in a 46-year-old man. Ultrastructural findings demonstrated that tumor cells were composed of fibroblast-like cells with abundant rough endoplasmic reticulum, histiocyte-like cells with numerous lysosomes and transitional cells with characteristics of both the fibroblast-like and histiocyte-like cells. Laryngeal microsurgery was performed as surgery and the patient has done well without recurrence to date (8 months after surgery).

A few remarks on the morphogenesis of the larynx.

After having reviewed the principal theories regarding the development of the larynx the authors performed a study on the morphogenesis of the above structure using four horizontally- and sagitally-sectioned human embryos. The conclusion of a study of these sections is that the LC constitutes the primordium of the laryngeal vestibule. From its lateral margin, caudally (Stage 19, Carnegie System), the primitive ventricles begin to form, while in the final stages the EL extends caudally enclosing a small cephalic infraglottic area and therefore completely separating the IG from the LC. Such find…

Response of the human larynx to silicone.

This report is concerned with tissue reaction to silicone and, in particular, long-term foreign body reaction. A histopathologic study was done on the larynx of a patient who had been injected 12 years before in the right vocal fold with room temperature vulcanizing silicone. The examination revealed particles of silicone encapsulated by thin fibrous tissue. There were few giant cells of the foreign body type and little cell infiltration. In addition, a questionnaire was returned by 54 of 90 patients who had vocal fold injected with less than 1.5 mL of either silicone fluid or room temperature…

Giant Teflon granuloma of the larynx.

Endolaryngeal injection of Teflon paste to augment and medialize the paralyzed hemilarynx aims to improve dysphonia and relieve aspiration. After an initial short-lived inflammatory reaction in the laryngeal tissues, a foreign body granuloma forms around the Teflon. In most cases the results are good but it may occasionally be necessary to consider removal of the granuloma if too large a volume has been injected, if it is inappropriately placed, or if the paralyzed vocal cord subsequently recovers. Surgical removal is difficult and the results are unpredictable. This case illustrates, for the …
