The aim of the study is to assess and correlate the stage of cholesteatoma with its surgical outcome using the ChOLE classification, and thus revalidate it. Disorders of the arteries, veins, or nerves, alone or in combination, can result in leg ulcers. The presentation in these cases varies with the cause, which in turn guides management. A differential diagnosis is critical, because treatment that is essential for one type of ulcer may be contraindicated in another.

BACKGROUND: Venous leg ulceration results from chronic venous insufficiency of the lower extremities. We recently showed that matrix metalloproteinase (MMP) -2 plays a major part in the pathogenesis of venous leg ulcers. In vitro activation of recombinant MMP-2 is controlled by the activity of the urokinase-type plasminogen activator (uPA), which acts as a fibrin-independent plasminogen activator. The activity of MMP-2 is potentiated by binding of uPA to the uPA receptor (uPAR).

BACKGROUND AND AIMS: Skin grafting is a simple and cheap method of treating leg ulcers, and if done in the early phase it can lower health care costs. This study analyses patients who underwent excision and skin-grafting of leg ulcers in the Department of Plastic Surgery during 1993-1995.

Twenty five (17 male, 8 Female) sickle cell disease patients with 30 leg ulcers were studied over a 3 year period (January 1985 to December 1987) to provide information on the pathogenesis, course and management. Chronic stasis ulcers of the lower extremity can prove difficult to treat. In the event that conservative treatment fails, one approach is to excise the ulcer, ligate the incompetent perforating veins, and close the defect by muscle transposition, followed by application of skin grafts to the transposed muscles.

During the recent 4 years, 135 patients (130 females and 5 males) with trophic persistent ulcers of the lower extremities, developed due to venous pathology, were treated conservatively. Eighty patients aged from 30 to 50 years, and only 55 patients were much older. The duration of the disease was from 6 months to 20 years. Ulcers were of different size from 3.2 to 180 cm2.
