CONCLUSIONS: SSF using the novel IOL could be a viable option for the management of aphakia, lens subluxation, and IOL-bag complex dislocation with unique characteristics of stability. The advantages of this technique were the absence of haptic manipulation, self-centration, and firm fixation of the IOL.

We describe clinical applications and surgical techniques for a new type of capsular tension segment (CTS) for use during cataract surgery. RATIONALE: Morgagnian cataracts represent a hypermature stage of cataract formation characterized by cortical liquefaction and a sunken nucleus. Spontaneous rupture of the lens capsule is extremely rare but can result in dislocation of the lens nucleus into the anterior chamber.

PURPOSE: To evaluate the use of the MTI photoscreener (Medical Technology Inc, Iowa City, Iowa) in assessing and documenting lens abnormalities in the pediatric ophthalmology clinic.

A 55-year old pseudophakic man with megalocornea presented with a progressive monocular diplopia. On examination the patient was found to have subluxation of the posterior chamber intraocular lens (IOL)/capsular-bag-complex. Surgical removal of the complex was performed resulting in aphakic status. The patient was subsequently fitted with a custom contact lens for aphakia, with successful visual rehabilitation.

Puncturing the anterior capsule in a patient with a very soft lens, an elastic capsule, and/or deficient zonular countertraction can be challenging even with a sharp needle or blade. The crossed-swords, capsule-pinch technique capitalizes on opposing forces from 2 needles directed toward each other with a "pinch" of the capsule between their tips. This affords a controlled and facile puncture of the capsule without creating stress on the zonules or anteroposterior displacement of the lens.

BACKGROUND: To report a case of spontaneous lens dislocation following pupillary dilatation during preoperative assessment for cataract surgery.
METHODS: Our patient had been punched on the left side of the face 40 years ago but throughout this period the patient's vision had remained normal and ophthalmic evaluation by opticians had also been normal on multiple visits. A 34-year-old man with Marfan's syndrome complained of redness and pain in the left eye. The right eye had been blind because of untractable retinal detachment. The left eye had had three surgical procedures for retinal detachment, and the lens had been luxated in the vitreous for more than 14 years. Ultrasonography demonstrated the spontaneously luxated lens as the possible cause of the inflammatory reaction. The diagnosis of lens-induced endophthalmitis was confirmed by histopathologic examination of the excised lens.
