Frequency of recurrent spontaneous abortion and its influence on further marital relationship and illness: the Okazaki Cohort Study in Japan.

AIMS: The aim of this study was to examine the influence of recurrent spontaneous abortion (RSA) on marital relationships, and the association between past/present illness and RSA. MATERIAL AND METHODS: A total of 2733 Japanese women who underwent a medical examination responded to the questionnaire. RESULTS: The frequency of recurrent miscarriage and two or more consecutive RSA were 0.88% and 4.2%, respectively. Women with a history of miscarriages (hazard ratio: 1.596) and RSA (hazard ratio: 3.103) were at a higher risk of their relationships ending as compared with the women without a his

Gene-environment interactions in panic disorder and CO₂ sensitivity: Effects of events occurring early in life.

Heterogeneous life events (LE) precede the onset of-and potentially increase the susceptibility to-panic disorder (PD). It remains unknown whether LE can act as moderators in the context of gene-by-environment interactions (G×E) that alter the susceptibility to PD and the related trait of CO₂ sensitivity, nor it is known whether such moderation may depend on occurrence of events at different epochs in life. In 712 general population twins we analyzed by Maximum Likelihood analyses of ordinal data whether life (major- and stressful) events moderate the genetic risk for PD and CO₂ sensitivity, a

A physician's personal experience as a cancer of the neck patient.

The author presents his personal experiences as a physician who had several surgical procedures to remove pyriform sinus squamous cell carcinoma, which included laryngectomy. He recently published a book that captures the three years of his life that followed his throat cancer diagnosis and tells his story of facing and dealing with medical and surgical treatments and adjusting to life afterwards. The editorial highlights some of the book's features that relate to the important role of head and neck surgeons in the care of patients with cancer.

Sibling agreement on retrospective reports of parental alcoholism and other childhood events.

UNLABELLED: Studies have used siblings to verify subject reports of retrospective data and examined variables influencing subject-sibling agreement, but questions remain. From 1998 to 2000, we examined a community sample of women (N = 143) in a metropolitan area, aged 21-60, balanced by race, parental alcoholism, and social class, as well as their siblings, using standardized, self-administered questionnaires and an interview. RESEARCH QUESTIONS: Do subject and sibling reports agree? Do reports vary by subject characteristics, or the type of childhood experience? Descriptive statistics showed

Autobiographical memory for trauma: update on four controversies.

Empirical research since the year 2000 on trauma and autobiographical memory in adults is reviewed and related to four enduring controversies in the field: Whether traumatic memories are inherently different from other types of autobiographical memory; whether memory for trauma is better or worse than memory for non-traumatic events; whether traumas can be forgotten and then recalled later in life; and whether special mechanisms such as repression or dissociation are required to account for any such forgetting. The review concludes that trauma and non-trauma memories differ substantially, but

Exposure to early life trauma is associated with adult obesity.

Exposure to traumatic events during childhood is associated with an elevated risk of adult obesity. It has been hypothesized that the psychological sequelae from childhood trauma account for this risk, though no study has examined whether an increased risk of obesity is found in persons without psychological disorders. We examined exposure to early life stressors and body mass index (BMI) in 696 adults without significant medical or psychiatric history. Bivariate correlation showed that the total number of early life stressors (r=0.08), age (r=0.19), and sex (r=0.16) were significantly related

Nocturnal panic and recent life events.

Recent research has hypothesized an association between traumatic events and nocturnal panic (NP). The purpose of this study was to investigate whether the onset of nocturnal panic attacks is associated with a higher frequency of and/or greater severity of stressful or traumatic life events than that of patients with panic disorders (PDs) who experience daytime panic attacks (DPs) while awake. A secondary aim was to investigate whether NP is associated with specific life events at the onset of the disorder. Our sample comprised 129 subjects with PD (DSM-IV). We investigated the number and type

Event attributes and the content of psychotic experiences in first-episode psychosis.

BACKGROUND: Previous psychosocial stress research, contemporary cognitive models, and new cognitive behavioural treatments for psychosis converge in suggesting that stressful events influence the content of psychotic experiences. In this paper we test whether the attributes of stressful events preceding the initial onset of psychosis are associated with core themes of the illness. METHOD: Forty-one people who had experienced a first episode of psychosis were assessed on the attributes of stressful events occurring in the year before onset, the themes (persecutory, depressive, and grandiose) a

Circumstances of suicide among individuals with schizophrenia.

Very little is known about the circumstances surrounding suicides in people with schizophrenia. Between September 1989 and August 1998, 15 and 100 suicide victims with and without schizophrenia, respectively, were examined from the Maryland Brain Collection (MBC). Next-of-kin interview and medical record review following death collected demographic and clinical characteristics, family history, psychiatric symptoms, and variables surrounding the suicide. Individuals with schizophrenia exhibited significantly more lifetime depressive symptoms than those without schizophrenia. Jumping from a heig

Previous pain experience in women with fibromyalgia and inflammatory arthritis and nonpainful controls.

OBJECTIVE: To examine the frequency of commonly occurring pain and adverse experiences throughout life by self-report in women with fibromyalgia (FM) and chronic inflammatory arthritis (IA) and nonpainful healthy women. METHODS: Fifty-one patients with FM and 44 with IA and 52 nonpainful healthy controls were consecutively interviewed in a tertiary clinic setting regarding the occurrence of lifetime common pain experience and adverse events, as well as a family history of FM and/or a childhood pain environment. RESULTS: Patients with FM reported significantly more irritable bowel syndrome, m

Psychological perspective on human developmental stability and fluctuating asymmetry: sources, applications and implications.

In recent years researchers from diverse disciplines in the life sciences have turned their attention to the issue of developmental stability in the organism. Their key measure, fluctuating asymmetry (FA), is defined as the random, stress-induced deviations from perfect symmetry that develop during the development of bilaterally symmetrical traits. This is regarded as a promising measure of the stress experienced by individuals during their development, as well as the interaction between genetic and environmental forces which affect that development. FA also sheds light on several domains with

Hypnosis in a case of long-standing idiopathic itch.

OBJECTIVE: This article presents the results of a brief hypnosis treatment of a woman with chronic, idiopathic vaginal and anal itch. METHODS: The patient was referred after 3 years of unsuccessful outcomes with standard topical and oral treatments prescribed by her family physician and three dermatologists. Treatment consisted of five sessions of self-hypnosis training in techniques of relaxation, deepening, and imagery, and home practice with an individualized instructional tape. RESULTS: After treatment, the patient reported substantial tissue healing, confirmed by her treating physician,

Effects of ruminative and distracting responses to depressed mood on retrieval of autobiographical memories.

Four studies explored the effects of self-focused rumination vs. distraction on dysphoric and nondysphoric students' retrieval of autobiographical memories. Dysphorics induced to ruminate subsequently recalled more negatively biased autobiographical memories in free recall (Study 1) and in response to prompts for memories (Study 2) than either dysphorics who first distracted themselves from their mood or nondysphoric controls. In Study 3, dysphoric rumination led students to recall negative events as occurring relatively frequently in their lives and positive events as occurring relatively inf

The trait and process of resilience.

Resilience is the ability of people to 'spring back' in the face of adversity. It is an important concept for nurses as we endeavour to assist individuals to meet the challenges of living with illness and ageing. Researchers from many disciplines in both the social and health sciences have investigated resilience of individuals throughout the life cycle in a variety of situations related to health as well as other life events. Some researchers have investigated resilience as a trait of individuals while other view resilience as a process. This article reviews the current literature on resilien

Resilience in children's adaptation to negative life events and stressed environments.

Functional adequacy (the maintenance of competent functioning despite an interfering emotionality) is a benchmark of resilient behavior under stress. While resilient adults can be identified as adults who once experienced a great deal of despair as children, I am not prepared to mark off the construct of resilience because such people may carry with them a realistic baggage of sadness and unhappiness. The very nature of despair that is present for children of the ghetto, the status of minority children in today's America, all have a reality that can neither be ignored or denied. Perhaps a port

Anxiety and adjustment disorder: a treatment approach.

Adjustment disorder with anxious mood can be difficult to differentiate from other anxiety disorders or personality disorders. Particular care must be taken to distinguish between the reasonable, expected response to psychosocial stressors and the inordinate response that may indicate the diagnosis of adjustment disorder. Once the diagnosis has been established, options for the management of these patients include nonpharmacologic approaches, pharmacotherapy, and combinations of both. The ultimate goals of treatment are to mobilize the patient's stress-coping mechanisms and to prevent the adju

Stress, marital separation and divorce.

Marital separation and divorce are common and major causes of stress in our society. The general practitioner should keep marital disharmony and pending separation in mind when one or both partners present with recurring medical problems that are often minor in nature. It is important for the GP to become involved with caring, support and appropriate counselling of the partners, their children and other family members. Any homicidal or suicidal threats should be taken very seriously.

Escape nightmares and postescape stressful events.

Correlation matrix based on questionnaire item responses by 38 Czechoslovak refugees suggested that "escape nightmares" (recurrent nightmares about being back in the exhomeland, wanting to or trying to re-escape to the free world) are unrelated to postescape incidence of various stressful events (e.g., illness, job difficulties, financial problems). However, refugees who reported a greater number of the stressful events also reported a somewhat higher incidence of nightmares on themes other than escape from homeland (r = .34).
