Clinical practice. Static, axial, and rotational deformities of the lower extremities in children.

Static, axial, and rotational deformities of the lower extremities are very frequent in children and often a reason for clinic visits. It is important to make a difference between physiological, usually spontaneously healing conditions, and real pathology. Flatfeet and less frequently cavus feet are the main foot problems. Special attention should be paid to the cavovarus foot that often has an underlying neurological disorder. Localized foot pain has usually a very specific cause and needs further investigation. Genua valga and genua vara are typical for a given age group and correct usually

Vascular reconstruction using deep vein for limb length discrepancy in a child.

Iatrogenic vascular injuries can result in claudication and limb length discrepancy in growing children. Traditional reconstruction has been performed with great saphenous vein as a conduit. We report the case of a 7-year-old boy with a symptomatic limb length discrepancy and vascular reconstruction using femoropopliteal vein. The use of deep vein as an autogenous conduit may facilitate reconstruction of iliofemoral arteries in preadolescent children, providing an excellent size match and an efficacious means of restoring normal blood flow.

The Dixie Cup technique for salvaging limb length and preserving nerve continuity in the mangled lower extremity.

The mangled lower extremity posses a difficult problem for the reconstructive surgeon. We describe a novel technique for preserving limb length and maintaining nerve continuity in a severely injured lower extremity. This specific example of fixating the viable, sensate distal foot tissues onto the proximal tibia was vital in maintaining optimal limb length. This allowed for a more desirable level of amputation, preserved knee function, and provided well-vascularized, sensate soft tissue coverage.

Long-term review of five leg replantations: emergency strategy and examples of lengthening of the leg on nerve regeneration.

The success rate for leg replantation has improved with the development of shortening-lengthening protocols. We checked whether this success was maintained long term in five cases of emergency reimplantation. The significant initial shortening of 93 mm, on average, enabled direct internal osteosynthesis, secondary lengthening was initiated swiftly, in the proximal metaphyseal area, and average lengthening was 85 mm. Consolidation was achieved in all cases within normal time periods, with an average inequality in residual length of 8mm. The speed of nerve regeneration was on average 1.926 mm/da

Gait analysis of adult patients with complete congenital dislocation of the hip.

Comprehensive gait analysis is valuable in understanding the performance of patients with lower limb disorders. The gait pattern of adult pa … m adduction moments of the hip and knee on the unaffected side. This asymmetry may have been due to leg length inequality. Thus, correlation of the leg length discrepancy may be important for unilateral CDH patients in improving their gait.

Long-term prognosis of Legg-Calvé-Perthes disease: a meta-analysis.

In the literature, long-term prognosis of Legg-Calvé-Perthes disease (LCPD) means prognosis for secondary osteoarthritis of the hip joint or leg-length inequality and its consequences. Most studies show results after conservative treatment. The long-term natural history of LCPD is not known. In spite of deformity, most patients do well in early adulthood. Radiographic and clinical osteoarthritis is increased in 20-year to 40-year follow-ups and degenerative joint disease develops in the majority of patients by the sixth or seventh decade of life. The reported average shortening of the affected

Ankle injuries in the young athlete.

Ankle injuries in the young athlete are quite different from those of an adult. Because the epiphyseal plates of the distal tibia and fibula are significantly weaker than surrounding ligaments, failure on stress occurs through the growth plates rather than through soft tissue. Growth-plate injuries can assume a distinct pattern based on the maturity of the physis. Leg length inequality and angular deformity are potential sequelae from significant injury to the ankle in the skeletally immature but, fortunately, occur infrequently. Soft-tissue injuries about the ankle are rare, but with advanced…

[Rehabilitation program for children and adolescents with limb defects or amputations of the lower extremity].

In rehabilitation of children and adolescents with lower limb deficiencies it is very important that the medical team taking care of the patient works out a treatment plan. Responsible cooperation with the parents is of utmost importance, the focus of all efforts is, of course, the handicapped child. The aims of rehabilitation are illustrated by examples demonstrating the course of successful rehabilitation of children and adolescents with damaged limbs. Equally, the psychological situation of the parents and child is taken into consideration. Children with limb deficiencies or amputation of t

[Lower leg lengthening using the Ilizarov system. Presentation of the various surgical techniques and analysis of lower leg lengthening procedures performed 1986-1989].

The technique of lengthening the tibia with the Ilisarov device is explained. Different variations (bifocal or simultaneous) of the method are demonstrated, as well as the results of 53 cases of tibia lengthening in 37 patients of our hospital between 1986 and 1989. In 14 patients, a second operative procedure was necessary during treatment. In no case was internal fixation or grafting necessary. The Ilisarov ring fixator is very efficient, especially in major lengthening procedures and for simultaneous correction of deformities.

Intercondylar Longitudinal Stress Fracture of Femur After Growth Suppression for Limb Length Discrepancy: A Case Report.

CASE: An 11-year-old boy with leg length discrepancy (LLD) secondary to Legg-Calvé-Perthes disease underwent growth suppression using a tension-band plate (TBP) in the distal femur. At 13 years of age, an intercondylar longitudinal stress fracture of the femur was identified on radiographs before implant removal, despite successful LLD correction. Although 3D CT revealed significant asymmetry in the bone morphology, conventional radiography showed no apparent changes. The fracture healed uneventfully after implant removal. CONCLUSION: This case underscores the need for careful postoperative m

Reconstruction of Bony Defects With Motorized Intramedullary Nails After Tumor Resection.

Motorized intramedullary lengthening nails allow for transport of a bone segment for limb lengthening, deformity correction, healing of nonunion, and intercalary distraction osteogenesis. Resection of tumors involving the bone can result in substantial defects that require reconstruction. Use of these nails allows for a biologic reconstruction with the incorporation of allograft or by distraction osteogenesis. Limb lengthening after an internal hemipelvectomy where the hip joint is resected can be performed to improve gait, decrease pain, and prevent the need for a custom shoe or shoe lift. Us

Femoral lengthening using limb reconstruction system augmented with only one antegrade flexible nail in children: early results.

Several techniques have been described for decreasing the duration of external fixator use, augmenting stability, and minimizing complications. The purpose of the present study was to evaluate the clinical results and complications of femoral lengthening procedures using the Limb Reconstruction System (LRS) in combination with a single antegrade flexible intramedullary nail (FIN). Femoral lengthening with LRS and FIN was applied to 14 patients (aged 6-16 years) between 2017 and 2021. The etiology was a congenital femoral deficiency in 12 patients and post-traumatic growth arrest in two. A sing

Limb lengthening for deformities in Ollier's disease: a systematic review.

BACKGROUND: The management of limb deformity, shortening, and bone defects in treatment of Ollier's disease is a major challenge. This study aims to summarize and compare the different surgical treatments, and to evaluate the outcome and possible prognostic factors of leg lengthening in these patients. MATERIALS AND METHODS: A systematic review of the literature from 1993 to 2017 was performed. Nineteen articles were found including a total of 121 patients with limb deformities because of Ollier's disease. The mean patients' age at the time of first surgery was 12 years. A total of 272 segmen

Use of internal lengthening nails in post-traumatic sequelae.

External fixators are a well-established modality for treating fractures with bone defects, leg-length discrepancy, malunion, nonunion and other post-traumatic consequences. However, use of internal lengthening rods has remarkably increased recently for post-traumatic conditions. The main advantage of internal lengthening rods is eliminating pin-site complications. Internal lengthening rods are also associated with less pain. Motorised internal lengthening rods show promising performance in post-traumatic cases. Rigorous pre-operative planning is paramount to reducing lengthening-related compl

Digital templating facilitates accurate leg length correction in total hip arthroplasty.

Digital templating of pre-operative radiographs is increasingly utilised by surgeons before total hip arthroplasty (THA) as part of an adequate preoperative preparation to minimise complications. Templating can accurately predict the required implant sizes but its use in facilitating correction of leg length discrepancy (LLD) has been underreported in the literature. We performed a retrospective analysis of a cohort of consecutive patients undergoing primary THA. A comparison was made of the implant sizes templated with actual sizes used. In addition, pre-operative leg-length discrepancy (LLD)

Combined fibular and tarsal agenesis in a case of lower extremity hemimelia.

Fibular hemimelia is a congenital disorder characterized by partial or total absence of the fibula. The appearance of the affected limb can range from barely detectable to severe deformity. An affected extremity typically displays a valgus foot and ankle, shortening of the leg, anterior bowing of the tibia and knee, tarsal coalition, as well as radiographic absence of 1 to several lateral rays (1, 2). We encountered an adult patient who displayed fibular agenesis in combination with absence of the ipsilateral talus and cuboid. Because of the rarity of this deformity, we believed that it was im

Deformities in conservatively treated closed fractures of the shaft of the femur in children.

We present the outcome of non-operative management of fractures of the shaft of the femur in children. Thirty children with non-operatively managed unilateral fractures of the shaft of the femur were studied. The mean age was 6.5 years (range: 1.2 to 12). Axial, angular and rotational malunion were assessed clinically and radiologically by plain films and computed tomography (CT). All children had shortening of the fractured limb with a mean shortening of 1.8 cm (range: 0.5 to 3.5). A significant level of shortening was seen in children over 7 years. In the sagittal plane, all had anterior ang
