It is a marathon rather than a sprint: an initial exploration of unmet needs and support preferences of caregivers of children with SCI.

STUDY DESIGN: Focus group study. OBJECTIVES: Explore unmet needs and support preferences of caregivers of youth with spinal cord injury (SCI). SETTING: One pediatric specialty rehabilitation hospital system in the United States. METHODS: Four focus groups were conducted with a convenience sample of 26 caregivers who were primarily mothers (96%) and married (65%), and had at least some college education (85%). Children living with SCI were on average 12.8 years old (SD = 3.3, 8-18). The average age of injury was 4.7 years (SD = 4.4, 0-16.2); mean injury duration was 8.2 years (SD = 3.9, 2-16

Adverse early life experience and social stress during adulthood interact to increase serotonin transporter mRNA expression.

Anxiety disorders, depression and animal models of vulnerability to a depression-like syndrome have been associated with dysregulation of serotonergic systems in the brain. To evaluate the effects of early life experience, adverse experiences during adulthood, and potential interactions between these factors on serotonin transporter (slc6a4) mRNA expression, we investigated in rats the effects of maternal separation (180 min/day from days 2 to 14 of life; MS180), neonatal handing (15 min/day from days 2 to 14 of life; MS15), or normal animal facility rearing (AFR) control conditions with or wi

Pain management in sickle cell disease: palliative care begins at birth?

People with sickle cell disease (SCD) are living longer, but their lives are impacted even more by the unpredictable intermittent or constant pain that is often poorly managed over a lifetime. To address this problem, an interdisciplinary team approach is needed that brings the medical professionals together for optimal compassionate care that is coordinated from the beginning of life and throughout the patient's lifespan. The hematologist, whenever possible, should take the lead. Effective models that have been developed to address SCD pain, such as the Day Hospital, The Recidivism Project and

Life satisfaction in later life: the role of health, social support, and social exclusion in Türkiye.

BACKGROUND: Examining the factors affecting life satisfaction among older adults in Türkiye is crucial for both scientific inquiry and social policy development. Population ageing in Türkiye has accelerated in recent years, and the increasing proportion of individuals aged 65 and over has begun to significantly reshape the demographic structure. Life satisfaction is a fundamental public health indicator that predicts disease development, psychosocial health status, mortality risk, and healthcare utilisation. Dynamics unique to Türkiye, such as the transformation of family structures and the gr

Pregnancy as a journey of care and connection: Indigenous women's experiences navigating health systems and community support.

INTRODUCTION: Pregnancy and childbirth are opportunities for Indigenous communities to welcome new life, integrate meaningful cultural practices, and tangibly strengthen communal wellbeing. Simultaneously, pregnant American Indian and Alaska Native (AIAN) women must navigate complex healthcare systems while experiencing structural inequities that contribute to elevated risks of complications and poor birth outcomes. This study explored the pregnancy experiences of AIAN women, examining how participants navigated healthcare systems, social supports, and structural challenges. METHODS: We condu

Evaluating Acute Polytrauma Rehabilitation: A Literature Review With Insights From the UK Major Trauma Network.

Major trauma is a leading cause of disability in young adults, and modern trauma systems increasingly recognise that survival alone is insufficient; functional recovery and quality of life are essential outcomes. This review evaluates the benefits, challenges, and future development needs of integrating acute polytrauma rehabilitation (APR) into trauma systems, with reference to London's Major Trauma System. It synthesises contemporary evidence from systematic reviews, clinical audits, the European and UK trauma guidelines, and observational studies addressing APR timing, delivery, outcomes, a

Family Self-Support in Managing Down Syndrome Children: A Qualitative Study.

Background and Aim. Down syndrome (DS) is the most common reason for disabilities caused by genetic disorders. Due to the special nature of this disease and the special needs of children with Down syndrome, they are required to receive their families' support. Therefore, the recognition of their problems and needs and also the alternatives for resolving them and promoting their life quality are very useful. Also, since very limited qualitative studies have been conducted, it seems necessary to design a qualitative study. Method. This qualitative study was conducted by the content analysis meth

Evolving power grids with self-organized intermittent strain releases: An analogy with sandpile models and earthquakes.

The stability of powergrid is crucial since its disruption affects systems ranging from street lightings to hospital life-support systems. While short-term dynamics of single-event cascading failures have been extensively studied, less is understood on the long-term evolution and self-organization of powergrids. In this paper, we introduce a simple model of evolving powergrid and establish its connection with the sandpile model and earthquakes, i.e., self-organized systems with intermittent strain releases. Various aspects during its self-organization are examined, including blackout magnitude

Post-diagnostic support in Australia: Perspectives of people recently diagnosed with dementia and their carers.

Introduction: Timely post-diagnostic support is necessary to help people with dementia and those who provide care adapt to the diagnosis and optimise independence and quality of life. However, evidence from people with dementia and carers regarding the support they need is scarce. Aim: To explore the perspectives of people with dementia and their carers regarding the support they had been offered, barriers and facilitators to accessing such support, and support they viewed as desirable or ideal. Method: This was a cross-sectional qualitative exploratory study. Data were collected from individu

Implementation of electronic patient-reported outcomes in supportive care for oncology patients.

PURPOSE OF REVIEW: Patient-reported outcomes (PROs) have become increasingly important in oncology, capturing the patient perspective on symptoms, treatment effects, and health-related quality of life. Transitioning to electronic platforms (ePROs) enables real-time monitoring, efficient integration into clinical workflows, and enhanced patient-centered care. This review explores recent evidence on the clinical utility, barriers, and future directions of integrating ePROs in supportive care in oncology. RECENT FINDINGS: ePROs improve symptom control, quality of life, engagement, and in some st

[Current perspectives on supportive care for lung cancer patients].

The fight against cancer comprises not only survival of the disease but also survival with the highest possible quality of life. Thus, supportive care in cancer aims at reducing physical and psycho-emotional symptom burden. Furthermore, supportive care in cancer includes self-management-support for patients and their families/caregivers. Due to high symptom prevalence and poor prognosis, lung cancer patients express more unmet supportive care needs than other patient populations with cancer. Interventions to meet these needs have been developed in the last decade. They involve new models of ca

Extracorporeal Membrane Oxygenation: The New Jack-of-All-Trades?

The role of extracorporeal membrane oxygenation in the adult patient population still is evolving. Technologic advancements have improved the ability to provide extracorporeal life support. The miniaturization and durability of these systems have made extracorporeal membrane oxygenation more convenient and mobile than ever. Because of these improvements, its use has increased steadily. The indications for use also have diversified. In this review, the authors provide a panoramic view of extracorporeal membrane oxygenation to provide a foundation of knowledge for anesthesiologists.

[Specific surgical strategy for gangrene of both lower extremities].

At admission of patients with gangrenes of both lower extremities the surgical strategy was not determined. The author has also failed to find corresponding recommendations in the available literature. On the basis of his clinical experiences with 12 patients who underwent big amputation of lower extremities for bilateral gangrene, the author proposes the surgical methods. The optimal variant is considered to be a by turns amputation with the interamputation period being determined by a relative stabilization of the life-support systems.

Evaluating the implementation of Electronic Palliative Care Co-ordination Systems (EPaCCS) and optimising future service provision for palliative and end-of-life care: a synopsis of the Optimal Care mixed-method study.

BACKGROUND: Electronic Palliative Care Co-ordination Systems are central to end-of-life care policy despite critical gaps in the evidence base underpinning their use. It is unclear how Electronic Palliative Care Co-ordination Systems are being implemented across England and there is a lack of research to understand their use in routine care. AIM: To understand how Electronic Palliative Care Co-ordination Systems are currently being used in routine care and to guide the development of interventions to support their optimal implementation and maximise patient benefit. METHOD: A mixed-method ap

Improving the clinical value and utility of CGM systems: issues and recommendations : A joint statement of the European Association for the Study of Diabetes and the American Diabetes Association Diabetes Technology Working Group.

The first systems for continuous glucose monitoring (CGM) became available over 15 years ago. Many then believed CGM would revolutionise the use of intensive insulin therapy in diabetes; however, progress towards that vision has been gradual. Although increasing, the proportion of individuals using CGM rather than conventional systems for self-monitoring of blood glucose on a daily basis is still low in most parts of the world. Barriers to uptake include cost, measurement reliability (particularly with earlier-generation systems), human factors issues, lack of a standardised format for display

Bridging the gap in pneumonia prevention: Qualitative insights on vaccine implementation from health leaders in middle-income countries.

Despite the well-documented life-saving potential of pneumococcal conjugate vaccines (PCV) and global efforts to widen vaccine availability, access to PCV in middle-income countries (MICs) has remained suboptimal due, in part, to vaccine pricing and limited external funding opportunities. To understand gaps and opportunities for improving vaccine equality, this qualitative study engaged leaders from governments, medical institutions, and health agencies who were based in MICs that did not currently have PCV in their national immunization programs to explore their perspectives on decision-makin
